Materials and Methods A total of 65 patients (mean age: 67.7 ± 14.4) who underwent mechanical thrombectomy at Gaziantep City Hospital were included. Arterial blood gas parameters (pH, SaO₂, PaCO₂, PaO₂, lactate, HCO₃) were recorded before thrombectomy and at the 1st, 3rd, and 5th hours post-thrombectomy. Clinical outcomes were assessed using pre- and post-treatment NIHSS scores, ASPECT scores, modified Rankin Scale (mRS) scores, and the presence of hemorrhage based on ECASS-II criteria. Results No significant correlation was found between arterial blood gas parameters and clinical outcome scores (NIHSS and 3-month mRS). However, an increase in preprocedural pH was significantly associated with higher post-procedural ASPECT scores (β = 0.737, p = 0.017), while elevated preprocedural lactate levels were significantly associated with lower post-ASPECT scores (β = − 2.326, p < 0.001). The most significant predictors of poor clinical outcome (3-month mRS ≥ 3) were higher baseline NIHSS (p = 0.019) and lower post-ASPECT scores (p = 0.019). Temporal changes in ABG parameters showed no significant effect on mRS scores. Conclusion Although certain pre-thrombectomy ABG parameters (particularly pH and lactate) are associated with neuroimaging findings, their independent predictive value for clinical outcomes appears limited. ABG monitoring may contribute to prognostic evaluation, yet its direct impact on functional outcome (mRS) is weak. ischemic stroke anterior circulation thrombectomy arterial blood gas İNTRODUCTİON Acute ischemic stroke (AIS) accounts for approximately 85% of all stroke cases and is characterized by the sudden onset of neurological deficits, requiring urgent intervention [ 1 ]. One of the most common underlying mechanisms in these patients is large vessel occlusion in the anterior circulation. In such cases, intravenous thrombolysis often proves insufficient, making mechanical thrombectomy (MT) the preferred treatment modality [ 2 , 3 ]. Successful early recanalization through MT can significantly improve functional outcomes by limiting the expansion of the infarct core [ 4 ]. However, despite achieving vascular reperfusion, not all patients experience clinical improvement. This phenomenon, referred to as "futile recanalization," may manifest as neurological deterioration, hemorrhagic transformation, or persistent functional dependency [ 5 ]. Therefore, identifying early prognostic biomarkers has gained increasing importance. Arterial blood gas (ABG) analysis is an invasive yet easily applicable tool that reflects the patient’s metabolic and respiratory status. Key ABG parameters including pH, oxygen saturation (SaO₂), partial pressures of oxygen (PaO₂) and carbon dioxide (PaCO₂), lactate, and bicarbonate (HCO₃) provide rapid information about tissue perfusion, hypoxia, acidosis, and metabolic dysfunction [ 6 ]. In critically ill patients, disturbances in these parameters have been linked to increased morbidity and mortality [ 6 , 7 ]. However, data on the prognostic significance of ABG alterations in stroke patients remain limited. In particular, the association between ABG values—measured before and after thrombectomy—and clinical scoring systems such as NIHSS, mRS, and ASPECT, or hemorrhagic complications, has yet to be clearly elucidated. Preliminary findings suggest that elevated lactate may be associated with poor clinical outcomes [ 8 ], while fluctuations in pH may reflect reperfusion injury and cerebral edema [ 9 , 10 ]. In this study, we aimed to investigate the temporal changes in arterial blood gas parameters at multiple time points before and after mechanical thrombectomy in patients with acute ischemic stroke, and to assess the relationship of these changes with clinical outcomes including early and late neurological recovery and intracranial hemorrhage. MATERİALS AND METHODS This study included 65 patients diagnosed with acute ischemic stroke due to anterior circulation large vessel occlusion who underwent mechanical thrombectomy and were followed in the Neurology Intensive Care Unit of Gaziantep City Hospital. Inclusion criteria: Age ≥18 years Diagnosis of acute ischemic stroke due to internal carotid artery (ICA) occlusion, middle cerebral artery (MCA) M1 occlusion, or tandem occlusion Baseline Alberta Stroke Program Early CT Score (ASPECTS) >6 Successful reperfusion after mechanical thrombectomy with a Thrombolysis in Cerebral Infarction (TICI) score of 2b or 3 Exclusion criteria: Presence of intracranial hemorrhage prior to thrombectomy Missing arterial blood gas data Chronic obstructive pulmonary disease (COPD), obstructive sleep apnea, or other active/chronic obstructive pulmonary conditions Acute respiratory distress syndrome (ARDS) Decompensated congestive heart failure Severe hepatic or renal dysfunction Demographic data (age, sex) and comorbidities were retrieved from patient medical records. Clinical assessments included pre- and 24-hour post-thrombectomy NIH Stroke Scale (NIHSS) scores, modified Rankin Scale (mRS) scores at baseline, 1 week post-procedure, and at 3-month follow-up, as well as ASPECT scores before and after thrombectomy. ASPECT score was evaluated by 2 different neurologists and there was agreement between the evaluators. Reperfusion success was evaluated using the TICI score. Arterial blood gas (ABG) parameters including pH, oxygen saturation (SaO₂), partial pressure of carbon dioxide (PaCO₂), partial pressure of oxygen (PaO₂), lactate, and bicarbonate (HCO₃) were measured at four time points: Before mechanical thrombectomy 1 hour after recanalization 3 hours after thrombectomy 5 hours after thrombectomy Post-thrombectomy hemorrhagic transformation was evaluated using the European Cooperative Acute Stroke Study II (ECASS-II) classification [11], and categorized into the following subtypes: Hemorrhagic infarction (HI): HI-1: Small petechiae along the margins of the infarct HI-2: More confluent petechiae within the infarcted area without mass effect Parenchymal hematoma (PH): PH-1: Hemorrhage involving 30% of the infarcted area with significant mass effect Statistical Analysis All statistical analyses were performed using IBM SPSS Statistics version 23 and the MASS package in R software. The normality of data distribution was assessed using the Shapiro–Wilk and Kolmogorov–Smirnov tests. Fisher’s Exact Test was used to evaluate associations between categorical variables. For comparison of two independent groups, the Independent Samples t-test was used for normally distributed variables, while the Mann–Whitney U test was applied for non-normally distributed variables.Pearson’s correlation coefficient was employed to assess relationships between normally distributed continuous variables, while Spearman’s rho was used for non-normally distributed variables. Robust regression analysis was applied to evaluate the effect of independent variables on non-normally distributed dependent variables. Binary logistic regression analysis was used to identify risk factors associated with poor clinical outcome and the presence of hemorrhage. Results were presented as frequency (percentage) for categorical variables and as mean ± standard deviation or median (minimum–maximum) for continuous variables. A p-value of <0.050 was considered statistically significant. The variables affecting the ASPECT (post-procedure) score were evaluated with robust regression analysis and the model was found to be significant (F = 3.722; p < 0.001). The variables explained 49.18% of the variance of the score. The increase in the pH value before thrombectomy significantly increased the ASPECT score (β = 0.737; p = 0.017), while the increase in the lactate level decreased the score (β=–2.326; p 0.05). In the analyses performed to evaluate chronic period clinical outcome with independent samples t test and Mann Whitney U test, it was found that advanced age, high NIHSS before and after the procedure and low ASPECT score after the procedure were significantly associated with poor clinical outcome, while blood gas parameters were not significantly associated with good or poor clinical outcome in the chronic period (Table 2 ). Multivariate modeling to determine the factors affecting mRS in the chronic period was significant, and high pre-procedure NIHSS and post-procedure hemorrhagic transformation were found to be associated with poor prognosis. However, blood gas parameters did not show a significant effect in this model. Again, in the same modeling, having a chronic disease was found to be 3.55 times more associated with poor prognosis. DİSCUSSİON İn this study, we evaluated the impact of arterial blood gas (ABG) parameters on clinical outcomes in patients who underwent mechanical thrombectomy (MT) due to anterior circulation large vessel occlusion. Notably, pre-procedural pH and lactate levels were found to be associated with the post-intervention ASPECT score, which reflects brain tissue reperfusion. An increase in pre-MT pH was significantly associated with a higher ASPECT score (β = 0.737; p = 0.017), while elevated lactate levels were associated with a decrease in ASPECT score (β = − 2.326; p < 0.001). These findings support previous reports highlighting the influence of systemic metabolic balance and tissue perfusion on ischemic brain tissue [ 6 ]. The post-intervention ASPECT score emerged as a strong predictor of functional outcome; a lower ASPECT score was associated with a 2.65-fold increased risk of poor outcome on the modified Rankin Scale (mRS) at 3 months (OR = 0.377; p = 0.019). This aligns with earlier studies suggesting that brain tissue integrity and successful reperfusion during the acute phase are crucial for long-term functional recovery [ 12 ]. Other independent predictors of poor outcome included a higher baseline NIHSS score (OR = 1.32; p = 0.019) and a lower post-MT ASPECT score. These findings are consistent with prior multicenter trials emphasizing the prognostic significance of initial stroke severity and infarct volume [ 3 , 12 ]. Several studies have proposed that increasing oxygenation before recanalization may help protect the ischemic penumbra, reduce blood–brain barrier damage, and limit infarct volume [ 6 , 7 ]. Conversely, hyperoxia during the post-recanalization period has been associated with increased oxidative stress and reperfusion injury [ 8 ]. While previous observational studies have linked elevated lactate levels or decreased pH to unfavorable outcomes in stroke [ 9 , 10 ], prospective studies evaluating serial ABG monitoring in patients undergoing MT remain limited. The association of these physiological changes with clinical scores (NIHSS, mRS, ASPECT) or complications (e.g., symptomatic intracranial hemorrhage) has yet to be clearly established. In our study, no significant correlation was found between ABG parameters measured at 1, 3, and 5 hours after MT and the 3-month mRS scores. This suggests that functional outcomes may depend not only on metabolic fluctuations but also on ischemic territory dynamics, collateral circulation, and reperfusion quality [ 6 , 7 ]. Similarly, no clinical or laboratory parameters were found to significantly predict post-MT hemorrhagic transformation. In our study, preprocedural arterial pH and lactate levels were found to be significantly associated with post-thrombectomy ASPECT scores, reflecting the extent of cerebral reperfusion. This is consistent with prior findings suggesting that metabolic derangements may influence infarct evolution and tissue viability. Brouns et al. demonstrated that elevated lactate levels in the cerebrospinal fluid were strongly associated with poor neurological outcomes and higher mortality in acute ischemic stroke, indicating a central role of metabolic stress in stroke progression [ 13 ]. Similarly, Matteucci et al. reported that arterial blood gas alterations, particularly in pH and lactate, correlated with post-thrombectomy complications and functional outcomes, emphasizing the interplay between systemic metabolic status and cerebral reperfusion injury [ 14 ]. These findings support the notion that metabolic markers may serve as adjunctive prognostic tools in acute stroke, beyond conventional imaging and clinical scores. Conclusion Metabolic indicators such as pH and lactate may be important markers reflecting the success of cerebral reperfusion following mechanical thrombectomy. Initial clinical presentation and imaging scores (NIHSS, ASPECT) are the strongest predictors of long-term functional outcomes. Short-term arterial blood gas monitoring may indirectly support prognostic evaluation by reflecting systemic metabolic status. Limitations of the Study This study has a retrospective design and is limited to data from a single center. Arterial blood gas parameters (pH, SaO₂, PaCO₂, PaO₂, lactate, HCO₃) were recorded before thrombectomy and at the 1st, 3rd, and 5th hours post-thrombectomy. Clinical outcomes were assessed using pre- and post-treatment NIHSS scores, ASPECT scores, modified Rankin Scale (mRS) scores, and the presence of hemorrhage based on ECASS-II criteria.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e\u003cp\u003eNo significant correlation was found between arterial blood gas parameters and clinical outcome scores (NIHSS and 3-month mRS). However, an increase in preprocedural pH was significantly associated with higher post-procedural ASPECT scores (β\u0026thinsp;=\u0026thinsp;0.737, p\u0026thinsp;=\u0026thinsp;0.017), while elevated preprocedural lactate levels were significantly associated with lower post-ASPECT scores (β = \u0026minus;\u0026thinsp;2.326, p\u0026thinsp;\u0026lt;\u0026thinsp;0.001). The most significant predictors of poor clinical outcome (3-month mRS\u0026thinsp;\u0026ge;\u0026thinsp;3) were higher baseline NIHSS (p\u0026thinsp;=\u0026thinsp;0.019) and lower post-ASPECT scores (p\u0026thinsp;=\u0026thinsp;0.019).
