Methods The literature related to the nursing of oral cancer patients in the core database of the Web of Science was systematically searched from January 1, 1978 to May 27, 2024. CiteSpace6.1.R6 software was used to visualize the authors, institutions, co-citations and keywords of the included documents. Results A total of 1154 literatures were included, and the number of published literatures showed an increasing trend. The United States had the highest number of publications, followed by the United Kingdom and China. The Journal with the most citations was the Journal Of Clinical Oncology (395 times), while the journal with the highest centrality of citations was The Lancet (0.13). The top five keywords were cancer (224 times), chemotherapy (154 times), care (126 times), quality of life (124 times) and management (123 times). A total of 8 significant clusters were formed, which could be summarized into ① different treatment methods and schemes; ② prevention of oral cancer; ③ symptomatic treatment of cancer pain; ④ prevention and treatment of complications. The research trends and directions in recent years mainly include education, outcome, impact and dysphagia, especially dysphagia. Conclusion The contents of nursing research on patients with oral cancer are rich, mainly in treatment methods; Prevention; Treatment of cancer pain and prevention of complications. Current research trends focus on education, outcomes, impact, dysphagia, and specifically dysphagia. Oral cancer Nursing CiteSpace Visual analysis Figures Figure 1 Figure 2 Figure 3 Figure 4 Figure 5 Introduction Oral cancer is a relatively common malignant tumor in the head and neck, of which at least 80% belongs to squamous cell carcinoma (squamous cell carcinoma), that is, the mucous membrane is mutated, and its incidence sites include lip, tongue, salivary gland, gum, floor of mouth, oropharynx, buccal mucosa and other parts in the mouth. It is one of the most common malignant tumors in the world, The five-year survival rate for it is about 50% [1-3] , and even lower in the elderly [4] . According to The Global Cancer Statistics in 2020, there were 377,713 new cases and 177,757 deaths in 2020 [5] . Its incidence varies widely around the world, with Asia, Europe, and Oceania having the highest incidence in the world, while Asia has the highest mortality rate [6] . Its cause may be related to bad lifestyle habits (such as smoking, drinking, chewing betel nut, etc.), or may be caused by human papillomavirus infection [7] 。In the early stage of oral cancer, surgery is the main method, while in the middle and late stage, chemotherapy, radiotherapy, cryotherapy and other methods are combined [8, 9] . Due to the surgery or the anatomical location of oral cancer, patients with oral cancer may suffer from slurred speech, dysphagia, eating disorders and changes in facial appearance, which not only seriously affected the quality of life and nutritional supply of the patients [10-13] , but also caused the patients to have anxiety, depression and other negative emotion s [14] 。Therefore, nursing care for patients with oral cancer is particularly important, which can not only help repairing wound, preventing infection and reducing complications, but also help patients recovering their speech and swallowing functions , improving their psychological condition and improving their quality of life as much as possible [15] . As a knowledge graph visualization software, CiteSpace can effectively manage information, with a focus on quantification, Citespace can identify research hotspots, major contributors and cooperative networks from massive data information, and present them in a visual form. Citespace plays a vital role in formulating guidelines in the research field, mining research hotspots and evaluating research trends. It is therefore becoming increasingly important in assessing research trends and developing guidelines [16] . This study will use CiteSpace software [16] to explore the development history of oral cancer patient nursing care for visual analysis, provide a clearer perspective for the cutting-edge analysis of oral cancer patient care scientific research, reveal research trends, and provide a clearer understanding of the development of this discipline, provide references for nurses and researchers. 1. Materials and methods 1.1 Materials Advanced search strategies were used to collect literature from the core collection of Web of Science databases widely used in bibliometric analysis ( https://www.Web of Science.com). According to the MeSH content provided by the National Center for Biotechnology Information (NCBI), the search terms are set as follows: Keywords: Mouth Neoplasm (MeSH) /Neoplasms, Oral/Cancer of Mouth/Oral Cancer/Cancer of the Mouth/Nursing (MeSH). Web of Science Core collection search: TS= (Mouth Neoplasm) OR (Neoplasms, Oral) OR (Cancer of Mouth) OR (Oral Cancer) OR (Cancer of the Mouth)) AND (nursing). The literature on the nursing care of patients with oral cancer was first published in 1978, so the search time of this study was set from 1978 to 2024, except for the publication time and country analysis, the rest of the literature analysis years were nearly 20 years, so the time period was limited from 2003 to 2024,, and the search date was 2024-5-27. The language was "English", and the types of documents that did not meet the research requirements were excluded. 1.2 Inclusion and exclusion criteria Inclusion criteria: ① Literature related to the field of nursing care for patients with oral cancer; ② The types of articles were research papers and reviews. Exclusion criteria: ① The types of articles were conference abstracts, dissertations, reports, achievements, newspapers, etc. ② Duplicate literature; ③ The author, keywords and other information were missed. 1.3 Methods The relevant literature, including the reviews, were retrieved from the core collection of Web of Science, and converted into a processable format with CiteSpace 6.1.R.6 software.The literature analysis were carried out after removing the duplicate literature. The time slicing was selected for 1 year, the Likelihood Ratio (LLR) method was selected for clustering, Pathfinder and Pruning sliced networks were selected to generate visual data images. Retain the default values for other parameters. The annual number of publications, countries and institutions, authors and cooperative relationships were analyzed. The co-citations of literature were analyzed. The keywords were sorted according to the frequency of occurrence, and the top 50 keywords were extracted for cluster analysis. Analyzed the burst keywords and emerging topics and trends. The whole process of literature search and analysis were carried out by two researchers independently. When opinions divergence, they were decided after discussion to ensure the accuracy of the results. 2 Results A total of 1451 literatures were retrieved in the core collection of Web of Science, and 1154 remained after the removal of duplicate literatures by CiteSpace. A total of 1005 literatures were published from 2003 to 2024. 2.1 Document quantity characteristcs and global distribution characteristics As shown in Fig. 1 , the overall number of published papers were less than 10 from 1978 to 1994, gradually increased from 1994 to 2006 and remained at 10–21, and increased sharply in 2007, 2016 and 2021. A peak of published papers appeared in 2021, with 92 published papers per year. In order to more accurately analyze the global distribution of the number of publications, this study focused on the analysis of the number of publications published in English by 1,154 scholars from different countries in this field, and found the top 10 countries/regions with the most contributions (see Table 1 ). Further using Citespace to visualize the distribution of global publications in countries/regions (Fig. 2 ), we set the threshold of the minimum number of publications in a country to 10, a total of 201 countries, of which 20 countries reached the threshold. The United States had the highest number of publications with 413, followed by the United Kingdom and China with 84 and 66. The top 10 distribution of research publications in the field of oral cancer nursing care in the global English literature were shown in Table 2 . Harvard University (38 articles) and Brigham & Women's Hospital (36 articles) were the most productive institutions. The authors with the highest number of publications were Hankinson SE and Tworoger SS, who published 21 and 20 articles respectively, far exceeding the other authors. The journal with the highest centrality of citations was The Lancet (centrality = 0.13,frequency = 214). The top 10 most frequently cited journals and their centrality were shown in Table 3 . The most frequently cited single article was an article on a systematic review of treatment compliance published by The Oncologist in 2016 (28 times). The highest centrality of a single paper with was published by Revista Latino-Americana de Enfermagem in 2015 (0.15). SONIS ST and PARTRIDGE AH had the highest frequency of co-cited authors (56 times), while PARTRIDGE AH still had the highest centrality of co-cited authors (0.46). Using Citespace for the network visualization of keywords, when the minimum threshold of keyword occurrence was set to 10 times, there were 148 keywords reached the threshold, and when the minimum threshold of keyword occurrence frequency was set to 35 times, there were 34 keywords reached the threshold, as shown in Fig. 3 . Based on the keyword co-occurrence network, the LLR algorithm was used for the clustering analysis of keywords. The specific clustering module information were shown in Table 5 , where the keyword clustering modulus Q = 0.4455 > 0.3 and the weighted silhouette value S = 0.5968 > 0.5, indicating that the clustering results were significant and reliable, and the consistency of cluster members were high. The literature forms 8 clusters through circles and labels. The clustering were based on the time axis, as shown in Fig. 4 . Different colors identified different categories. These included 0#carboplatin, 1#cryotherapy, 2#oral chemotherapy, 3#hpv vaccination, 4#sedation, 5#transdermal fentanyl, and 6#pulmonary embolism, 7#density lipoprotein cholesterol. We used Citespace to detect burst keywords to determine research frontiers, we found that the top 13 burst keywords were shown in Fig. 5 . The burst keyword “dysphagia”, which had recently emerged in 2022 and continued to be hot until now, was expected to continue to become a research hotspot in this field. The rapid rose in the number of published papers might be related to the implementation of policies related to stomatology and the increasing attention of the public to stomatology. The care of oral cancer had also attracted the attention of scholars around the world. After 2021, considering the factors such as the global economic downturn caused by the COVID-19 epidemic and the relatively dispersed research hotspots, etc, the number of papers in this field had decreased slightly, and the world may need to actively find new opportunities and innovative points to stimulate the growth of the total amount of the research. Under the increasing high attention, the future of oral cancer nursing care is bright. In terms of the distribution of publishing countries and institutions, developed countries led by the United States and the United Kingdom have a high number of publications. Although China had the top three publications, its centrality was not very prominent, only 0.05, indicating that China's literatures influence were weak and its cooperation with other countries was not very close, which cannot reach the level of developed countries such as Canada = 0.09 and Italy = 0.08. The institution with the highest publication in China is Sun Yat-sen University, which ranks 11th with 8 publications. Therefore, China also needs to conduct more in-depth expansion and frontier mining in the field of oral cancer nursing care, and strengthen exchanges with the international level. From the high cited documents, Journal Of Clinical Oncology, as a journal specialized in oncology, had the highest co-citations frequency, while The Lancet, as one of the most authoritative comprehensive medical journals allover the world, had the highest centrality, indicating that it had the greatest influence among all co-citations journals. 3.2 Research hot spots and trends As can be seen from the co-occurrence analysis of key words, hot topics of research included: ①cancer / chemotherapy. Oral cancer is a kind of malignant tumor that is more sensitive to chemotherapy in head and neck tumors, some patients needed to use chemotherapy before or after surgery for comprehensive treatment, Although more and more new patients were diagnosed, most patients were still already in middle or terminal stage when initially diagnosed [ 17 ] , for patients in terminal stage, radiotherapy and chemotherapy were mainly used, especially for patients who have lost the opportunity for surgery [ 18 ] ; ②care / quality of life/management. The mass and operation of oral cancer would partially destroy the functions of the patient's mouth, throat and neck, resulting in oral dysfunction, facial deformity, difficulty in chewing, difficulty in swallowing, speech dysfunction and respiratory dysfunction, thus leading to a decline in quality of life. Therefore, scientific management and nursing care were needed to help the patient gradually recover some functions as much as possible, minimized the inconvenience caused by disease to life, improved the quality of life. This was consistent with the research of Chang Wei in China [ 19 ] ; ③breast cancer / therapy / adherence / cancer patient, Since many researchers included all cancer patients, and oral cancer treatment (Oral targeted drug therapy) are the first-line treatment in metastatic breast cancer(MBC), so the keyword here appeared “breast cancer” [ 20 ] . In addition, there were other keywords such as "therapy / adherence / cancer patient" and so on. According to the results of keyword cluster analysis, the research focused on ① different treatment methods, including chemotherapy, cryotherapy, oral chemotherapy and so on: 0#carboplatin / 1#cryotheqrapy / 2#oral chemotherapy, the clustering keywords were“hepatotoxicity; asthma;oral cryotherapy; 5-fluorouracil༛self efficacy; quality improvement; multivariate; multidisciplinary team”, this was basically consistent with the results of keyword co-occurrence analysis ① and ②༛② Prevention of oral cancer: 3#hpv vaccination, the clustering keywords were“vaccination; hpv; dentist; granular cell tumor”, studies had reported that about 25% of oral cancers were associated with high-risk HPV types 16 and 18, which might be attributed to the similar epithelium between the cervix and oropharynx, which is squamous cell type [ 21 ] . HPV-associated oral cancers had been reported in Brazil [ 7 ] , India [ 22 ] , the United States [ 23 ] , the Middle East [ 24 ] and other places. The prevalence of oral HPV was increasing because high-risk sexual practices, such as sexual activities between oral and genital, result in transmission of the virus between the mouth and genitals [ 25 ] . In addition to adults, prevalence was also increasing among young adults and adolescents, suggested that there were other modes of transmission [ 26 ] ; ③ Symptomatic treatment of cancer pain: 4#sedation / 5#transdermal fentanyl, the clustering keywords were“death; palliative sedation; of life care; heart failure; palliative care unit; embolization; rotation; patient controlled analgesia”, In perioperative and palliative treatment stage for advanced cancer, patients often felt unbearable pain, fear, and accompanied by corresponding symptoms, so they needed to use analgesics, anesthetics, etc. to control the symptoms and improve their quality of life [ 27 ] ༛④Prevention and treatment of complications; 6#pulmonary embolism / 7#density lipoprotein cholesterol, the clustering keywords were“oral anticoagulant therapy; c-reactive protein; weight gain; contraception; c-peptide; male; food frequency questionnaire; blood glucose”, Pulmonary embolism was a common complication of malignant tumors, and the incidence of pulmonary embolism was also high after major oral and maxillofacial surgery [ 28 ] , we need to do a good job of observation and prevention in advance. As can be seen from the bust keyword, the research trend began in 2003 with the research on cancer pain, the use of double blind, trial and other methods, followed by meta-analysis, and research on symptom. Therefore, the literature peaks occurred in 2007 and 2016 respectively. After the emergence of bust keywords, their popularity has continued to the present day, such as “education, outcome, impact, and dysphagia”, the strongest one is the “outcome”, while the latest one was “dysphagia” (2022). With the continuous progress of medical technology, people are gradually paying more attention to oral cancer. There are more and more studies on oral cancer education, its impact on patients' lives, and patient outcomes. Most patients with oral cancer had swallowing problems, especially patients in the acute stage after surgery and patients receiving radiotherapy. More than half of the patients have moderate to severe dysphagia [ 29 , 30 ] , dysphagia can seriously affected postoperative nutritional supply, sometimes it can cause food to regress into the trachea, resulting in cough, dyspnea, aspiration pneumonia, suffocate [ 31 ] . Patients should be given personalized swallowing rehabilitation in time to improve swallowing function, improve nutritional status and quality of life. 4 Conclusions In this paper, the literature related to oral cancer nursing care in WoS database was analyzed by metrology, and its research status, research hotspot and development trend were analyzed, so as to provide reference for further research on oral cancer nursing care. There are some limitations in this study, because only the relevant research of the core collection of Web of Science is included, and there may be a situation of missing literature in other databases. Due to the software's inability to cross-language analysis and other problems, the search language was limited to English, and no documents in other languages were included. Because of the wide range of search terms, there is a problem of reducing the accuracy while improving the recall rate. In the future, relevant research should expand the scope of domestic and foreign databases and literature in languages other than English, and improve the search strategy. In the future, the database sources can be expanded to include more high-quality literature, or a more comprehensive and systematic analysis can be combined with expert review opinions. Based on the keyword co-occurrence network, the LLR algorithm was used for the clustering analysis of keywords. CiteSpace6.1.R6 software was used to visualize the authors, institutions, co-citations and keywords of the included documents.\u003c/p\u003e\u003cp\u003e\u003cb\u003eResults\u003c/b\u003e\u003c/p\u003e \u003cp\u003eA total of 1154 literatures were included, and the number of published literatures showed an increasing trend. The United States had the highest number of publications, followed by the United Kingdom and China. The Journal with the most citations was the Journal Of Clinical Oncology (395 times), while the journal with the highest centrality of citations was The Lancet (0.13). The top five keywords were cancer (224 times), chemotherapy (154 times), care (126 times), quality of life (124 times) and management (123 times). A total of 8 significant clusters were formed, which could be summarized into ① different treatment methods and schemes; ② prevention of oral cancer; ③ symptomatic treatment of cancer pain; ④ prevention and treatment of complications.
