Abstract

Abstract Objective purpose of this study was to investigate the various forms of cataracts, associated risk factors, and their impact on VA [VA] in patients who visited the outpatient clinics and ophthalmology department at Damascus Hospital [Al-Mujtahid]. Methods and materials This is a retrospective study that included 202 patients diagnosed with one of types of cataracts during a period extending from 1-7-2019 and 31-1-2020. Statistical analysis was conducted using Excel and SPSS-28, study data was analyzed according to age, gender, risk factors, types of cataracts, and risk factors: VA was used to categorize patients into four groups: [1 or less/10], [2–4/10], [5–7/10], and [8–10/10]. Patients were divided into four age groups [12–30 years], [31–50 years], [51–70 years], [older than 70 years], effect of bilateral and unilateral cataracts on VA was evaluated in both eyes. Results This study included 202 patients, there were 464 cataract cases [250 unilateral and 214 bilateral] that had more than one type of cataract. The majority of patients [60.4% of all patients] were between 51 and 70, followed by those older than 70 [22.8% of all patients], male/female ratio: 51.5%/48.5%. Diabetes and Hypertension are the most common risk factors for cataracts [36.6%, 30.7%, respectively], most common types of cataracts in 51–70 years, males and females are [unilateral and bilateral posterior subcapsular cataract], followed by [unilateral and bilateral nuclear cataract], which are the most to affect VA in both eyes, and are the most common among current smokers. Conclusion When a mature cataract affects only one eye, as opposed to both, the impact on VA is larger in the unilateral case. Both unilateral and bilateral mature cataracts in left eye affect VA in the same way. Compared to unilateral posterior subcapsular cataract, bilateral posterior subcapsular cataract impairs VA more.

Furthermore, cataract has a major load on economy and public health services due to increased aging population and increased cataract surgery services [ 14 ]. People over 50 years old are the most affected and the risk increases significantly with age. To clarify, cataracts are present in 20% of people between 65–74 years old and over 50% of those older than 74 have cataract [ 15 ]. Cataract types are mainly divided to nuclear, cortical, posterior sub-capsular, mature and hypermature (not a type but the final stage for all cataracts) and congenital [ 16 ]. Surgery is still the only treatment for cataracts, and when done well, it can reduce the rates of blindness [ 16 ] which is composed of the removal cloudy eye lens and then substituting it with a synthetic lens. This procedure is widely performed over the world, particularly in Germany, where about 800,000 people get it done each year [ 17 ]. Visual acuity (VA) is the major parameter for testing the quality of vision, and it is most commonly tested in our country using Snellen chart [ 18 ] (Fig. 1 ). Methods Materials This is a retrospective study included patients who were diagnosed with cataract at department of ophthalmology and Outpatient Clinics at Damascus Hospital [Al-Mujtahid], during 1-7-2019 and 31-1-2020. In total, 202 male and female patients with cataract diagnosis were included in this study, there were 464 cataract cases [250 unilateral and 214 bilateral] that had more than one type of cataract. Study data were analyzed according to age, gender, and risk factors. Patients were divided into four age groups: [12–30 years], [31–50 years], [51–70 years], and [older than 70 years], for the whole study population, percentage of cataracts of all type was examined. We reviewed each type of cataract and its frequency in each age group and in both males and females. Medical and smoking history were studied to evaluate risk factors for cataract. VA of both eyes was assessed using a letter board [Snellen chart] [Figure 1 ], which is defined as the patient's ability to recognize letters or numbers on a standard Snellen chart at a specified viewing distance. Patients were divided according to VA into 4 groups: [1 or less/10], [2–4/10], [5–7/10] and [8–10/10]. Effect of smoking on cataracts was assessed, and patients were divided into three categories: smoker, current smoker and Ex-smoker [must have quit smoking for 1 year]. We evaluated the distribution of unilateral and bilateral cataract types, and the effect of cataract [unilateral or bilateral] on VA in both eyes. All patients with [diabetic retinopathy, hypertensive retinopathy, retinal dystrophy and patients with corrected vision] were excluded, regardless of the patient's age. Data was analyzed by Excel and IBM SPSS-28 Statistics, Chi – Square was used to evaluate relationship between variables, and considered p. Value < 0.05 is statistically significant. Results Demographic variables: Number of patients in our study is 202 patients, number of cataract cases with more than one type of cataract is 464 eyes. Most prevalent age group in our study was [51–70] 60.4% of all patients, followed by those [older 70 years] 22.8% of all patients, male/female ratio: 51.5%/48.5%. 19.3% of patients are current smokers [n = 39], 26.2% of patients are Ex-smokers [one year after quitting smoking] [n = 53]. Moreover, both unilateral and bilateral posterior subcapsular cataracts showed a decline in VA [P 0.02]. Additionally, unilateral intumescent cataract is common among non-smokers, but bilateral intumescent cataract is common among current and EX- smokers [P = 0.001]. Cataract has been identified as the leading cause of visual impairment and blindness, with statistics ranging from 33.3% in Denmark to 82.6% in India. Published data indicates that 1.2% of the total population of Africa is blind, with 36% of this blindness attributable to cataract. In a survey conducted in three regions in the Punjab plains, the overall prevalence rate of cataract was 15.3% among 1269 individuals older than 30 years and 4.3% across all age groups. This rate significantly increased to 67% for individuals older than 70 years. Analysis of blind registration data in West Scotland has shown that cataract is one of the four main causes of blindness. In our study, the majority of patients were between 51–70 years old [60.4% of total Patients], followed by patients older than 70 years [Table 1 ], similar to [study of Haddeh] between 1-6-2017 and 31-12-2017[ 8 ] and similar to [study of Al homsi] between 1-10-2019 and 30-9-2020 [ 9 ]. The presence of unilateral and bilateral nuclear cataracts is more common in 51–70 age group, [p = 0.000]. Unilateral hypermature cataracts are more common in 31–50 age group, while bilateral hypermature cataracts is as much common as in age group 51–70 years and older than 70 [p = 0.011]. In our study, the proportion of females [48.5%] was lower than that of males [51.5%][Table 1 ], similar to findings of [Haddeh] and [Al homsi] studies. Unilateral Cortical cataracts was more common in males, while bilateral cortical cataracts was more common in females, [P = 0.024]. According to various published studies, females are more susceptible to most types of cataract due to hormonal changes after menopause [i.e., decreased estrogen levels after menopause] [ 10 ]. However, Martinez et al. did not observe a difference between genders in their analysis [ 12 ]. The most common risk factor of cataracts is diabetes [36.6%], followed by hypertension [30.7%], then cardiovascular diseases, thyroid diseases, steroid use, presence of eye trauma, and Hyperlipidemia [10.4%, 8.4%, 7.9%, 6.4%, 5% respectively]. 31.2% of patients have cataract of unknown cause. The main types of cataracts are cortical, subcapsular, and nuclear [ 8 , 9 ]. We found 69, 76 cases of unilateral and bilateral nuclear cataracts respectively, 40 ,38 cases of unilateral and bilateral cortical cataracts respectively, 77, 86 cases of unilateral and bilateral posterior subcapsular cataracts respectively, and 15, 5 cases of unilateral and bilateral anterior subcapsular cataracts respectively [Table 3 ]. However, the presence of mature, hypermature, and intumescent cataracts suggested delayed diagnosis and were presented within the results for comparison with other types of cataract according to age and gender to determine the percentage of patients referred to the hospital in advanced stage of the disease. Best corrected visual acuity BCVA result was 10/10 according to the Snellen scale. We divided VA into four categories: [1/10 or less], [2–4/10], [5–7/10] and [8–10/10]. Effect of cataracts on VA at right eye: is found 69 ,76 cases of unilateral and bilateral nuclear cataracts respectively, with 28 ,29 cases respectively having VA less or 1\10, and 22 ,30 eyes respectively having VA of 2–4/10. [P = 0.693], 40, 38 cases of unilateral and bilateral cortical cataracts respectively, with 18 ,13 eyes respectively having VA less than or equal to 1, and 13, 19 eyes respectively having VA of 2–4/10,[P = 0.113]. Additionally, 77, 86 cases of unilateral and bilateral posterior subcapsular cataracts respectively, with 31, 32 eyes respectively having VA less than or equal to 1, and 24, 37 eyes respectively having VA of 2–4/10,[ P = 0.395]. also is found 15, 5 cases of unilateral and bilateral anterior subcapsular cataracts respectively, with 5, 1 eyes respectively having VA less than or equal to 1, and 6 ,2 eyes respectively having VA of 2–4/10, [ P = 0.475]. Effect of cataract on VA at left eye: It is found 69, 76 unilateral and bilateral nuclear cataracts respectively, 27, 39 eyes respectively having VA less or 1\10, and 24 ,26 eyes, respectively having VA of 2–4/10 [P = 0.11]. Additionally 40, 38 unilateral and bilateral cortical cataracts respectively, with 18, 16 eyes respectively having VA less or equal to 1, and 10, 16 eyes respectively having VA of 2–4/10 [P = 0.457]. Furthermore, we had 77, 86 cases of unilateral and bilateral posterior subcapsular cataract respectively with 27, 43 eyes respectively having VA less than or equal to 1/10 and 26 ,24 eyes respectively having VA of 2–4/10 [ P = 0.02]. Also, it is found 15, 5 cases of unilateral and bilateral anterior subcapsular cataracts respectively with 7 ,4 eyes respectively having VA less than or equal to 1\10, and 5, 0 eyes respectively having VA of 2–4/10 [ P = 0.751]. These results are consistent with those of Haddeh and Al homsi. The presence of VA less than or equal to 1\10 and 2–4/10 indicates the presence of more than one type of cataract in the same eye. Martin proved that in terms of visual acuity, the effect of cortical cataract on VA is less than the effect of nuclear cataracts, which is consistent with our findings. Most cases of mature, hypermature and intumescent cataract had very poor VA [1 or less /10], which is reasonable and logical as these types represent the final stage of cataract when the lens becomes completely white and swollen. Usually, the effect of cortical cataract on VA is minimal [ 9 ]. Effect of smoking on development of cataract: Patients were divided into three groups: [smoker, current smoker, Ex-smoker (must have been abstinent from smoking for a year)]. Unilateral mature cataract was common in smokers, while bilateral mature cataract was common in Ex-smokers [P = 0.039]. Also, the percentage of unilateral intumescent cataract was higher in non-smokers, while bilateral intumescent cataract is common in current and Ex-smokers [P = 0.001]. Smoking leads to changes in the lens cells through oxidation and contributes to accumulation of heavy metals in the lens, which leads to development of cataract [ 11 ], similar to other retrospective studies, we encountered difficulties with data collecting as a result of inadequate data archiving. Conclusion The most common type of cataract is posterior subcapsular cataract, followed by nuclear cataract. Unilateral and bilateral nuclear cataract are most common in the group age of 51–70 years. Unilateral hypermature cataract is most common in 31–50 years and 51–70 years, while hypermature cataract is most common in 51–70 years, and older than 70 years. Unilateral cortical cataract is more common in males, while bilateral cortical cataract is more common in females. In the right eye: a unilateral mature cataract has a greater effect on visual acuity, while if it is bilateral, its effect is less. In the left eye: unilateral and bilateral mature cataracts have the same effect on visual acuity. Bilateral subcapsular cataracts have a greater effect on VA than unilateral posterior subcapsular cataracts. Unilateral mature cataract is more common in nonsmokers, while bilateral intumescent cataract is more common in nonsmokers and Ex- smokers. The main types of cataracts are cortical, subcapsular, and nuclear [\u003cspan citationid=\"CR8\" class=\"CitationRef\"\u003e8\u003c/span\u003e, \u003cspan citationid=\"CR9\" class=\"CitationRef\"\u003e9\u003c/span\u003e]. We found 69, 76 cases of unilateral and bilateral nuclear cataracts respectively, 40 ,38 cases of unilateral and bilateral cortical cataracts respectively, 77, 86 cases of unilateral and bilateral posterior subcapsular cataracts respectively, and 15, 5 cases of unilateral and bilateral anterior subcapsular cataracts respectively [Table \u003cspan refid=\"Tab3\" class=\"InternalRef\"\u003e3\u003c/span\u003e]. However, the presence of mature, hypermature, and intumescent cataracts suggested delayed diagnosis and were presented within the results for comparison with other types of cataract according to age and gender to determine the percentage of patients referred to the hospital in advanced stage of the disease. Best corrected visual acuity BCVA result was 10/10 according to the Snellen scale. The majority of patients [60.4% of all patients] were between 51 and 70, followed by those older than 70 [22.8% of all patients], male/female ratio: 51.5%/48.5%.
