This prospective study examined women undergoing surgical treatment for deep infiltrating endometriosis (DIE) in Southern Brazil from January 2010 to January 2012, analyzing the number, anatomical location, and uni- versus multifocality of DIE lesions using eight main site categories and statistical testing with Statistica 8.0. Among 143 women with 577 lesions, the most frequent locations were the uterosacral ligament (41.4%), retrocervical area (15.7%), intestinal involvement (14.4% total; predominantly rectosigmoid), and the vagina/round ligament (each 8.7%), with ovarian endometrioma present in 39.9% and intestinal infiltration on histology in 45.4%. The study found that multifocal disease predominated (p<0.0001) with a mean of 4 DIE lesions per patient.
