Abstract

Pakistan continues to face recurrent, increasingly severe dengue outbreaks, and 2025 has again exposed deep systemic failures rather than an inevitable seasonal pattern. Despite national and provincial policies promoting integrated vector management and climate-informed preparedness, implementation remains fragmented, reactive, and heavily reliant on short-term fogging. Early-warning alerts issued after heavy rains and favourable temperature--humidity conditions did not trigger timely action, while monsoon flooding, urban infrastructure deficits, and unmanaged waste created extensive Aedes breeding environments. Overwhelmed hospitals, patchy surveillance, limited entomological capacity, and inconsistent community engagement further amplified transmission. This short communication argues that sustainable dengue control in Pakistan now depends on operationalising district-level integrated vector management, linking climate alerts to pre-agreed response triggers, strengthening health-system surge planning, investing in entomology and data integration, and addressing structural urban vulnerabilities. Without cross-sectoral accountability and consistent implementation, dengue will remain an annual indictment of broader public-health governance.
