P28.09: Accuracy of ultrasound for the preoperative evaluation of deep infiltrating endometriosis

Abstract

Of the 10% of women affected by endometriosis 15% have deep infiltrating endometriosis (DIE). We determined whether our ultrasound department is as accurate as suggested in the literature in diagnosing DIE. Women with a history suggestive of endometriosis or infertility underwent a focussed ultrasound for DIE. The ultrasound was performed by sonologists trained in the comprehensive technique to diagnose DIE. Most women had bowel preparation prior to the ultrasound. Women with findings suggestive of DIE were referred to colorectal colleagues for pre-operative planning. Scan findings were correlated with intra-operative findings and histopathology. We have scanned 88 women suspected to have DIE on history or examination. 28 (32%) of these women have had surgery. We have reported 100% sensitivity and specificity for the diagnosis of DIE confirmed with histopathology. By disease site we have > 95% sensitivity for endometriosis affecting the ovaries, retrocervix and Pouch of Douglas. Our disease site of lowest sensitivity were the vaginal vault (50%) and uterovesical pouch (12.5%). Of the 3 ultrasounds that reported no evidence of DIE and have had surgery, 2 had no endometriosis and 1 had mild pelvic side wall disease.
