To date, no studies have examined the correlation between the weight-adjusted waist index (WWI) and LBP. This study aimed to investigate the clinical and demographic profiles of individuals with and without LBP and examine the potential role of WWI as a risk factor and predictor of LBP. Methods: In this cross-sectional study, we examined a cohort of adult participants who provided comprehensive data on WWI and LBPfrom the 1999–2004 National Health and Nutrition Examination Survey. WWI was calculated by dividing waist circumference by the square root of body weight. We explored the association between WWI and LBPusing binary logistic regression modeling and propensity score matching. Results: The study included 8,041 participants aged over 20 years, of whom 3,134 had a history of LBP. A significant positive correlation was found between WWI and LBP in participants aged over 65 years, indicating that WWI is a strong predictor of LBP in old age groups. In contrast, the relationship between WWI and LBP was less consistent in participants aged 20–64 years, suggesting that other factors may have a more significant influence in other age groups. Conclusion: The findings of this study underscore the significance of the WWI as a potential predictor of LBP, particularly in older adults. The robust association observed between higher WWI and the increased prevalence of LBP in individuals highlights the importance of central adiposity as a clinical marker. Figures Figure 1 Introduction Low back pain (LBP) is a common musculoskeletal disorder characterized by pain and discomfort localized below the costal margin and above the inferior gluteal folds, with or without associated leg pain 1 . Globally, LBP is a leading cause of disability, affecting approximately 540 million people at any given time and contributing significantly to the global disease burden 1 , 2 . The prevalence of LBP increases with age and is more common among individuals with physically demanding occupations, sedentary lifestyles, and certain comorbid conditions 3 . Obesity is defined as the excessive accumulation of body fat, typically quantified using the body mass index (BMI) 4 , 5 , where a BMI of 30 kg/m² or higher is classified as obese. Globally, obesity has reached epidemic proportions, with over 650 million adults classified as obese in 2016. This condition is associated with a range of systemic complications, including cardiovascular diseases, type 2 diabetes mellitus, certain cancers, and musculoskeletal disorders 3 , 6 .The mechanisms by which obesity leads to these complications include chronic low-grade inflammation, increased mechanical load, insulin resistance, and alterations in adipokine secretion 7 , 8 . The relationship between obesity and LBP is well-documented 7 , 9 , with numerous studies revealing a positive association between increased body weight and the prevalence and severity of LBP. This interaction is likely to be multifactorial, involving both mechanical load and biochemical pathways. Excess body weight increases the mechanical load on the spine, potentially leading to degenerative changes and pain. Additionally, obesity-related inflammation may contribute to the development and persistence of LBP 8 , 10 . Traditionally, BMI has been the primary indicator of obesity. However, BMI does not account for body fat distribution, which can vary significantly among individuals with the same BMI. This study utilizes the weight-adjusted waist index (WWI) as a more precise measure of obesity 11 .WWI is calculated as the waist circumference (WC) divided by the square root of body weight, providing an indicator of central adiposity that adjusts for individual body size and weight. Our research examined the relationship between WWI and LBP using data from the National Health and Nutrition Examination Survey (NHANES) database from 1999 to 2004 to provide a comprehensive analysis of this association. We aimed to investigate the clinical and demographic profiles of individuals with and without LBP and examine the potential role of WWI as a risk factor and predictor of LBP.Using logistic regression models and propensity score matching (PSM) techniques, we aimed to adjust for confounding variables such as BMI, education level, neck pain, smoking status, sex, hyperlipidemia, and race. This approach allowed us to isolate the effect of WWI on LBP risk, providing valuable insights into the complex interplay between obesity and LBP. Methods Survey population This study analyzed data from the NHANES conducted between 1999 and 2004. The sample consisted of 8,041 participants aged 20 years and older. Participants were selected using a complex, multistage probability sampling design to ensure representation of the U.S. civilian, non-institutionalized population. The inclusion criteria required complete data on LBP, WWI, and other covariates of interest. Calculation of WWI The WWI was calculated as the WC in centimeters divided by the square root of body weight (kg). This measure was chosen over the traditional BMI because it provides a more accurate assessment of central obesity, which is more closely associated with metabolic complications and potentially LBP. Covariates of interest Covariates included demographic variables (age, sex, race/ethnicity), socioeconomic status (education level), lifestyle factors (smoking status, alcohol consumption), and health status indicators (BMI, neck pain, hyperlipidemia, diabetes mellitus, hypertension, and bone mineral density). These covariates were selected based on their potential confounding effects on the relationship between WWI and LBP. Statistical analysis Descriptive statistics are described using means, standard deviations, ranges, and percentages for normally distributed variables, and medians and interquartile ranges for non-normal distributions. Normally distributed variables were compared using independent samples t-test and analysis of variance. Categorical, ordinal, and continuous variables with skewed distributions were compared using the Mann–Whitney U test or chi-square test. Binary logistic regression models were used to examine the association between WWI and LBP, stratified by age group to control for multicollinearity. To mitigate the influence of confounding variables, PSM was implemented, followed by reevaluation using the Mann–Whitney U test. Statistical significance was set at p < 0.05. Analyses were conducted using Statistical Package for the Social Sciences software version 26.0 (IBM Corp., Armonk, New York, USA). Results Baseline Characteristics Table 1 shows the clinical and demographic profiles of the study population with and without LBP. The study comprised 8,041 participants, with a mean age of 55.53 ± 16.64 years for those with LBP and 55.76 ± 17.24 years for those without. Males constituted 45.66% of the LBP group and 50.48% of the non-LBP group. The mean WWI was significantly higher in the LBP group (11.13 ± 0.84) than in the non-LBP group (11.00 ± 0.82). Compared with the group without a history of LBP, the group with a history of LBP had a higher percentage of comorbidities, including hypertension, hyperlipidemia, diabetes mellitus, neck pain, a higher prevalence of cigarette smoking, and higher BMI. Notably, there were no significant differences between the groups in terms of age, race, or height ( p > 0.05). and LBP, logistic regression analyses were performed for each age group, excluding variables prone to multicollinearity, such as weight, height, BMI, and WC. As shown in Table 5 , WWI is not necessarily a strong predictor or risk factor for LBP across all age groups. Although a positive correlation was observed in the 20–44-year age group (odds ratio [OR] = 1.15; 95% confidence interval [CI], 1.02–1.31, p < 0.05) and the over 65 age group (OR = 1.35; 95% CI, 1.20–1.52, p 0.05), the results indicated a negative association, suggesting the presence of confounding factors. Associations between lower back pain (LBP) and various influencing factors are also shown in the Table 5 and Fig. 1 . Gender (male) showed a protective association in older adults (OR = 0.78; 95% CI, 0.66–0.92 p < 0.05), while a risk factor(OR = 1.15; 95% CI, 1.04–1.27 p < 0.05) for the younger group. Lower educational attainment was linked to higher odds of LBP across age groups. Non-Hispanic White and Other Hispanic individuals exhibited higher LBP odds, with Non-Hispanic Whites showing the strongest association (OR = 2.06; 95% CI, 1.50–2.83, p < 0.001). Comorbid conditions, such as hypertension and hyperlipidemia, were moderately associated with LBP, and neck pain demonstrated a strong correlation across all age groups (ORs ranging from 4.20 to 4.46, p < 0.001). Additionally, smoking and waist-to-height index (WWI) were positively associated with LBP, with WWI being particularly influential in older adults (OR = 1.35; 95% CI 1.20–1.52, p < 0.001). These findings underscore the relevance of demographic, lifestyle, and health-related factors in LBP risk. To address potential confounding factors, PSM was employed, followed by reevaluation using the Mann–Whitney U test (Tables 1 to 4 ). After matching, the association between WWI and LBP remained statistically significant only in the older age groups, with a p -value indicating a robust relationship. However, in the 20–64-year age groups, the p -values were no longer significant after PSM. Discussion The findings of this study highlight a significant association between the WWI and LBP. Our analysis demonstrated that individuals with a higher WWI were more likely to report LBP than were those with a lower WWI. This relationship remained robust even after adjusting for various confounding variables, suggesting that WWI is an independent risk factor for LBP. The positive correlation between WWI and LBP observed in this study is consistent with previous research linking central obesity to musculoskeletal pain. WWI, as a measure of central adiposity, reflects not only overall body weight but also the distribution of fat, which is more metabolically active and likely to contribute to systemic inflammation. This inflammation could be a key factor in the development and persistence of LBP 6 , 7 . In addition to WWI, several other fat distribution parameters have been explored for their potential associations with LBP in previous research. For instance, the visceral adiposity index (VAI), body adiposity index, and waist-to-height ratio (WHR) have been studied in relation to their ability to predict metabolic disturbances, which are closely related to central obesity and systemic inflammation 12 , 13 . VAI, in particular, has shown a strong correlation with visceral fat and metabolic disturbances, suggesting its utility in evaluating LBP risk. Similarly, WHR has been recognized as a superior predictor of cardiometabolic risks compared to BMI, indicating its potential for assessing LBP risk 14 , 15 . Other studies have identified the importance of fat infiltration in specific muscle groups, such as those assessed using the subcutaneous fat index (SFI) and SF tissue thickness (SFTT), in relation to lumbar spine health and LBP 16 , 17 . These parameters highlight the significance of regional fat distribution rather than general obesity, emphasizing their relevance as more precise indicators of LBP risk than traditional measures such as BMI 18 . Overall, while WWI remains an important marker for LBP owing to its reflection of central adiposity, other fat distribution indices, such as VAI, WHR, SFI, and SFTT, offer a broader perspective by considering the distribution and metabolic activity of fat. These indices are crucial for improving the risk prediction and management of LBP, particularly in individuals with central obesity, as they provide insights into the underlying metabolic disturbances that contribute to LBP. The relationship between the WWI and LBP can be explained through several mechanisms. First, the mechanical load theory suggests that excess abdominal fat, particularly in the context of central obesity, shifts the body’s center of gravity forward 7 . This shift necessitates compensatory postural adjustments, which increase the mechanical strain on the lumbar spine. Over time, these adjustments may lead to accelerated degenerative changes in intervertebral discs and facet joints, contributing to chronic LBP development 19 . Additionally, adipose tissue, especially visceral fat, secretes pro-inflammatory cytokines such as interleukin-6, tumor necrosis factor-alpha, and C-reactive protein 8 , 10 . These cytokines contribute to a state of chronic low-grade inflammation that exacerbates pain via direct inflammatory pathways. Moreover, this systemic inflammation affects central sensitization mechanisms in the nervous system, potentially leading to heightened pain sensitivity 20 , 21 . This could partially explain why individuals with a higher WWI are more likely to experience persistent LBP. Metabolic disturbances associated with central obesity, such as insulin resistance and hyperlipidemia, may also contribute to musculoskeletal pain. Insulin resistance can lead to reduced perfusion and oxygenation of spinal tissues, increasing the risk of ischemic pain and disc degeneration 14 , 22 , 23 . Furthermore, hyperlipidemia may contribute to the development of atherosclerosis, which can impair blood flow to spinal structures and exacerbate degenerative processes and pain 24 . This study further highlights potential age- and sex-related differences in the relationship between WWI and LBP. Specifically, older adults and females who typically exhibit greater fat accumulation and distinct fat distribution patterns may be more susceptible to the adverse effects of central obesity on LBP. Age-related factors, such as sarcopenia (age-associated muscle loss) combined with increased fat deposition, particularly in the abdominal region, could significantly exacerbate the likelihood of LBP in these populations 25 . Additionally, in females, fat-free mass (lean body mass) has been found to be inversely correlated with the intensity of LBP. This suggests that a higher proportion of muscle mass in women could mitigate the severity of LBP, whereas the loss of lean body mass, commonly observed with aging, could worsen this condition 26 . For example, while middle-aged individuals may show a weaker association between central obesity and LBP, potentially due to relatively preserved muscle mass, older adults, especially women, may experience a stronger correlation due to the compounded effects of aging, central adiposity, reduced lean body mass, and worse fatty infiltration of the erector spinae 27 . These findings suggest that targeted interventions that account for age- and sex-specific differences in fat distribution, muscle composition, and fat-free mass may be more effective in managing and preventing LBP. Tailoring strategies to address the unique needs of these subgroups could enhance the efficacy of treatment and prevention efforts, ultimately reducing the burden of LBP in older adults and females. One of the strengths of this study is the use of WWI, a novel and precise measure of central obesity that may offer a better assessment of obesity-related health risks than that by BMI. The large, nationally representative sample size and robust statistical methods, including PSM, further enhance the reliability of our findings. In clinical practice and future research, these findings underscore the importance of monitoring and managing central obesity to prevent and alleviate LBP. Clinicians should consider incorporating measures of central obesity, such as WWI, into routine assessments for patients with or at risk of LBP 28 . Future research should explore the underlying mechanisms linking central obesity and LBP in more detail and evaluate the effectiveness of targeted interventions aimed at reducing central obesity in preventing and managing LBP. This could lead to the development of more effective and personalized treatment strategies that improve patient outcomes and reduce the burden of LBP on individuals and healthcare systems. The present study offers different insights but is subject to a few limitations. First, its cross-sectional design limits the ability to establish causal relationships between WHR and LBP. Longitudinal studies are required to clarify the temporal dynamics of this relationship. Second, the reliance on self-reported LBP data introduces a risk of recall bias. Future research should incorporate objective measures, such as clinical assessments or imaging, and leverage wearable technology to track physical activity and sedentary behaviors providing more accurate lifestyle data. Additionally, the binary classification of variables, such as LBP and smoking, may oversimplify the complexity of these conditions, potentially obscuring the nuances of severity or frequency and introducing confounding factors. Furthermore, residual confounding is possible, as psychosocial factors such as stress and depression, both of which are linked to obesity and LBP, were not included in the analysis. While the NHANES provides a robust dataset, the study’s findings may have limited generalizability outside the U.S. population, given the variations in cultural, dietary, and activity patterns. To validate WWI as a predictor of LBP across diverse populations, replication in different cohorts is necessary. Lower educational attainment was linked to higher odds of LBP across age groups. Non-Hispanic White and Other Hispanic individuals exhibited higher LBP odds, with Non-Hispanic Whites showing the strongest association (OR\u0026thinsp;=\u0026thinsp;2.06; 95% CI, 1.50\u0026ndash;2.83, p\u0026thinsp;\u0026lt;\u0026thinsp;0.001). Comorbid conditions, such as hypertension and hyperlipidemia, were moderately associated with LBP, and neck pain demonstrated a strong correlation across all age groups (ORs ranging from 4.20 to 4.46, p\u0026thinsp;\u0026lt;\u0026thinsp;0.001). Additionally, smoking and waist-to-height index (WWI) were positively associated with LBP, with WWI being particularly influential in older adults (OR\u0026thinsp;=\u0026thinsp;1.35; 95% CI 1.20\u0026ndash;1.52, p\u0026thinsp;\u0026lt;\u0026thinsp;0.001). WWI was calculated by dividing waist circumference by the square root of body weight. We explored the association between WWI and LBPusing binary logistic regression modeling and propensity score matching.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults: \u003c/strong\u003eThe study included 8,041 participants aged over 20 years, of whom 3,134 had a history of LBP. A significant positive correlation was found between WWI and LBP in participants aged over 65 years, indicating that WWI is a strong predictor of LBP in old age groups.
