The aim of the current study was to examine the prevalence of poor oral hygiene (tooth brushing less than two times per day) and to identify the associated factors among adolescents in Mongolia. We analyzed data from the 2019 Mongolian Global School-based Health Survey (GSHS) conducted nationwide among 10–18-year-old students. Univariable and multivariable analyses were performed to assess the correlates of poor oral hygiene. Overall, 35.6% of the adolescents reported to have poor oral hygiene in 2019. Multivariable analysis showed a significant association in the total sample of poor oral hygiene with male gender, poor hand hygiene behaviors including did not wash hand before eating, did not wash hand after toilet and handwashing without soap, being bullied, lack of close friends, smoking, exposure to second-hand smoke, physical inactivity and sedentary behavior. Several factors, including hand hygiene behaviors, mental distress, substance use, and health risk behaviors were found to be associated, which can aid in designing school-based dental health education intervention strategies to help students to develop habits that improve their health. Introduction The According to the World Health Organization (WHO) definition, “Oral Health” is the state of the mouth, teeth, and orofacial structures that enables people to perform significant roles, such as eating, breathing, and speaking, and includes psychosocial dimensions, and live without discomfort [ 1 ]. There are more than 300 diseases and conditions that affect human beings as a consequence of oral diseases including dental caries and periodontal diseases [ 2 ]. The WHO’s Global Oral Health Status Report (GOHSR) estimated that oral diseases affect nearly 3.5 billion people worldwide, and 2 billion adults and 514 million children suffer from dental caries [ 3 ]. In 2019, oral diseases affected an estimated 800 million people, including tooth decay, periodontal diseases, and tooth loss in the Western Pacific Region (WPR) [ 4 ]. Among school adolescents in WPR such as Philippines in 2011 and Malaysia in 2012 reported infrequent tooth brushing were 12.2%-12.9% [ 5 , 6 ]. In six Southeast Asian countries, the results showed that 17.1% had poor oral hygiene (brushing teeth less than twice a day) [ 7 ], whereas in Indonesia 10.8% had poor oral hygiene according to the GSHS (Global School-based Health Survey) in 2014–2015 [ 8 ]. Poor oral hygiene among adolescents has been associated with being male [ 9 ], older age or grade [ 10 ], hand hygiene behaviors [ 11 ], mental distress including being bullied and lonely [ 12 ], smoking and alcohol consumption [ 13 ], and health risk behaviors such as inadequate exercise and sedentary leisure time behavior [ 7 ]. The Fourth Mongolian STEPS Survey on the Prevalence of Non-Communicable Disease (NCD) and Injury Risk Factors showed that the prevalence of tooth brushing two or more times a day was 57% in people aged 15−69 years [ 14 ]. According to the Mongolian GSHS factsheets, brushing their teeth “less than once a day or never” were 8.2% and 5.1% among school-going students in 2010 and 2013 [ 15 , 16 ]. There is a lack of recent national information on oral hygiene behavior among adolescents in Mongolia. Therefore, the aim of the present paper is to update a previous GSHS study conducted in 2013, to assess the prevalence, and risk factors of poor oral hygiene practice, among adolescents in Mongolia. Results Sample characteristics of adolescents included in the study by sex. The Mongolian GSHS 2019 was conducted with a total sample size of 7149 students. The characteristics of the sample are shown in Table 1 . More than one-third (35.6%) of adolescents reported to have had poor oral hygiene in the past 30 days of the survey. According to the univariable analysis (Table 2 ), students who reported poor oral hygiene were more likely to be male and be in grade older than elementary grade. They had poor handwashing practices including did not wash hand before eating, did not wash hand after toilet, and handwashing without soap; had no close friends, reported being bullied; smoked cigarettes, were exposed to second-hand smoke at home, and drank alcohol. Students who reported being physically inactive and spent more than 3 hours per day sitting were more likely to have insufficient tooth brushing. All the independent variables were a significant relationship with poor oral hygiene among school-aged adolescents in Mongolia. (see Table 2 ) Results of multivariable logistic regression analysis for poor oral hygiene behavior . The multivariable logistic regression model is shown in Table 3 . Compared with the results of univariable analyses, the multiple model showed small differences in the predictors of oral hygiene in the total samples. Male students were 1.73 times (Adjusted Odds Ratio; (AOR) = 1.73; 95% Confidence Interval (CI) [1.56-–1.92]) more likely to have poor oral hygiene compared to the females. Concerning hand hygiene behaviors, students who had poor handwashing before eating were 1.54 times (AOR = 1.54; 95% CI [1.37–1.73]), poor handwashing after toilet were 1.56 times (AOR = 1.56; 95% CI [1.39–1.75]) and handwashing without soap were 1.31 times (AOR = 1.31; 95% CI [1.18–1.46]) more likely to be associated with poor oral hygiene. Moreover, students who reported being bullied were 1.16 times (AOR = 1.16; 95% CI [1.02–1.33]) and had no close friend were 1.37 times (AOR = 1.37; 95% CI [1.10–1.69]) more likely to have poor oral health than students who reported did not being bullied and had a close friend. As to smoking behaviors, students who reported cigarette smoking past 30 days of the survey were 1.29 times (AOR = 1.29; 95% CI [1.00–1.55]), and those being exposed to second-hand smoke were 1.15 times (AOR = 1.15; 95% CI [1.03–1.30]) more likely to report poor oral hygiene. Regarding health risk behaviors, students who were physically inactive were 1.35 times (AOR = 1.35; 95% CI [1.18–1.53]) and students who spent sitting more than 3 hours per day were 1.16 times (AOR = 1.16; 95% CI [1.05–1.29)]) as likely as to have poor oral hygiene compared to the counterparts. (see Table 3 ) Discussion This was a cross-sectional study among adolescents based on the GSHS 2019. Nearly 36% of Mongolian school-aged adolescents had poor oral hygiene seemed almost the same as the previous study [ 18 ]. In addition, this result was also higher than Indonesia in 2015 (10.8%) [ 19 ], Suriname in 2016 (14.3%), and Trinidad and Tobago in 2017 (22.1%) [ 20 ], but lower than Ghana in 2012 (37.4%) [ 12 ]. Our study investigated associations between demographic factors, hand hygiene behaviors, mental distress, substance use, health risk behaviors, and oral hygiene practice among adolescents in Mongolia. Our observation that males tended to have poorer oral hygiene practices than females was similar to findings from other studies [ 21 , 22 ]. This tendency might be because female students were more concerned about dental aesthetics and more aware of dental problems than male students. The findings that hand hygiene practices were associated with oral hygiene practice are in line with prior studies [ 11 ]. It is possible that good hand hygiene practices, which are known to have positive effects on well-being, act as a mediator in the context of poor oral hygiene. Our study confirmed previous findings showing an association between substance use, including current smoking and exposure to second-hand smoke, and poor oral hygiene in the adolescent population [ 23 ]. The adolescents with good oral hygiene practices were more likely to avoid smoking, and reducing their second-hand smoke exposure. An association between substance use and poor dental hygiene behavior may refer to a clustering of risky behaviors. The results of the present study showed a positive association between unhealthy risk behaviors such as being physically inactive and leisure time sedentary behavior and infrequent tooth brushing were consistent with evidence from the previous research [ 24 , 25 ]. Physical activity could indicate a disposition towards a healthy lifestyle and the capacity to keep general and oral health [ 26 ]. Children's sedentary behavior, such as the use of electronic media or screen-based activities suggested that excessive game players had less free time for tooth brushing as they chose to take part in screen-based activity [ 27 ]. Strengths and Limitations GSHS was a large, nationally representative survey, the data of which were collected by the WHO and the Centers for Disease Control and Prevention (CDC). GSHS is globally recognized and implemented providing highly generalizable data and findings. Schools and students were randomly selected from both urban and rural areas. The findings of this investigation must be viewed in light of its limitations. Firstly, the data were cross-sectional, so did not prove causation, but they did provide information on correlations. Secondly, a self-reported questionnaire was applied, so it might be possible that the children provided invalid answers. Thirdly, this study focused on only school-aged adolescents meaning that the findings are not generalizable to all adolescents in the country. Lastly, the involvement of parents/guardians may protect young people from risk factors, and the GSHS in Mongolia does not assess parental engagement. Conclusions The prevalence of poor oral hygiene (tooth brushing less than twice a day) was almost the same as the previous GHSH 2013 in Mongolia. Adolescents need to be encouraged to take responsibility for their oral hygiene. Several factors, including poor hand hygiene behaviors, mental distress, substance use, and other health risk behaviors were found to be associated, which can aid in designing school-based dental health education intervention strategies for that target adolescents and may help students to develop habits that improve their oral health. Methods Participants and Procedures. This study was a secondary data analysis of the 2019 Mongolian GSHS data. The GSHS 10 core questionnaire modules address the leading causes/risk factors of morbidity and mortality among children: tobacco, alcohol, and other drug use; dietary behaviors; hygiene; mental health; physical activity; sexual behaviors that contribute to HIV infection, other sexually transmitted infections, and unintended pregnancy; unintentional injuries and violence; and respondent demographics. Students were asked to participate voluntarily in the survey, and written informed consent was obtained from each student and parents/guardians. The GSHS used a two-stage cluster sampling design where participants were given a self-reported questionnaire. The first stage, the probability of schools being selected was proportional to the number of students enrolled. The second stage, involved classes being randomly selected and all students in the selected classes were eligible to participate [ 17 ]. The Mongolian GSHS protocol was approved by Resolution No. 88 of the Ethical Committee of the National Center for Public Health in November 2018. Altogether 7149 students participated in the 2019 Mongolian GSHS survey. Measures. With the exception of grade, all variables were dichotomized as yes or no/good or poor answers. Oral hygiene as the dependent variable is based on the following question: “During the past 30 days, how many times per day did you usually clean or brush your teeth?” (Response options were 1 = Did not brush my teeth during the past 30 days, 2 = less than 1 time per day, 3 = 1 time per day, 4 = 2 times a day, 5 = 3 times a day and 6 = 4 or more times per day; coded 1–3 = poor oral hygiene was defined as brushing teeth less than two times per day and 4–6 = good oral hygiene when brushing 2 or more times per day). The independent variables were demographic factors, hand hygiene behaviors, mental distress, substance uses, and health risk behaviors. Demographic factors: Gender “What is your sex?” (Response options were 1 = male and 2 = female; coded 0 = male and 1 = female). Grade “Which grade are you study?” (Response options were 1 = 5th grade, 2 = 6th grade, 3 = 7th grade, 4 = 8th grade, 5 = 9th grade, 6 = 10th grade, 7 = 11th grade and 8 = 12th grade). Hand Hygiene behaviors: Handwashing before eating “How often did you wash your hand before eating?” (Response options were 1 = never, 2 = rarely, 3 = sometimes, 4 = most of the time and 5 = always; coded 1–4 = no and 5 = yes). Handwashing after toilet “How often did you wash your hands after using the toilet or latrine?” (Response options were 1 = never, 2 = rarely, 3 = sometimes, 4 = most of the time and 5 = always; coded 1–4 = no and 5 = yes). Handwashing with soap “How often did you use soap when washing your hands?” (Response options were from “1 = never, 2 = rarely, 3 = sometimes, 4 = most of the time and 5 = always; coded 1–4 = no and 5 = yes). Mental Distress: Being bullied : “During the past 30 days, on how many days have you been bullied?” (Response options were 1 = 0 day, 2 = 1 or 2 days, 3 = 3 to 5 days, 4 = 6 to 9 days, 5 = 10 to 19 days, 6 = 20 to 29 days and 7 = All 30 days; coded 1 = no and 2–7 = yes). [Description provided in the questionnaire: Bullying occurs when one or more students or someone else about your age teases, threatens, ignores, spreads rumors about, hits, shoves, or hurts another person over and over again]. Close friend “How many close friends do you have?” (Response options were 1 = 0 friend, 2 = 1 friend, 3 = 2 friends and 4 = 3 or more; coded 1 = no and 2–4 = yes). Substance use: Cigarette smoking “During the past 30 days, how many days have you smoked cigarettes?” (Response options were 1 = 0 day, 2 = 1 or 2 days, 3 = 3 to 5 days, 4 = 6 to 9 days, 5 = 10 to 19 days, 6 = 20 to 29 days and 7 = All 30 days; coded 1 = no and 2–7 = yes). Health risk behaviors: Physical activity: “ During the past 7 days, on how many days were you physically active for a total of at least 60 min per day?” (Response options were 1 = 0 day, 2 = 1 day, 3 = 2 days, 4 = 3 days, 5 = 4 days, 6 = 5 days, 7 = 6 days and 8 = 7 days; coded 1 = physically inactive and 2–8 = physically active) Sedentary behavior “How much time do you spend during a typical or usual day sitting and watching television, playing computer games, talking with friends, or doing other sitting activities?” (Response options were 1 = Less than 1 hour per day, 2 = 1 to 2 hours per day, 3 = 3 to 4 hours per day, 4 = 5 to 6 hours per day, 5 = 7 to 8 hours per day and 6 = more than 8 hours per day; coded 3–6 = yes and 1–2 = no). Data Analysis . Data analysis was carried out with IBM SPSS (Statistical Package for the Social Sciences) version 28 (SPSS Inc., Chicago, IL, USA). Descriptive statistic was done to describe the study sample; distributions were calculated according to the number of current answerers. Univariable logistic regression analyses were conducted to examine unadjusted associations between oral hygiene and independent variables. Multivariable logistic regression analysis was used to assess the independent contribution of demographic factors, hand hygiene behaviors, mental distress, substance uses, and health risk behaviors to poor oral hygiene. The independent variables involved in the regression analysis were student gender, grade, and hand hygiene behaviors including handwashing before eating, handwashing after toilet and handwashing with soap, being bullied, having no close friends, cigarette smoking, exposure to second-hand smoke, alcohol drinking, being physically inactive, and sedentary behavior. The student’s grade was considered as a continuous variable in the model. Odds ratio (OR) and 95% CI of OR were used to indicate the association between the oral hygiene and the selected independent variables. Statistical significance was defined at p < 0.05. There are more than 300 diseases and conditions that affect human beings as a consequence of oral diseases including dental caries and periodontal diseases [\u003cspan citationid=\"CR2\" class=\"CitationRef\"\u003e2\u003c/span\u003e]. The aim of the current study was to examine the prevalence of poor oral hygiene (tooth brushing less than two times per day) and to identify the associated factors among adolescents in Mongolia. We analyzed data from the 2019 Mongolian Global School-based Health Survey (GSHS) conducted nationwide among 10\u0026ndash;18-year-old students. Univariable and multivariable analyses were performed to assess the correlates of poor oral hygiene. Overall, 35.6% of the adolescents reported to have poor oral hygiene in 2019. Multivariable analysis showed a significant association in the total sample of poor oral hygiene with male gender, poor hand hygiene behaviors including did not wash hand before eating, did not wash hand after toilet and handwashing without soap, being bullied, lack of close friends, smoking, exposure to second-hand smoke, physical inactivity and sedentary behavior.
