Clinical and immunological aspects of early reproductive losses in patients with adenomyosis

This study assessed clinical and immunological features of early pregnancy in 110 women with first spontaneous pregnancies at 6–9 weeks, comparing 43 women with grade 1 adenomyosis and threatened miscarriage, 32 with grade 1 adenomyosis without threatened miscarriage, and 35 women without an adenomyosis history. Using cytokine profiling of peripheral blood and cervical mucus and measuring the antimicrobial peptide LL-37, the authors found that physiologically progressing first-trimester pregnancies with grade 1 adenomyosis showed a pro-inflammatory cytokine shift and increased proangiogenic activity (VEGF-R2/VEGF-A ratio), whereas threatened miscarriage in grade 1 adenomyosis was associated with significant changes in the cytokine profile, decreased proangiogenic activity, and higher LL-37 in cervical mucus. The authors note that the findings may reflect chronic endometritis not detected by noninvasive methods, and require further in-depth study.
