Abstract
Traditional neuropsychological memory assessments perpetuate a profound methodological disconnect between artificial laboratory conditions and the complex cognitive demands of everyday functioning, failing to capture the contextual, multifaceted nature of real-world memory while demonstrating poor correspondence with subjective complaints and caregiver observations—limitations that critically impede early detection of and trajectories of care for mild cognitive impairment and prodromal Alzheimer's disease. Virtual reality emerges as a paradigmatic solution to this ecological validity crisis, leveraging immersive, naturalistic environments that elicit authentic cognitive processing while maintaining experimental control, thereby revealing subtle deficits that compensatory mechanisms often mask in conventional assessments. Empirical evidence demonstrates VR's superior diagnostic sensitivity, with studies showing enhanced differentiation between healthy aging, MCI, and AD through tasks that capture the interdependent operation of memory alongside executive function and spatial navigation—reflecting how cognition actually functions rather than isolating artificial cognitive constructs. The technology's capacity to foster presence and naturalistic behavior yields assessment data that correlates more strongly with lived experiences of cognitive decline, bridging the persistent chasm between objective performance metrics and the everyday memory failures that prompt clinical consultation. This methodological evolution represents more than incremental improvement; it constitutes a fundamental reconceptualization of memory assessment that promises clinically meaningful evaluations essential for early intervention in neurodegenerative processes, though implementation challenges regarding standardization and accessibility remain to be addressed.
