Methods A total of 732 patients with initially diagnosed non-metastatic resectable gastric cancer between 2004 and 2016 were screened using the Surveillance, Epidemiology, and Results database of the National Cancer Institute of the United States and retrospectively analyzed. Among them, 366 patients received postoperative chemotherapy and 506 patients did not. The propensity score matching was used to balance the two groups of confounding factors, the Kaplan–Meier method was used for survival analysis, and the logrank test was used to compare the differences between the survival curves. The Cox proportional hazards regression model was used to screen independent prognostic factors and establish a nomogram survival prediction model. The patients were divided into high-, moderate-, and low-risk groups according to the overall survival prediction score generated by X-tile software based on the nomogram. Results Multivariate analysis showed that the independent prognostic factors of gastric cancer in the group not receiving chemotherapy were history, ypT stage, ypN stage, and examined lymph node count, which were included in the nomogram prediction model. The C-index for the model was 0.727 (95% confidence interval, 0.65056–0.80344). The patients were divided into three different risk level groups based on the nomogram prediction score. Low- and moderate-risk patients did not benefit from adjuvant chemotherapy, while high-risk patients did. Conclusion The nomogram model in the present study can effectively evaluate the prognosis of patients with resectable gastric cancer. For advanced GC, radical R0 resection is the only treatment method, and completion of radical resection has a crucial impact on survival. Even after radical resection, 40–60% of patients still experience tumor recurrence or metastasis. With continuous in-depth study of GC in recent years, the treatment for GC has gradually moved from a single surgical treatment mode to a multidisciplinary integrated treatment mode [ 2 – 4 ]. As early as 1982, some American scholars have put forward the concept of neoadjuvant chemotherapy (NACT) [ 5 ]. NACT has gradually attracted clinical attention because of its advantages in reducing tumor stage and improving R0 resection rate. In 2006, the MAGIC study demonstrated that surgery combined with NACT significantly prolonged the survival of patients with resectable GC compared to patients treated with surgery alone, with a 13.3% increase (36.3% vs. 23.0%, P = 0.009) in the overall survival (OS) in five years [ 6 ], establishing the important position of NACT in the treatment of advanced GC. However, only about 50% of GC patients can receive adjuvant chemotherapy (ACT) after completing NACT and surgery, and the effect of ACT is controversial. Lin et al. found that among patients who completed NACT and surgery, 74.1% received ACT and 25.9% did not. The three-year survival rate of patients receiving ACT was higher than that of patients not receiving it (60.1% vs. 49.3%, P = 0.02) [ 7 ]. Lichthardt et al. found that the three-year survival rate of 72 patients who completed NACT and R0 resection and received ACT was lower than that of the non-receiving group (71.2% vs. 90.0%, P = 0.038) [ 8 ]. After receiving NACT, the radical resection of advanced GC was completed. Whether ACT can benefit patients and help to screen individuals who can benefit from ACT needs to be investigated. In the present study, patient prognosis was evaluated by establishing a nomogram. The patients were divided into risk groups to choose the ACT by consulting the Surveillance, Epidemiology, and End Results (SEER) and external validation databases. Methods Patient cohort The SEER * Stat (version 8.3.5) software was used to identify 12,619 patients with GC (T1-4N0-3M0) who completed NACT and radical resection between 2004 and 2016. Inclusion and exclusion criteria are described in Fig. 1 . Patients were divided into non-ACT (506) and ACT (366) groups based on whether they received ACT. The included variables were age, sex, race, grade, histology, location, size, ypT stage, ypN stage, examined lymph node (ELN) count, and patient prognosis information. Patients with unknown surgical status, pathology, and information on the above variables were further excluded. Statistical analysis The chi-squared test was used to compare the distribution of clinicopathological factors between the ACT and non-ACT groups. In order to reduce the confounding bias caused by grouping, significant pathological grouping factors in the chi-squared test were included in propensity score matching (PSM) analysis. The non-ACT and ACT groups were matched 1:1 [ 9 ]. The Kaplan–Meier method and log-rank test were used for survival analysis. The prediction model was established as follows: (1) Cox was used to analyze the correlation between OS and variables; (2) statistically different variables (P < 0.05) in the univariate analysis were included in the Cox multivariate model; and (3) a nomogram survival prediction model was established on the basis of the Cox multivariate model. Next, the effectiveness of the prediction model was tested as follows: the discrimination index C-index was used to quantify discrimination [ 10 ]; the consistency between the predicted survival rate and the actual survival rate was represented intuitively using a calibration curve, while avoiding excessive fitting of the model. Finally, patient prognosis risk was divided into high-, middle-, and low-risk groups according to the OS prediction score generated by X-tile software based on the nomogram [ 11 ]. SPSS 24.0 and R (version 4.0.0) software packages were used for all statistical analysis in this study, and P < 0.05 was considered as statistically significant. Results Patient demographics Prior to PSM, a total of 872 postoperative patients with GC who completed NACT were included, consisting of 506 in the non-ACT group and 366 in the ACT group (Fig. 1 ). The median patient survival time was 21 months (0–160 months), while the number of deaths was 362 (41.5%). Chi-squared test analysis showed that ACT was significantly correlated with age, race, grade, histology, size, ypN stage, and ELN count (P < 0.05). After the variables related to ACT were included in PSM, the final patient number was 732, including 366 in the non-ACT group and 366 in the ACT group. The median patient survival time was 20 months (0–156 months), while the number of deaths was 287 (39.2%; Table 1 ). There was no difference in the prognosis between the groups with and without ACT after PSM (five-year survival rate: 56.5% vs. 47.7%, P > 0.05; Fig. 2 B). Nomogram Construction The data for patients who did not receive ACT after PSM were included in the Cox risk proportion model (Table 2 ). Univariate analysis showed that grade, history, ypT stage, ypN stage, and ELN count were correlated with OS (P < 0.05). Further incorporating these variables into multivariate analysis showed that histology, ypT stage, ypN stage, and ELN count were independent prognostic factors (P < 0.05). A nomogram prediction model was generated on the basis of these data to predict the three- and five-year survival rates of GC patients after surgery (Fig. 3 ). The nomogram calibration curve for the three- and five-year OS showed that the predicted survival probability was in good agreement with the actual survival probability (Fig. 4 A, B). The DCA of the nomogram were higher than that of ELN count system (Fig. 4 C, D). Patient Risk Stratification System All patients were divided into three risk groups by the nomogram and X-tile software as follows (Table 3 ): low-risk (score ≤ 153, n = 295), moderate-risk (score 154–221, n = 322), and high-risk (score ≥ 223, n = 115) groups (Fig. 5 ). The five-year survival rates for the low-, moderate-, and high-risk groups were 70.7%, 36.4%, and 6.7%, respectively. The difference was not statistically significant (P > 0.05, Fig. 2 C). Using the existing scoring system, the non-ACT group was also divided into high-, moderate-, and low-risk groups and found that their five-year survival rates were 72.7%, 31.9%, and 4.5% respectively, with no statistically significant difference (P > 0.05, Fig. 2 D). The existing scoring system was also used to divide the ACT group into high-, moderate-, and low-groups and found that their five-year survival rates were 68.0%, 40.9%, and 12.8%, respectively, with no significant difference (P > 0.05, Fig. 2 E). The median patient survival time was 43 months (0–78 months), and the number of deaths was 90 (44.6%). There was no significant difference in the prognosis between the ACT group and the non-ACT group (five-year survival rate: 60.2% vs. 54.7%, P > 0.05, Fig. 6 A). All patients were divided into three risk groups based on the existing risk scores as follows: low-risk (score ≤ 153, n = 106), moderate-risk (score: 154–221, n = 34), and high-risk (score ≥ 223, n = 62) groups. The five-year survival rates for the low-, moderate-, and high-risk groups were 75.3%, 33.8%, and 6.8% respectively, with no statistically significant difference (P > 0.05, Fig. 6 B). The non-ACT group was divided into low-risk (n = 73), moderate-risk (n = 17), and high-risk (n = 25) groups based on the existing scoring system. The five-year survival rates for the three groups were 78.0%, 20.0%, and 6.5%, respectively. The difference was not statistically significant (P > 0.05, Fig. 6 C). In the ACT group, the five-year survival rates for the low-risk (n = 33), moderate-risk (n = 17), and high-risk (n = 37) groups were 70.0%, 45.4%, and 10.4%, respectively, and the difference was not statistically significant (P > 0.05, Fig. 6 D). While patients in the low-risk (HR = 1.48; 95% CI: 0.64–3.42; P > 0.05; Fig. 6 E) and moderate-risk (HR = 0.45; 95% CI: 0.19–1.10; P > 0.05; Fig. 6 F) groups did not benefit from ACT, patients in the high-risk group did (HR = 0.53; 95% CI: 0.27–1.03; P < 0.05; Fig. 6 G). Discussion The five-year OS for patients with stage II GC reached 50%, and the survival rate for patients with stage III GC was < 20%. In order to improve the prognosis of patients with GC, NACT have achieved some results [ 12 – 14 ]. NACT can improve the resection rate of surgery, control or even eliminate micrometastasis before surgery, and reduce the possibility of recurrence after surgery by lowering the stage of patients with advanced GC [ 15 – 18 ]. In 2006, the MAGIC study included 503 resectable GC patients, 250 of whom received NACT plus surgery, and 253 received just surgery. The results showed that the NACT group was superior to the surgery group in terms of five-year OS (HR = 0.75; 95% CI: 0.60–0.93; P = 0.009) and PFS (HR = 0.66; 95% CI: 0.53–0.81; P < 0.001) [ 6 ]. The MAGIC research has established the important position of NACT in GC. However, only 66% of patients have completed ACT, and whether the ACT really affects the patient's OS remains unknown. The present study included patients who had received NACT and completed surgery, analyzed the pathological factors affecting patient prognosis, scored the pathological factors using a nomogram, and screened the patients who really needed ACT according to the risk of recurrence for each patient. A meta-analysis of 58 studies by Zhao et al. in 2021 showed that the prognosis of signet ring cell carcinoma was better in early GC (HR = 0.59; 95% CI: 0.45–0.79; P = 0.003) and worse in advanced GC (HR = 1.19; 95% CI: 1.13–1.27; P < 0.05) [ 19 ]. Our study found that the prognosis of signet ring cell carcinoma was worse (HR = 2.131; 95% CI: 1.475–3.079; P < 0.001). We included patients with resectable advanced GC, which was consistent with the results by Zhao et al. With the progress of T and N stages, the overall GC stage has also progressed from stage I to stage IV, and the prognosis has become worse [ 20 ]. Our research also showed that T and N stages are factors of poor patient prognosis. Lymph node metastasis is the most important mode of GC metastasis. It is also the reason why GC is difficult to cure completely. The current N stage mainly focuses on the number of metastatic lymph nodes. However, due to the different methods of lymph node dissection by surgeons, the number of lymph nodes to be cleaned, and the variation in the lymph node position in the specimen after being fixed, the error in the process of determining the status of lymph nodes is large [ 21 – 23 ]. Therefore, new N staging methods, such as the metastatic lymph node ratio, negative lymph node count, and log odds of positive lymph nodes, have been proposed to reduce human error [ 24 – 30 ]. For stage III GC, Zhang et al. recommended detecting 31 ELN counts, and found that patient prognosis is good when the ELN count is > 31 (HR = 0.235; 95% CI: 0.125–0.444; P < 0.001). The OS for T4bN0MO patients is equivalent to that of T4bN1MO patients if the ELN count is < 31, suggesting that stage bias may occur [ 31 ]. Gu et al. found that the patient prognosis is good in patients with N0 (HR = 0.644; 95% CI: 0.516–0.804; P 15 [ 32 ]. Our study likewise found that patients with an ELN count of > 15 had a relatively good prognosis (HR = 0.595; 95% CI: 0.408–0.865; P = 0.007). One prior study evaluated 4,392 patients with resectable upper GC. All patients completed NACT, 732 patients received ACT, while 3,660 patients were treated using observation strategies. The results showed that ACT improved the patient prognosis (HR = 0.79; 95% CI: 0.72–0.88; P < 0.01) [ 17 ]. Coimbra et al. found that after receiving NACT, ACT improved patient survival in patients with advanced GC (HR = 0.55; 95% CI: 0.33–0.91; P = 0.019) [ 33 ]. It remains controversial whether patients with resectable GC can benefit from NACT after surgery [ 7 , 8 ] Therefore, the present study analyzed the high-risk factors that affect patient prognosis, scored them, and grouped them to screen the individuals who can benefit from ACT. The study results showed that only the high-risk groups could benefit from ACT (HR = 0.61; 95% CI: 0.38–0.97; P < 0.05), while the external database also verified this result. A meta-analysis showed that the prognosis of GC patients receiving NACT was better than that of patients receiving ACT (HR = 0.74; 95% CI: 0.61–0.89; P < 0.01). Due to the phase-lowering effect of preoperative NACT, most of the benefits in GC patients receiving NACT came from preoperative treatment, while the effect of postoperative treatment was unknown [ 34 – 36 ]. By scoring the high-risk factors that affect patient prognosis and recurrence risk, they were divided into low-, moderate-, and high-risk groups. Our study found that low- and moderate-risk patients could not benefit from ACT, while high-risk patients could (HR = 0.61; 95% CI: 0.38–0.97; P < 0.05). The principle of ACT is to reduce the local and remote recurrence in patients, thereby improving their prognosis [ 37 ]. Our research results showed that only the high-risk patients had a relatively high recurrence rate, suggesting that they may need ACT. Low- and medium-risk patients may encounter more side effects than benefits from chemotherapy and may experience excessive treatment, which is why ACT is not recommended. Our study has several limitations. First, this was a retrospective study and more RCT s or prospective studies will be necessary in the future. Moreover, as the database fails to provide specific data for NACT or ACT schemes, efficacy, and time, there may be some differences between the database and external validation data, which may lead to bias. However, there is presently no survival prognosis model for resectable GC patients receiving NACT incorporating the above clinical and pathological factors. Our study’s innovation lies in the risk stratification of patients using this model, which is of great significance to guide individualized ACT. Since there is no conclusive evidence showing whether GC patients receiving NACT will receive ACT after surgery and because patient compliance after surgery is poor, it is difficult to carry out large-scale RCTs and prospective research, which also highlights the importance of the present work. Conclusion Our nomogram can effectively predict the prognosis of resectable GC patients receiving NACT. ACT is thereby recommended for high-risk patients. Among them, 366 patients received postoperative chemotherapy and 506 patients did not. The propensity score matching was used to balance the two groups of confounding factors, the Kaplan\u0026ndash;Meier method was used for survival analysis, and the logrank test was used to compare the differences between the survival curves. The Cox proportional hazards regression model was used to screen independent prognostic factors and establish a nomogram survival prediction model. The patients were divided into high-, moderate-, and low-risk groups according to the overall survival prediction score generated by X-tile software based on the nomogram.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003eMultivariate analysis showed that the independent prognostic factors of gastric cancer in the group not receiving chemotherapy were history, ypT stage, ypN stage, and examined lymph node count, which were included in the nomogram prediction model. The C-index for the model was 0.727 (95% confidence interval, 0.65056\u0026ndash;0.80344). The patients were divided into three different risk level groups based on the nomogram prediction score.
