The study was aimed to identify the risk factors for GDM and its impact on pregnancy outcomes. Methods A multicenter retrospective cohort study was conducted to analyze the involving 41,254 pregnant women delivered in Hangzhou, China, from May 2014 to April 2024. Of these, 5,545 cases were GDM and 35,709 cases were non-GDM. Collection and analysis of electronic medical record front page data was performed. Univariate logistic regression was used to assess pregnancy outcomes, and significant variables were incorporated into multivariate binary logistic regression for further analysis by adjusted odds ratio (aOR) and 95% confidence interval (CIs). Results Multivariate binary logistic regression analysis identified maternal age, maternal weight and gravidity as independent risk factors for GDM, while parity was protective factor for GDM. For pregnancy outcomes, GDM was significantly associated with gestational hypertension (GH) (aOR = 1.796, 95%CI:1.573–2.051), preeclampsia (PE) (aOR = 1.947, 95%CI:1.594–2.379), dyslipidemia (aOR = 1.428, 95%CI:1.258–1.620), polyhydramnios (aOR = 1.530, 95%CI:1.123–2.084), preterm premature rupture of membranes (PPROM) (aOR = 1.497, 95%CI:1.093–2.050), preterm birth (aOR = 1.212, 95%CI:1.026–1.432), increased hospital day (aOR = 1.404, 95%CI:1.254–1.571), and macrosomia (aOR = 1.607, 95%CI:1.396–1.850), all P <0.05. Conclusion Maternal age, maternal weight and gravidity were independent risk factors for GDM, and parity was protective factor for GDM. GDM was significantly associated with GH, PE, dyslipidemia, polyhydramnios, PPROM, preterm birth, increased hospital day and macrosomia. gestational diabetes mellitus pregnancy outcomes risk factors Figures Figure 1 Introduction Gestational diabetes mellitus (GDM) is defined as the first detection of abnormal glucose tolerance during pregnancy. GDM is accompanied by decreased insulin sensitivity and relative deficiency of insulin produced by the endocrine system [ 1 ] . The global prevalence of GDM is estimated to be 14.0%, ranging from 2.3% to 24.2% in different regions, and the incidence of GDM has been steadily increasing, posing substantial health and economic burdens [ 2 ] . Previous studies have identified that the major determinants of maternal susceptibility to GDM included obesity, family history of diabetes, and advanced maternal age [ 3 , 4 ] . Despite extensive researches, a comprehensive understanding of all risk factors contributing to GDM remained elusive. GDM has both short- and long-term effects on mothers and newborns, increasing the risk of cesarean delivery, gestational hypertension (GH), and preeclampsia (PE), as well as the risk of fetal distress, preterm birth and macrosomia [ 5 ] . In addition to these immediate perinatal complications, children born to mothers with GDM were more prone to develop childhood obesity and early-onset type 2 diabetes [ 6 , 7 ] . Identifying risk factors for GDM in the population is crucial to facilitate early diagnosis and to implement intensive lifestyle modifications and medical interventions to manage blood glucose levels, with the goal of reducing the risk of GDM-related complications and the preventing or mitigating the adverse outcomes associated with GDM. Hence, a multicenter retrospective cohort study by electronic medical record information was designed to discuss the risk factors for GDM, and examined its impacts on pregnancy outcomes, in order to provide a scientific basis for promoting maternal and infant outcomes. Materials & methods Study Population A retrospective cohort study was designed from the front-page electronic medical record data of 44,700 pregnant women from May 2014 to April 2024. After applying the exclusion criteria outlined in Fig. 1 , a total of 41,254 pregnant women were included in the study, comprising 35,709 non-GDM and 5,545 GDM. The pregnant women were recruited from Hangzhou Women's Hospital, Affiliated Hospital of Hangzhou Normal University, and Maternal and Child Health Care Hospital of Linping District in Hangzhou. The study was in agreement with Hospital Ethics Committee (2024-A-119). No informed consent was required as this was a retrospective study. Diagnostic criteria In accordance with the International Association of Diabetes and Pregnancy Study Groups, 24–28 weeks gestation was acceptable to screen pregnant women for diabetes, and at least one of the following 75 g oral glucose tolerance test results must be met to diagnose GDM: fasting blood glucose ≥ 92 mg/dL, 1-h plasma glucose ≥ 180 mg/dL, 2-h plasma glucose ≥ 153 mg/dL [ 8 ] . Exclusion criteria The exclusion criteria were as follows: insulin-dependent diabetes mellitus, type 2 diabetes; history of immunotherapy and blood transfusion; history of chronic diseases such as heart disease, kidney disease, connective tissue disease, hematologic disorders; fetal death or stillbirth; chromosomal abnormalities such as trisomy 21, trisomy 18, trisomy 13; structural abnormalities or other birth defects; twin or multiple pregnancy; in vitro fertilization; incomplete data. Classification and definition of variable subgroups Classification and definition for maternal and neonatal outcomes were based on the latest 10th edition of the Chinese obstetrics and gynecology textbook [ 9 ] . Hypertensive disorders of pregnancy (HDP) were divided into GH, PE, chronic hypertension and normal blood pressure. GH was defined as systolic blood pressure ≥ 140 mmHg and/or diastolic blood pressure ≥ 90 mmHg after 20 weeks of gestation. GH with proteinuria was defined as PE. Intrahepatic cholestasis of pregnancy was defined as fasting total bile acid ≥ 10µmol/L or random total bile acid ≥ 19µmol/L. Amniotic fluid was categorized into polyhydramnios, oligohydramnios and normal level. Polyhydramnios was defined as the ultrasound examination of the maximum vertical pocket ≥ 8 cm or amniotic fluid index ≥ 25 cm; and oligohydramnios was defined as the maximum vertical pocket ≤ 2 cm or amniotic fluid index ≤ 5 cm. Premature rupture of membranes (PROM) was defined as spontaneous rupture of membranes before the onset of labor. Placenta previa was defined as the placenta adjacent to or covering the cervix after 28 weeks of gestation. Placental abruption was defined as part or all of the placenta separation from the uterine wall after 20 weeks of gestation. Prenatal anemia was defined as maternal peripheral hemoglobin < 110g/L. Intrauterine growth restriction was defined as an estimated fetal weight < 10th percentile for gestational age on ultrasonographic examination. Infant weight was classified as low birthweight (< 2500 g), normal birthweight (2500–4000 g), and macrosomia (≥ 4000 g). Statistical analysis The study used IBM-SPSS 21.0 statistics (IBM-SPSS, Chicago, USA). Data normality was analyzed by one-sample Kolmogorov-Smirnov test. The skewed data were expressed as the medians and percentile [M(IQR)]. Mann-Whitney U test was for comparation of quantitative skewed data between the two groups. Pearson chi-square test or continuity correction b test was used for the univariate analysis of qualitative data. Multivariate binary logistic regression analysis was for the calculation of odds ratio (OR) and 95% confidence interval (CI) for each indicator. Adjusted OR (aOR) was for adjusted potential confounding variables. A two-sided P -value < 0.05 was considered significant. Results Characteristics of the gravida and newborn Compared to the control group, maternal age was 30.00 years and maternal weight was 58.00 kg in the GDM group, which were all significantly higher (29.00 years, 54.50 kg; Z = 17.388, Z = 28.663; all P < 0.001). Gestational age in the two groups was both 39.00 weeks, but the difference was statistically significant ( Z = 13.668, P 0.05). Hospital day of GDM group was longer than that of control group (5.00 days vs. 4.00 days, Z = 14.617, P < 0.001). In addition, gravidity, parity, scar uterus and household registration were all statistically significant between the two groups (χ 2 = 61.820, χ 2 = 27.187, χ 2 = 34.199, χ 2 = 7.327; all P < 0.05) (Table 1 ). GDM group showed significant difference in infant weight (3300g vs. 3260g, Z = 5.234, P 0.05) (Table 2 ). Multivariate binary logistic analysis of pregnancy complications In further multivariate binary logistic analysis, the risk factors for GDM included maternal age (aOR = 1.526, 95%CI:1.433–1.624, age 30-35years; aOR = 1.641, 95%CI:1.268–2.124, age ≥ 35years), maternal weight (aOR = 1.607, 95%CI:1.396–1.850, weight>50kg), gravidity (aOR = 1.650, 95%CI:1.332–2.044, gravidity ≥ 5). Parity was inversely associated with GDM (aOR = 0.813, 95%CI: 0.736–0.898, parity = 2; aOR = 0.622, 95%CI: 0.457–0.848, parity ≥ 3). For pregnancy outcomes, GDM was significantly associated with GH (aOR = 1.796, 95%CI:1.573–2.051), PE (aOR = 1.947, 95%CI:1.594–2.379), dyslipidemia (aOR = 1.428, 95%CI:1.258–1.620), polyhydramnios (aOR = 1.530, 95%CI:1.123–2.084), PROM (aOR = 1.497, 95%CI:1.093–2.050), preterm birth (aOR = 1.212, 95%CI:1.026–1.432), increased hospital day (aOR = 1.404, 95%CI:1.254–1.571), and macrosomia (aOR = 1.607, 95%CI:1.396–1.850), all P <0.05 (Table 4 ). Discussion GDM is a crucial public health issue, and the global prevalence of GDM has reached a worrying level. The purpose of this study is to calculate the risk ratio after adjusting the mixed factors to evaluate the risk factors of GDM and the independent correlation between GDM and pregnancy outcomes. Our study found that the age of pregnant women, the weight of pregnant women, and the number of pregnancies were independent risk factors for the occurrence of GDM. GDM elevated the incidence of GH, PE, dyslipidemia, polyhydramnios, PPROM, preterm birth, hospital day and macrosomia. In this study, 98.6% of pregnant women with GDM were over 25 years old, and there was a significant correlation between the increased maternal age and the risk of GDM. Advanced maternal age pregnancy was usually referred to women aged 35 and above, as the pregnancy rate of older pregnant women continued to rise, there was concern about maternal and neonatal outcomes [ 10 ] . Old age might bring physiological changes related to reproductive aging, such as increased insulin resistance and reduced placental efficiency [ 11 ] . We observed that pregnant women with higher gravidity were more likely to suffer from GDM (aOR = 1.650, 95%CI: 1.332–2.044, gravidity ≥ 5). In a systematic review and meta-analysis, gravidity ≥ 2 was associated with a higher risk of GDM compared with gravidity = 1, but was not statistically significant. The potential cause might be cumulative physiological changes of each consecutive pregnancy increasing insulin resistance [ 4 ] . On the contrary, higher parity was associated with lower risk of GDM in our study (aOR = 0.813, 95%CI:0.736–0.898, parity = 2; aOR = 0.622, 95%CI:0.457–0.848, parity ≥ 3). A study of 198,237 pregnant women also found that there was an independent negative correlation between parity and GDM. Multiparity exerted the greatest protective effect against GDM in women under 30 years of age [ 12 ] . Postpartum women had a fuller understanding of the risk factors related to GDM, richer parenting experience, and lower psychological pressure. However, as women age, decreased insulin sensitivity and accumulated metabolic comorbidities might attenuate the beneficial effects of multiparity [ 13 ] . We observed that women with GDM were more likely to develop GH and PE, with aOR values of 1.796 and 1.947 respectively. GDM and HDP shared common pathophysiological processes characterized by systemic inflammation, insulin resistance and endothelial dysfunction [ 14 ] . Studies have established that GDM was a key risk factor for PE in twin pregnancies, with the risk of PE being at least 1.5 times higher. GDM might progress to complications, such as proteinuria, dyslipidemia, thrombocytopenia and pulmonary edema, ultimately leading to PE [ 15 ] . We found that hospitalization duration was significantly longer in GDM group (aOR = 1.404, 95%CI:1.254–1.571). Also, in a study including 3,436,671 pregnant patients, the GDM group had longer hospital stays, higher hospitalization costs, and higher rates of PE, GH and premature birth compared to non-GDM [ 16 ] . These findings suggested that GDM was related to duration and cost of hospitalization, which might be attributable to worsened maternal and infant outcomes. This study observed that GDM was an independent risk factor for polyhydramnios (aOR = 1.530, 95% CI: 1.123–2.084). Maternal hyperglycemia facilitated the transplacental passage of excess glucose into the fetal circulation, inducing fetal hyperglycemia and subsequent hyperosmotic diuresis and contributing to polyhydramnios, PPROM, and premature birth [ 17 ] . Notably, fetal membranes from GDM pregnancies exhibited higher rupture strength compared to those from non-diabetic patients [ 18 ] . Glycosylated hemoglobin, insulin at 120 min after glucose loading and homeostasis model assessment insulin resistance levels had been identified as significant predictors of PPROM in patients with GDM [ 19 ] . A retrospective cohort study found that the presence of GDM was associated with a two-fold increase in the development of macrosomia [ 20 ] . An 18-month prospective cohort study revealed that birth weight of neonates in the GDM group was significantly higher than that in the non-GDM group, with GDM conferring a significantly elevated likelihood of macrosomia [ 21 ] . These adverse neonatal outcomes were likely mediated by maternal hyperglycemia and fetal hyperinsulinemia, resulted in enhanced nutritional utilization and accelerated somatic growth, ultimately leading to aberrant fetal growth patterns [ 22 ] . In the present study, GDM significantly increased the risk of dyslipidemia during pregnancy (aOR = 1.428, 95%CI:1.258–1.620). Studies indicated that lipid metabolism was a key factor in insulin resistance. The GDM group exhibited significantly higher triglyceride levels, along with lower high-density lipoprotein and higher low-density lipoprotein levels [ 21 ] . A positive correlation was observed between maternal hemoglobin (Hb) concentration and the probability of GDM, with anemic women showing a reduced risk of GDM (aOR = 0.723, 95% CI: 0.671–0.779). Elevated Hb concentration might reflect better nutritional status, which was usually associated with increased probabilities of obesity and metabolic dysfunction. Compared to Hb concentration of 110–119 g/L, Hb concentration ≥ 140 g/L was associated with an increased risk of GDM, while Hb concentration ≤ 99 g/L was linked to a reduced risk [ 23 ] . There existed some limitations in the study. Our multicenter retrospective cohort study was designed upon established knowledge in the field and failed to demonstrate sufficient novelty. Due to the lack of maternal body mass index (BMI) information, we failed to correct or stratify and the results might be partially confused by BMI to a large extent. Therefore, larger researches of more detailed variables and innovative design are needed to elucidate the risk factors for GDM and the impacts on pregnancy outcomes in order to promote maternal and infant health. Conclusion GDM is one of the common complications during pregnancy. The main risk factors of GDM are advanced maternal age, higher maternal weight and gravidity, which can lead to poor pregnancy outcomes including GH, PE, dyslipidemia, polyhydramnios, PPROM, preterm birth, macrosomia and increased hospital day. Therefore, it is strongly recommended to strengthen maternal health care and implement intervention strategies to minimize the occurrence of potentially life-threatening obstetric complications and improve the overall outcomes of pregnant women. The study was aimed to identify the risk factors for GDM and its impact on pregnancy outcomes.\u003c/p\u003e\u003ch2\u003eMethods\u003c/h2\u003e\u003cp\u003eA multicenter retrospective cohort study was conducted to analyze the involving 41,254 pregnant women delivered in Hangzhou, China, from May 2014 to April 2024. Of these, 5,545 cases were GDM and 35,709 cases were non-GDM. Collection and analysis of electronic medical record front page data was performed. Univariate logistic regression was used to assess pregnancy outcomes, and significant variables were incorporated into multivariate binary logistic regression for further analysis by adjusted odds ratio (aOR) and 95% confidence interval (CIs).\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e\u003cp\u003eMultivariate binary logistic regression analysis identified maternal age, maternal weight and gravidity as independent risk factors for GDM, while parity was protective factor for GDM.
