This thesis reviews the prevalence and embryological origins of Mullerian anomalies, which affect 4-7% of women and often present with symptoms such as chronic pelvic pain, infertility, and endometriosis. It outlines standard surgical interventions including hysteroscopic metroplasty for septate uterus and laparoscopic approaches for bicornuate or didelphys uteri to improve fertility outcomes. The work notes that while removing a vaginal septum is beneficial, leaving a cervical septum may be preferable in certain cases to maintain function. Relevance to endometriosis: listed as one indication for GnRH antagonists, though the paper's main focus is uterine fibroids.

Abstract

Mullerian anomalies of the female genital tract have a prevelance between 4-7% in the general population. They are defined as deviations from normal anatomy and are the result of embryological maldevelopment of the paramesonephric ducts. Teenage girls and women experience a variety of symptoms due to these anomalies ranging from chronic pelvic pain to endometriosis and infertility. Surgery for correcting these anomalies is mainly indicated to resolve fertility problems specifically recurrent ones.In cases of severe abdominal pain too removal of the non functional organ is recommended. The classic surgical approach for Mullerian anomalies is: 1) hysteroscopic metroplasty, 2) abdominal metroplasty, 3) hemi hysterectomy and 4) vaginoplasty. The results of surgery are: 1) for septate uterus good fertility outcome as compared with poor outcome in the uncorrected septum, 2) for bicornuate, didelhpys and arcuate uterus there is increased fertility and pregnancy outcome after surgery and 3) if vaginal septum is present is best to remove it, but leave the cervical septum if any.

| | | Supervisor: | Σαρδέλη, Χρυσάνθη Αναπληρώτρια καθηγήτρια | | Advisory / Examination Committee: | Δανιηλίδης, Άγγελος Αναπληρωτής καθηγητής Γκρόζου, Φανή Επίκουρη καθηγήτρια Πανεπιστήμιο Ιωαννίνων | | Defense Date: | 07/11/2022 | | Keywords: | Συγγενείς ανωμαλίες της μήτρας (του Muller), Mullerian anomalies of the uterus, Hysteroscopic & Laparscopic metroplasty, Υστεροσκοπική και Λαπαροσκοπική μετροπλαστική, Αποτελέσματα γονιμότητας, Fertility results | | DOI: | 10.26262/heal.auth.ir.343624 | | Related files: | Fulltext: PDF File with license: | Record created 2022-12-21, last modified 2022-12-29 - Add to personal basket - Export as BibTeX, MARC, MARCXML, DC, EndNote, NLM, RefWorks - Send to Mendeley - Share via:
