EP29.06: Multicystic benign peritoneal mesothelioma: a case report

We report a 39-year-old lady presenting with a history of primary infertility for the past 9 years. There was prior history of metroplasty and ovarian cystectomy for endometriosis There was no history of fever, weight loss or abdominal pain. She was referred for a routine pelvic scan for the workup of infertility. The transvaginal scan showed a multicystic mass composed of a cluster of small cysts filling the entire pouch of Douglas and surrounding the uterus and ovaries measuring approximately 10.0×7.9 cm. Power Doppler showed minimal flow in the intervening septa of the mass. There was no ascites. The right ovary was normal and there was an endometrioma in the left ovary. Uterus was normal. Tumour markers were normal. She underwent cytoreductive surgery with peritonectomy. A soft yellowish grape like mass with multiple cysts seen on laparoscopy was resected. These cysts showed clear fluid. Histopathology showed cysts lined by a single layer of flattened to cuboidal mesothelioma cells without atypia or tissue invasion. No mitotic activity seen. All tumour cells showed immunoreactivity for calretinin and cytokeratin confirming the mesothelial origin of the mas The final diagnosis was benign multicystic peritoneal mesothelioma.
