Intérêt du drainage pelvien prophylactique dans la prise en charge chirurgicale de l’endométriose

Abstract

The objective was to evaluate the value of prophylactic pelvic drainage in the surgical management of deep endometriosis. We conducted a retrospective study comparing 176 patients with prophylactic drainage to 138 patients without drainage. For each patient, the surgical data collected included whether or not a digestive resection was performed and the type of resection possible (segmental or discoid). The primary endpoint was the occurrence of an early post-operative complication. We also analysed the length of stay, the occurrence of voiding dysfunction, surgical resumption, hospital readmission, reconsultation in emergency departments, and use of post-operative analgesics. The role of prophylactic pelvic drainage in reducing the post-operative complication rate after rectal surgery remains controversial. Most studies have been conducted in oncologic surgery, but few publications especially concern the surgical management of endometriosis, while the clinical situations of these two surgeries are very different. Prophylactic pelvic drainage was associated with more post-operative complications (31% vs. 7%). Post-operative digestive (7%, 13/176) and urinary (22%, 39/176) complications were most often associated with pelvic drainage. The overall reintervention rate was 3.5% (11/314), significantly associated with more drainage (5.7%, 10/176). No differences were observed in terms of readmission to hospital or emergency department visits. Patients with prophylactic pelvic drainage had more post-operative complications.
