Although there are numerous and sometimes partially contradictory perioperative transfusion guidelines, implementation of perioperative transfusion guidelines is sometimes difficult due to local differences in clinicians' knowledge and experience and characteristics of surgery. The aim of this study was to evaluate perioperative transfusion practices in patients undergoing elective major surgery. Methods : A cross-sectional study conducted in Damascus Hospital (Al-Mujtahid) in the period between November 2023 and February 2024. The study included patients of the following departments: general surgery, orthopedic surgery, neurosurgery, and vascular surgery. Patients who underwent selective surgeries before and after surgery were studied, and their number reached 130 patients. Results : Our study included 148 patients; the percentage of males (60.1%) compared to females (39.9%). The largest age group was “45-69” (43.2%). The most common blood group is O+ (24.3%). More than half of the patients were not taking any medications (55.4%). Medications taken: anti-coagulant (7.4%), blood pressure and diabetes medications (2.7%). Medical history was: diabetes mellitus (8.1), diabetes and hypertension (6.1%). The type of surgery was as follows: orthopedic and general surgery (33.8%) each, neurosurgery (20.3%), and vascular surgery (12.2%). Blood transfusion was more frequent postoperatively (23%), and a higher percentage did not undergo a blood transfusion (27%). Whole blood transfusion was the most widely used strategy, especially in orthopedic surgeries. Conclusion : We found that approximately 70% of patients who underwent major elective surgery underwent blood transfusions, to a lesser extent, to one or more of its derivatives, and the largest percentage, 23%, underwent blood transfusions postoperatively. This review updates the current understanding of the physiological rationale behind perioperative transfusion, available blood components and their indications, the techniques to assess and reduce blood loss, the role of point-of-care (POC) testing in managing transfusion and the complications of transfusion. Though numerical transfusion triggers are helpful in guiding transfusion, one should not miss the overall clinical picture.[ 1 ] Restrictive transfusion strategy is now widely accepted.[ 1 ] Employing physiological criteria like patient hemodynamics, the response to immediate volume resuscitation and signs of clinical coagulopathy is very important. Correlation between the laboratory abnormality and clinical coagulopathy could be unpredictable and inconsistent. Transfusion could have several repercussions as exemplified by a higher incidence of cancer recurrence in nonsurgical patients who received blood transfusions possibly due to transfusion-related immune modulation. [ 2 ] Aim of the study: Evaluate perioperative blood transfusion practices in patients undergoing elective major surgery. The patient files were studied, blood tests in particular were reviewed, and data that served the purpose of the study was obtained. The approval of the Syrian Private University and the approval of Damascus Hospital (Al-Mujtahid) were obtained before starting the study. Statistical analysis: Patient data was tabulated and entered into the computer into the Excel program, and then the Statistical Package for the Social Sciences (SPSS) version (26) was used to analyze this data, and the following statistical methods were relied upon in the analysis: Descriptive statistics: It consists of finding the relative frequency distributions of the categorical study variables. Inferential statistics: This part of the analysis aims to present and interpret the results and infer them in order to reach the goal of the study, by conducting a Chi-Square test of independence to study whether a relationship exists between two descriptive variables by applying the Chi-Square statistic. Results Our study included 148 male and female patients; the percentage of males was 89 (60.1%) compared to females 59 (39.9%) (Table 1 ). The largest age group was “45–69” with a percentage of 64 (43.2%), followed by “18–44” with a percentage of 39 (26.4%). The tables provide more information. (Table 3 ) shows the blood types of the patients, where the largest percentage was the “O+” group at 36 (24.3%), followed by the “A+” group at 33 (22.3%). Regarding blood transfusion, (Table 8 ) shows that blood transfusion was done after surgery in a percentage of 34 (23%), during surgery in 26 (17.6%), and before surgery in 25 (16.9%), and there was a percentage who did not undergo a blood transfusion in a percentage of 40. In our study, the practice of blood transfusion was evaluated during the three perioperative periods, because the decision to transfuse is made by different doctors with different indications at each stage. In general, anesthesiologists are the primary decision makers during surgery, while surgeons make decisions regarding blood transfusion before and after surgery. We documented a percentage of blood transfusions in the perioperative period, and found that the highest percentage of blood transfusions occurred postoperatively, in contrast to what a large Turkish study found in 2020, which found that the highest percentage of blood transfusions occurred during surgical operations; Preoperative and postoperative blood transfusions constitute an approximately equal proportion of perioperative blood transfusions. [ 3 ] According to our results, whole blood transfusions were performed postoperatively by the hemoglobin threshold and the average pre-transfusion hemoglobin value was 8.7 g/dL. Current guidelines indicate that RBC transfusion is recommended when the hemoglobin concentration is 10 g/dL. /dL, and a target hemoglobin concentration of 7–9 g/dL is recommended during active bleeding. Adapting to lower hemoglobin thresholds for transfusion reduces the use of red blood cells. However, the indications for intraoperative RBC transfusion have not been clearly defined, and the hemoglobin concentration range of 7–10 c/dL, in particular, is left to the discretion of clinicians. This study included patients undergoing major surgery, and therefore, the case mix of this sample can be considered representative of the surgical population. We documented data for both transfusion recipients and nonrecipients. A limitation is that postoperative blood transfusion was only observed on the first day after surgery; Some patients in the study group may have received blood transfusions in the following days, which could have affected the results. Finally, post-transfusion hemoglobin concentration was not recorded after each transfusion, so the intended purpose of the transfusion cannot be determined. Conclusion We found that approximately 70% of patients who underwent major elective surgery underwent blood transfusions, to a lesser extent, to one or more of its derivatives, and the largest percentage, 23%, underwent blood transfusions postoperatively. The study included patients from the following departments: general surgery, orthopedic surgery, neurosurgery, and vascular surgery. Patients who underwent selective surgeries before and after surgery were studied, and their number reached 130 patients. The patient files were studied, blood tests in particular were reviewed, and data that served the purpose of the study was obtained. The aim of this study was to evaluate perioperative transfusion practices in patients undergoing elective major surgery.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods\u003c/strong\u003e: A cross-sectional study conducted in Damascus Hospital (Al-Mujtahid) in the period between November 2023 and February 2024. The study included patients of the following departments: general surgery, orthopedic surgery, neurosurgery, and vascular surgery. Patients who underwent selective surgeries before and after surgery were studied, and their number reached 130 patients.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e: Our study included 148 patients; the percentage of males (60.1%) compared to females (39.9%). The largest age group was “45-69” (43.2%). The most common blood group is O+ (24.3%). More than half of the patients were not taking any medications (55.4%). Medications taken: anti-coagulant (7.4%), blood pressure and diabetes medications (2.7%). Medical history was: diabetes mellitus (8.1), diabetes and hypertension (6.1%). The type of surgery was as follows: orthopedic and general surgery (33.8%) each, neurosurgery (20.3%), and vascular surgery (12.2%). Blood transfusion was more frequent postoperatively (23%), and a higher percentage did not undergo a blood transfusion (27%).
