Abstract

This study evaluates ultrasound in the diagnosis of anterior, lateral, posterior parametrial endometriosis. Although our are few cases, ultrasound can detect endometriosis of the parameters, with a lower sensitivity and a much higher specificity for the anterior and lateral sites, while in the posterior compartment the sensibility is higher. Ultrasound is a useful tool in the preoperative assessment of parametrial involvement, although further studies are needed. Objectives

This study evaluates ultrasound in the diagnosis of anterior, lateral, posterior parametrial endometriosis.

Methods

We retrospectively analysed 36 patients with pathological diagnosis of endometriosis from Jan 2022 to March 2023. The performances of preoperative TVS diagnosis in evaluating the anterior, the lateral and the posterior parametria with regarding to assess the sensitivity (SE), specificity (SP), positive predictive value (PPV), and negative predictive value (NPV), in comparison with the surgical-pathological findings were evaluated.

Results

- Sensitivity, specificity, PPV, NPV, for the sonographic diagnosis of anterior parametrial endometriosis were 50%, 96.9%, VPP = 35, VPN = 50 respectively. - Sensitivity, specificity, PPV, NPV, for the sonographic diagnosis of lateral parametrial endometriosis were 43,75%, 95%, VPP = 87,5, VPN = 67,86 respectively. - Sensitivity, specificity, PPV, NPV, for the sonographic diagnosis of posterior parametrial endometriosis were 88,46%, 60%, VPP = 85,19 VPN = 66,67 respectively.

Conclusions

Although our are few cases, ultrasound can detect endometriosis of the parameters, with a lower sensitivity and a much higher specificity for the anterior and lateral sites, while in the posterior compartment the sensibility is higher. Ultrasound is a useful tool in the preoperative assessment of parametrial involvement, although further studies are needed.
