Abstract
To compare the therapeutic effect of ultrasound-guided aspiration and sclerotherapy with operation in ovarian endometriomas. 229 patients having received laparoscopy and laparotomy in our hospital from December 2004 to December 2005 and 231 patients (including 41 cases of postoperative recurrence) receiving transabdominal or transvaginal aspiration and absolute alcohol sclerotherapy under sonographic guidance from January 2003 to December 2005 in OPD were followed-up. The average ages were 36.2 and 34.6 years respectively. 130 patients in the operation group and 144 in the ultrasound-guided aspiration and sclerotherapy group were seen repeatedly for more than 1 year. We found out that one tubal pregnancy and 11 intrauterine pregnancies occurred after operation with one fetal death and three term deliveries. Thirteen patients in the ultrasound-guided aspiration and sclerotherapy group had one baby each. The recurrences of endometrioma in these two groups were 24 and 27 respectively. Seventy-seven patients in the ultrasound-guided aspiration and sclerotherapy group were cured and 40 cases had their cysts shrink and symptoms relieved or lightened. Operation and ultrasound-guided aspiration and sclerotherapy are both effective treatment in ovarian endometriomas. The ultrasound-guided aspiration and sclerotherapy, however, is carried out in out-patient clinics, being cheaper and less painful, and can be repeated if necessary. In case of disease recurrence, it is the method worthy of special attention.
