Materials and Methods: A total of 108 formalin-fixed paraffin-embedded tissue samples were included, comprising 36 OSCC, 36 OPMDs (OSMF, OED, OLP), and 36 normal oral mucosa. Immunohistochemical analysis was performed using primary antibodies against MUC20 and SPRR1B. Expression levels were scored based on intensity and proportion of positively stained cells. Statistical analysis was conducted using SPSS v23, with significance set at p < 0.05. Results MUC20 and SPRR1B expression was significantly higher in OSCC compared to OPMDs and normal mucosa (p < 0.001). In OPMDs, MUC20 expression was highest in mild dysplasia, while SPRR1B expression increased progressively with dysplasia severity. In OSCC, higher MUC20 and SPRR1B levels were associated with poorer histological differentiation. No significant correlation was observed between expression and OSCC clinical stage. Conclusion Both MUC20 and SPRR1B show progressive upregulation from normal mucosa to OPMDs and OSCC, with expression correlating to histopathological grade and dysplasia severity. These findings suggest their potential utility as biomarkers for early detection and prognostic evaluation in oral carcinogenesis.
