On the relationship between fetal growth parameters and heart rate variability

Abstract

Abstract Background: Fetal growth restriction (FGR) is a challenging pathology causing short and long-term complications. Autonomic malfunction and delayed neurological maturation were found in growth-retarded fetuses. Fetal heart rate variability (HRV) is critically dependent on autonomic regulation. The study was focused on the detection of the relationship between neonatal biometry parameters and HRV variables. Methods: This descriptive cross-sectional study was performed among 40 pregnant women at 22-36 weeks of gestation. FGR was detected in 9 women. The fetal growth was appropriate in 31 patients. The fetal were obtained from an RR-interval time series registered from the maternal abdominal wall via fetal non-invasive electrocardiography (NI-ECG). The stress index (SI) was selected for evaluation among all linear HRV variables. The following conventional cardiotocographic parameters were determined: short-term variation (STV) and long-term variation (LTV). The variables of AC (acceleration capacity) and DC (deceleration capacity) were also detected. Results: The study's findings showed that the detected variables in the FGR were different. However, the small number of cases did not allow statistical significance to proceed. A strong linear correlation was detected between all checked variables of HRV: AC, DC, SI, STV, and LTV. AC and DC demonstrated a significant correlation between them and Apgar score. The results of the multivariate regression model with SI showed a statistically significant relationship with a term of birth and a tendency to have a statistically significant relationship with body weight and head circumference. This suggests that this temporal HRV index may be associated with the anthropometric parameters of newborns. The receiver operating characteristic analysis could be the next stage for the investigation of efficient diagnostic algorithms based on fetal NI-ECG. Conclusions: SI could have utility as a screening program based on fetal NI-ECG in case of a lack of access to high-quality ultrasound or be a valuable addition to existing perinatal strategies.

The fluctuations of fetal hemodynamics known as heart rate variability (HRV) reflect the fetal ability to support nutrition and well-being. The HRV level is critically dependent on autonomic regulation [1]. The autonomic malfunction was found in FGR and reflected the delayed neurological maturation in growth-retarded fetuses [2]. Ultrasonography is known as a gold standard of FGR diagnosing [3]. However, several studies found that HRV variables were the possible predictors of FGR [4, 5, 6]. Fetal non-invasive electrocardiography (NI-ECG) is a prospective technique for obtaining cardiac signals from the maternal abdominal wall. There are some challenging issues associated with low signal-to-noise ratio [7]. The systems for remote monitoring of fetal HRV via NI-ECG could be a good option for low-resource settings [8]. It is appropriate during wartime [9]. Fetal NI-ECG captures primary electrophysiological processes in the heart. The variations in the durations of cardiac cycles reflect the continual changes in sympathovagal balance. Nowadays, both linear and non-linear methods are used to proceed with HRV. The phase rectified signal averaging – acceleration capacity and deceleration capacity (AC/DC) are useful in fetal well-being assessment [10]. METHODS The study was focused on the detection of the relationship between neonatal biometry parameters and HRV variables. This descriptive cross-sectional study was performed among pregnant women who were admitted to Kharkiv Municipal Perinatal Center between 1 April 2024 and 30 June 2024. The data were obtained from the hospital automation system. Ethics approval was received from the Research Council and Ethical Committee of Kharkiv National Medical University, No25.0224p. Informed consent was obtained from all the patients. The eligible participants were informed about the study’s methodology, its aims, objectives, indications, and eventual complications before enrollment. Patients from the department of maternal-fetal medicine were selected randomly. The FGR was diagnosed in case fetal weight parameters were lower than the 10 th percentile according to ultrasound. Inclusion criteria: healthy pregnancy with appropriate fetal growth, FGR. Exclusion criteria: chromosomal abnormalities, multiple pregnancy, preexisting medical disorders like diabetes mellitus, metabolic syndrome, cardiac diseases, renal disease, and thyrotoxicosis. Totally 40 patients at 22-36 weeks of gestation were enrolled. FGR was detected in 9 women. The fetal growth was appropriate in 31 patients. The fetal HRV variables were obtained from an RR-interval time series registered from the maternal abdominal wall via NI-FECG. The equipment “Cardiolab Baby Card” (Scientific Research Center “KhAI-Medica”, Ukraine) was used in the study. The registration was carried out over 30-60 minutes. The stress index was selected for evaluation among all linear HRV variables. SI = AMo (%) / (2×Mo×Var); Var = NNmax – NNmin; AMo (the most frequent value of NN interval or the highest column in the histogram) – the number of NN intervals included in the pocket corresponding to the mode measured in percentages (%). The obtained fetal RR interval time series was transformed into cardiotocographic (CTG) tracing. The following CTG parameters were determined: short-term variation (STV) and long-term variation (LTV). The variables of AC/DC were also detected [11]. The study protocol also included several parameters: term at the investigation of HRV, term at birth, neonatal body weight, body length, head circumference, and Apgar score. The Statistical Package for Social Sciences (SPSS) program (IBM SPSS Statistics for Windows, Version 25.0. Armonk, NY: IBM Corp.) was used for statistical analysis. Results were presented as means and standard deviations for numerical variables, and frequencies and percentages for categorical data. The suitability of numerical variables to normal distribution was evaluated using skewness values and histograms. The Independent sample t-test was used in comparing numerical variables fitting normal distribution. Variables that did not conform to normal distribution were analyzed by the Mann-Whitney U test. The Chi-Square (or Fisher’s exact) test was used for comparing categorical variables. Depending on their distribution, Spearman or Pearson correlation analysis was used to assess the relationships between numerical variables. For multivariate examinations, a logistic regression analysis with the entered model was used. A p-value of <0.05 was considered sufficient for statistical significance. Results The average age in the study population was 24.1±6.8. The body mass index was 26.5±7.2 in the observed patient. The study's findings showed that the detected variables in the FGR were different (table 1). However, the small number of cases did not allow statistical significance to proceed. The decreased term of investigation, AC, DC, STV, LTV, term at birth, body weight, body length, head circumference, and Apgar score were found in patients with FGR. The linear correlation between the detected variables showed some regularities (table 2). A significant correlation was found between the term at investigation and all other parameters. A strong correlation was detected between all checked variables of HRV: AC, DC, SI, STV, and LTV. AC and DC demonstrated a significant correlation between them and Apgar score. The model itself was meaningful. The different models with AC, DC, STV, and LTV did not show any relationships with biometry variables. Table 3. However, some of these indices may not fully represent all the potential issues in cases of FGR. The linear correlation showed similarities and mutual origin between AC, DC, SI, STV, and LTV. The correlation was found between AC and Apgar score, and DC and Apgar score, demonstrating the usefulness of these variables in diagnosing fetal distress [12]. However, the linear correlation did not provide evidence of a potential connection between fetal HRV and neonatal biometry parameters. This outcome may have been influenced by the significant interval between fetal NI-ECG and the time of birth. The relationships between fetal growth and maturation throughout gestation were clearer [13]. The multivariate logistic regression indicated a potential link between SI and the term of birth, body length, and head circumference. This suggests that this temporal HRV index may be associated with the anthropometric parameters of newborns. FGR is known as a syndromal pathology. However, the aetiological reason for FGR is query or undetermined in the majority of cases. Idiopathic FGR is associated with placental disorders [14]. Therefore, screening for FGR is a key point in the management of patients. The humanitarian crisis in the area of armed conflict determines the utility of low-resource techniques. These circumstances emphasize the prospect of fetal NI-ECG [15]. The limitation of this study is associated with a small observed number of FGR. The receiver operating characteristic analysis could be the next stage for the investigation of efficient diagnostic algorithms based on fetal NI-ECG. Conclusion SI demonstrated a relationship with neonatal anthropometric parameters. This variable could have utility as a screening program based on fetal NI-ECG in case of a lack of access to high-quality ultrasound or be a valuable addition to existing perinatal strategies. This descriptive cross-sectional study was performed among pregnant women who were admitted to Kharkiv Municipal Perinatal Center between 1 April 2024 and 30 June 2024. The data were obtained from the hospital automation system. Ethics approval was received from the Research Council and Ethical Committee of Kharkiv National Medical University, No25.0224p. Informed consent was obtained from all the patients. FGR was detected in 9 women. The fetal growth was appropriate in 31 patients. The fetal were obtained from an RR-interval time series registered from the maternal abdominal wall via fetal non-invasive electrocardiography (NI-ECG). The stress index (SI) was selected for evaluation among all linear HRV variables. The following conventional cardiotocographic parameters were determined: short-term variation (STV) and long-term variation (LTV). The variables of AC (acceleration capacity) and DC (deceleration capacity) were also detected.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e The study's findings showed that the detected variables in the FGR were different. However, the small number of cases did not allow statistical significance to proceed. A strong linear correlation was detected between all checked variables of HRV: AC, DC, SI, STV, and LTV. AC and DC demonstrated a significant correlation between them and Apgar score. The results of the multivariate regression model with SI showed a statistically significant relationship with a term of birth and a tendency to have a statistically significant relationship with body weight and head circumference. This suggests that this temporal HRV index may be associated with the anthropometric parameters of newborns.
