Ангиогенез является ключевым биологическим процессом, обеспечивающим регенерацию и функциональную активность эндометрия, однако его дисбаланс играет патогенетическую роль в развитии широкого спектра гинекологических и сосудистых заболеваний. В представленном литературном обзоре рассмотрены современные данные о связи ангиогенных факторов с патологией репродуктивной системы и сосудистыми нарушениями у женщин. Установлено, что при аномальных маточных кровотечениях, лейомиоме матки, эндометриозе и аденомиозе отмечается повышение экспрессии проангиогенных стимулов, включая сосудистый эндотелиальный фактор роста (VEGF) и адреномедуллин (ADM), что свидетельствует о системном характере сосудистых дисфункций и их выходе за пределы локальных изменений эндометрия и миометрия. По данным литературы, имеется ассоциация нарушений ангиогенеза с репродуктивными потерями и неудачами имплантации, где снижение VEGF рассматривается как критический фактор снижения васкулярной поддержки эмбриона. Данные молекулярно-биологических исследований указывают на роль мРНК в регуляции клеточной миграции и сосудистой перестройки, а также на вовлечение лимфангиогенеза в патогенез аденомиоза. Данные механизмы имеют прямое отношение к развитию хронической венозной недостаточности и варикозной болезни вен нижних конечностей у женщин, что позволяет рассматривать ангиогенные факторы как универсальные регуляторы, объединяющие репродуктивные и сосудистые патологии. Angiogenesis is a key biological process that ensures the regeneration and functional activity of the endometrium, but its imbalance plays a pathogenetic role in the development of a wide range of gynecological and vascular diseases. The presented literature review examines current data on the relationship between angiogenic factors and reproductive system pathology and vascular disorders in women. It has been established that abnormal uterine bleeding, uterine leiomyoma, endometriosis and adenomyosis are associated with an increased expression of proangiogenic stimuli, including vascular endothelial growth factor (VEGF) and adrenomedullin (ADM), indicating a systemic nature of vascular dysfunctions and their extension beyond local changes in the endometrium and myometrium. Particular importance is attached to the association of angiogenesis disorders with reproductive losses and implantation failures, where a decrease in VEGF is considered a critical factor in reducing vascular support for the embryo. Molecular biology data indicate the role of mRNA in the regulation of cell migration and vascular remodeling, as well as the involvement of lymphangiogenesis in the pathogenesis of adenomyosis. These mechanisms are directly related to the development of chronic venous insufficiency and varicose veins of the lower extremities in women, which allows us to consider angiogenic factors as universal regulators that combine reproductive and vascular pathologies.
