Methods A retrospective analysis was performed of 347 patients with primary gastrointestinal lymphomas admitted to the First Affiliated Hospital of Xi’an Jiaotong University Between January 2014 and January 2024. The clinical characteristics of GIB, GIO, and GIP were recorded. This study employed propensity score matching (PSM) to identify risk factors for gastrointestinal complications (GICs). Results In our study of primary gastrointestinal lymphoma cohort, GICs occurred in 34.0% of patients. In our study, GIB occurred in 75 cases (21.6%), GIO occurred in 45 cases (12.9%) and GIP occurred in 28 cases (8.0%). PSM analysis indicated that decreased hemoglobin (112.57 ± 19.95 vs. 100.24 ± 23.20, P = 0.041) and LDH (272.00, 189.00-399.00 vs. 188.00, 166.00-230.90, P = 0.011) levels, as well as the presence of stomachache (P = 0.027) are high-risk factors for the development of GIB. Stomachache (28 [62.2%] vs. 37 [82.2%], P = 0.034), vomiting (12 [26.7%] vs. 21 [46.6%], P = 0.031), and endoscopic evidence of hyperplastic lesions (P = 0.022) are identified as high-risk factors for GIO. Moreover, patients with endoscopic findings of ulcerative lesions (4 [14.2%] vs. 12 [42.9%]), decreased hemoglobin levels (119.5 ± 22.32 vs. 102.79 ± 25.50, P = 0.012), stomachache (16 [57.1%] vs. 23 [82.1%], P = 0.048), and constipation (8 [28.6%] vs. 3 [10.7%], P = 0.043) are more likely to experience GIP. Introduction Primary extranodal lymphoma is defined as a type of lymphoma that originates in tissues and organs other than the lymph nodes, rather than primarily involving the lymph nodes.[ 1 ] The digestive system, particularly the gastrointestinal tract, is rich in lymphatic tissue, making it a common site for lymphoma invasion. Primary gastrointestinal lymphomas, although less frequent than epithelial cancers, still account for approximately 1% of all gastrointestinal tumors. Among these, the stomach is the most frequently affected organ, followed by the small and large intestines. In contrast, involvement of the esophagus, appendix, and rectum is relatively rare.[ 2 ] Primary gastrointestinal lymphoma can be categorized into B-cell lymphoma and natural killer (NK)/T-cell lymphoma. Additionally, based on the disease course, it is divided into aggressive and indolent types. Indolent B-cell lymphomas, such as mucosa-associated lymphoid tissue (MALT) lymphoma and follicular lymphoma (FL), typically cause minimal invasive damage to the gastrointestinal tract.[ 3 ] Primary gastrointestinal T/NK cell lymphomas (PGI-TNKL) are aggressive malignancies that encompass a variety of subtypes, each characterized by distinct clinicopathological features and prognoses.[ 4 ] Aggressive gastrointestinal lymphomas exhibit more invasive growth patterns and carry a worse prognosis compared to their indolent counterparts. When aggressive lymphomas occur in the gastrointestinal tract, severe gastrointestinal complications (GICs) such as gastrointestinal bleeding (GIB), gastrointestinal obstruction (GIO), and gastrointestinal perforation (GIP) are also common at the time of initial presentation. These complications are particularly prevalent after chemotherapy. Notably, GIP can lead to a very high mortality rate.[ 5 ] Timely identification of risk factors for GICs may facilitate early intervention and improve the prognosis for patients with aggressive gastrointestinal lymphoma. In a retrospective analysis of 148 patients, 56.8% experienced GICs, encompassing GIB, GIO, GIP, and multiple complications.[ 6 ] Patients with gastrointestinal lymphoma who develop GICs, such as GIB, GIO and GIP, and others, experience a significant decline in quality of life and may face severe clinical consequences, including life-threatening situations.[ 7 ] The occurrence of these complications was related to the aggressiveness of the tumor and the complexity of the treatment, and may indicated disease progression.[ 8 ] Therefore, early identification and active treatment of these complications are crucial for improving patients’ prognosis. However, there was no systematic summary of the risks of GICs in primary gastrointestinal lymphoma in current studies. Accurately identifying high-risk patients for GICs among primary gastrointestinal lymphoma patients was crucial for improving treatment outcomes and overall survival. In this study, we attempted to systematically analyze diagnosed with primary gastrointestinal lymphoma at our medical center in the past. We compared various indicators between the GICs group and the No-GICs group, identified risk factors for predicting GICs, which is conducive to the early identification of GICs by clinicians and attempts to provide evidence-based support for treatment. 2. Materials and Methods Patient Selection and evaluation criterion The patients included inpatient and outpatient lymphoma patients at our hospital from January 2014 and January 2024, and followed up until January 2025. Follow-up methods included in-patient review, outpatient review, and telephone follow-up. Primary gastrointestinal lymphoma was defined as predominant lesions in the alimentary tract with or without regional lymph node involvement.[ 9 ] All cases were diagnosed according to the World Health Organization diagnostic criteria.[ 1 ] All patients underwent gastroscopy and colonoscopy. The exclusion criteria were as follows: age under 18 years, infection with human immunodeficiency virus (HIV), lack of informed consent, inconsistent pathological diagnosis, or inability to undergo gastroscopy and colonoscopy. The clinical data of all patients at the time of initial diagnosis were collected in our cancer center, including age, gender, BMI, disease stage, blood lactate dehydrogenase (LDH), albumin, hemoglobin, symptoms, treatment methods and endoscopy or endoscopic ultrasonography. This retrospective research was approved by The Medical Research Ethics Committee of The First Affiliated Hospital of Xi’an Jiaotong University and conducted in accordance with the declaration of Helsinki. Macroscopy/endoscopy The macroscopic characteristics of lesions, including their morphology and size, were meticulously reviewed by a panel of expert endoscopists and pathologists. These assessments were based on detailed endoscopic observations and/or histopathological examination of specimens obtained from surgical resection. The lesion morphology was classified into four distinct categories: ulcerative, exophytic, hyperplastic, and punctate hemorrhage. Evaluation outcome criteria GICs were confirmed by clinical symptoms, laboratory tests, imaging, and/or surgery. GIO, including partial and complete obstruction, was considered to be the absence of passage of flatus or feces.[ 10 ] GIP was defined as the presence of free air under the diaphragm on abdominal imaging, or intestinal perforation during laparotomy.[ 11 ] GIB was defined as overt bleeding (hematemesis, bloody stools) and drop in hemoglobin of ≥ 2 g/dL.[ 12 ] All the above GICs were confirmed using imaging or surgery. During the course of the disease, it was possible to develop two or more complications. Statistical analysis The statistical analysis of the data was conducted using SPSS 26.0 (IBM Corp, Armonk, NY, USA). To ensure comparability between groups, propensity score matching (PSM) was utilized to perform 1:1 matching with a tolerance level of 0.02. The variables incorporated into the matching process included age, body mass index (BMI), sex, type of operative treatment, and disease stage. To evaluate the distribution of continuous variables within the sample, the Kolmogorov-Smirnov test was employed. Quantitative data that adhered to a normal distribution were reported as mean ± standard deviation (SD), while those exhibiting a non-normal distribution were described using the interquartile range (IQR). Categorical data were presented as frequency and proportion. For statistical comparisons, normally distributed quantitative data were analyzed using the independent samples t-test. In contrast, non-normally distributed data were assessed using the nonparametric Wilcoxon rank-sum test. Categorical data were examined using the chi-square test, Fisher’s exact test, or rank-sum test, as appropriate for the specific analysis. 3. Results GIB This study included a total of 347 patients. Before (PSM), there were 272 patients in the non-GIB group and 75 patients in the GIB group. After PSM, both the non-GIB and GIB groups had 75 patients each. Before PSM, the mean age of patients in the non-GIB group was 57.03±14.33 years, and in the GIB group, it was 57.45±16.02 years. After PSM, the mean age of patients in the non-GIB group was 58.08±15.55 years, and in the GIB group, it remained 57.45±16.02 years. The P-values for age differences between the two groups were not statistically significant both before and after PSM. Before PSM, the non-GIB group had 143 males (52.6%), and the GIB group had 42 males (56.0%). After PSM, the non-GIB group had 41 males (54.7%), and the GIB group had 42 males (56.0%). Before PSM, there was a significant statistical difference in the number of patients who underwent surgical treatment between the two groups (P=0.004). After PSM, all baseline indicators between the two groups were not statistically significant (P>0.05).(shown in Table 1.) Table 1. Among all the indicators included in this study, hemoglobin levels showed a significant difference between the two groups (112.57±19.95 vs. 100.24±23.20, P=0.041); LDH levels also showed a significant difference (272.00, 189.00-399.00 vs. 188.00, 166.00-230.90, P=0.011). Regarding abdominal symptoms, abdominal pain was significantly different between the two groups (P=0.027).(shown in Table 2.) Table 2. Among all the indicators included, there were significant differences between the two groups in stomachache (28 [62.2%] vs. 37 [82.2%], P=0.034) and vomiting (12 [26.7%] vs. 21 [46.6%], P=0.031). There was also a significant difference in the type of endoscopy between the two groups (P=0.022). Notably, hyperplastic lesions were significantly more common in the GIO group than in the non-GIO group.(shown in Table 4.) Table 4. After PSM, both groups had 28 patients each. After PSM, there were no significant differences in all baseline indicators between the two groups (P > 0.05). The age of patients in the non-GIP group was 55.14±18.45, and in the GIP group, it was 55.07±18.61. The proportion of males was 16 (57.1%) in the non-GIP group and also 16 (57.1%) in the GIP group. Among the non-GIP group, 11 (39.3%) patients underwent surgical treatment, while in the GIP group, 7 (25.0%) patients underwent surgical treatment.(shown in Table 5.) Table 5. Similarly, there were significant differences between the groups in terms of stomachache (16 [57.1%] vs. 23 [82.1%], P=0.048) and constipation (8 [28.6%] vs. 3 [10.7%], P=0.043). Regarding endoscope types, there was a significant difference in the prevalence of ulcerative lesions between the groups (4 [14.2%] vs. 12 [42.9%]). (shown in Table 6.) Table 6. Discussion Primary gastrointestinal lymphoma has the potential to arise in any segment of the digestive tract. It is most frequently encountered in the stomach, with the small intestine being the next common site, followed by the colon and rectum. In contrast, its occurrence in the esophagus is relatively rare.[ 13 ] Owning to the rapid progression of the primary gastrointestinal lymphoma, the enlargement and invasive characteristics of the mass were easy to cause GICs, the management of GICs remained a clinical challenge.[ 11 ] GICs were life-threatening acute abdomens such as GIO, GIP, and GIB. In particular, the perforation rate of small intestine was more than 50%, and the mortality rate was high.[ 14 ] In our study of primary gastrointestinal lymphoma cohort, GICs occurred in 34.0% of patients. In our study, GIB occurred in 75 cases (21.6%), GIO occurred in 45 cases (12.9%) and GIP occurred in 28 cases (8.0%). PSM analysis indicated that decreased hemoglobin and LDH levels, as well as the presence of stomachache, are high-risk factors for the development of GIB. Stomachache, vomiting, and endoscopic evidence of hyperplastic lesions are identified as high-risk factors for GIO. Moreover, patients with endoscopic findings of ulcerative lesions, decreased hemoglobin levels, stomachache, and constipation are more likely to experience GIP. Stomachache serves as a crucial warning sign in patients with primary gastrointestinal lymphoma, potentially signaling the emergence of severe complications such as bleeding, perforation, or obstruction. Timely identification and intervention are vital for enhancing patient outcomes.[ 15 ] This is mainly related to the growth pattern, location of the tumor and its impact on the surrounding tissues.[ 16 ] Tumor growth may induce the formation of mucosal ulcers, which can subsequently lead to bleeding or perforation. Concurrently, the mechanical compression exerted by the tumor can cause narrowing of the gastrointestinal lumen, resulting in obstruction.[ 17 ] The growth patterns of these tumors can lead to acute abdomen syndromes, which are primarily characterized by abdominal pain, such as obstruction, perforation, bleeding, and intussusception. Moreover, certain highly aggressive lymphoma types and the side effects of chemotherapy may further increase the risk of these complications.[ 18 ] Thus, in these patients, abdominal pain serves as a vital warning signal that may herald the onset of severe complications. Timely identification and treatment are of utmost importance for improving patient outcomes. Primary gastrointestinal lymphoma often manifests as submucosal tumors. As the tumor grows, it can lead to the formation of mucosal ulcers, which in turn can cause bleeding.[ 19 ] This bleeding may present as hematemesis, melena, or hematochezia, resulting in a decrease in hemoglobin levels.[ 20 ] Further development of the ulcers may perforate the gastrointestinal wall, leading to perforation. Perforation can cause peritonitis, characterized by severe abdominal pain, fever, and abdominal muscle rigidity, and in severe cases, may lead to sepsis and shock. Perforation can also cause internal bleeding, further reducing hemoglobin levels.[ 21 ] Chemotherapy or the natural progression of the tumor may lead to tumor necrosis.[ 22 ] Necrotic tissue releases LDH into the bloodstream, causing an increase in LDH levels.[ 23 ] Along with tumor progression and necrosis, the risk of gastrointestinal bleeding and perforation also increases. Therefore, changes in hemoglobin and LDH levels are important clinical indicators that can help doctors to identify severe complications such as bleeding and perforation at an early stage. Timely diagnosis and treatment are crucial for improving patients' prognosis. Ulcers have the potential to erode blood vessels within the gastrointestinal wall, thereby triggering acute bleeding. This form of bleeding may present as hematemesis, which is the vomiting of blood, melena characterized by black, tarry stools, or hematochezia indicated by bright red blood in the stool. In particularly severe instances, it can progress to hemorrhagic shock.[ 24 ] The risk of blood vessel erosion increases with the depth and extent of the ulcers. When ulcers are larger, they are more likely to penetrate multiple layers of the gastrointestinal wall, thereby heightening the probability of bleeding.[ 25 ] Primary gastrointestinal lymphoma exhibits a certain degree of invasiveness, with tumor cells potentially invading blood vessels directly, thereby causing bleeding. Petechial bleeding typically signifies that the gastrointestinal mucosa has already sustained damage and that the tissue has become fragile. This increased fragility elevates the risk of gastrointestinal wall perforation.[ 26 ] Petechial bleeding may be associated with local inflammation and tissue necrosis. Necrotic tissue is more prone to rupture, which can lead to perforation. Chemotherapy can induce tumor cell necrosis and shedding, causing further damage to the gastrointestinal mucosa and thereby increasing the risk of perforation. [ 27 ] The detection of ulcers and petechial bleeding during endoscopy is an important warning sign, indicating that patients may be at risk of bleeding and perforation. Early identification of these risks and the implementation of appropriate interventions are crucial for improving patients' prognosis. This study is a limited-scale, single-center retrospective investigation with a relatively small sample size. Given these limitations, it is essential to conduct large-scale prospective trials to identify appropriate predictors for GIB, GIO and GIP. There is a wide array of chemotherapy regimens available for primary gastrointestinal lymphoma, and these treatments often induce a variety of gastrointestinal complications, including bleeding, obstruction, and vomiting. In future research endeavors, we plan to explore these issues in conjunction with specific chemotherapy regimens. 5. Conclusion Patients with gastrointestinal lymphoma often experience GIB, GIO or GIP. Gastrointestinal complications can significantly reduce quality of life, prolong hospital stays, and decrease long-term survival rates. This study attempts to identify risk factors for GIB, GIO, and GIP in patients with primary gastrointestinal lymphoma through PSM analysis. The clinical characteristics of GIB, GIO, and GIP were recorded. This study employed propensity score matching (PSM) to identify risk factors for gastrointestinal complications (GICs).\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e\u003cp\u003eIn our study of primary gastrointestinal lymphoma cohort, GICs occurred in 34.0% of patients. In our study, GIB occurred in 75 cases (21.6%), GIO occurred in 45 cases (12.9%) and GIP occurred in 28 cases (8.0%). PSM analysis indicated that decreased hemoglobin (112.57\u0026thinsp;\u0026plusmn;\u0026thinsp;19.95 vs. 100.24\u0026thinsp;\u0026plusmn;\u0026thinsp;23.20, P\u0026thinsp;=\u0026thinsp;0.041) and LDH (272.00, 189.00-399.00 vs. 188.00, 166.00-230.90, P\u0026thinsp;=\u0026thinsp;0.011) levels, as well as the presence of stomachache (P\u0026thinsp;=\u0026thinsp;0.027) are high-risk factors for the development of GIB. Stomachache (28 [62.2%] vs. 37 [82.2%], P\u0026thinsp;=\u0026thinsp;0.034), vomiting (12 [26.7%] vs. 21 [46.6%], P\u0026thinsp;=\u0026thinsp;0.031), and endoscopic evidence of hyperplastic lesions (P\u0026thinsp;=\u0026thinsp;0.022) are identified as high-risk factors for GIO.
