Abstract

異所性子宮内膜症は比較的稀であり,外科的治療の対象となることは少ない.著者らはS状結腸に発生した異所性子宮内膜症を腹腔鏡補助下手術にて切除した一例を経験したので報告する.症例は42歳の女性で,月経時下血を主訴に近医を受診した.近医での大腸内視鏡と注腸ではS状結腸に高度の狭窄像を認め,生検で異所性子宮内膜症と診断した.約6カ月間のホルモン療法に抵抗性であったため,外科的治療の適応と考え,術後早期退院を目標とし腹腔鏡下で両側付属器切除,S状結腸部分切除を施行した.病理組織学所見では,結腸の筋層に浸潤する子宮内膜の増生を認め,異所性子宮内膜症と診断された.術後間痛も軽度であり,早期離床可能であった.術後は周期的な下血や腹痛は認めておらず,軽度の更年期症状を呈するのみで経過は良好であった.
