Methods A convenience sample of 299 stroke patients from a tertiary hospital in Henan Province (February-May 2023) was assessed using a general information questionnaire, the Perceived Benefits and Barriers to Exercise Scale, and the International Physical Activity Questionnaire-Short Form. Latent profile analysis identified distinct categories of perceived exercise behavior and ordered logistic regression analyzed the impact of these categories and demographic factors on physical activity levels. Results Perceived exercise behavior was categorized into three groups: high perceived barriers but satisfaction with the exercise environment (10.6%), moderate perceived benefits but concerns about time consumption (48.6%), and high perceived benefits but worries about social interaction (40.8%). A positive correlation was found between perceived exercise behavior and physical activity levels. Age and perceived exercise behavior category significantly influenced physical activity (both P < 0.05). Conclusion Stroke patients' perceived exercise behavior falls into three latent categories and serves as a modifiable factor affecting physical activity. Future interventions should tailor cognitive-behavioral strategies based on these categories to enhance patients' physical activity levels. Health sciences/Diseases Health sciences/Health care latent profile analysis stroke motion perception physical activity influencing factors Figures Figure 1 1 Introduction Non-communicable diseases (including heart disease, stroke, cancer, diabetes, and chronic respiratory diseases) and their causes of injuries account for more than 80% of all global deaths[ 1 ] with cardiovascular diseases accounting for the largest share[ 2 ] Stroke is the second leading cause of death and the third leading cause of disability among cardiovascular diseases after heart disease.[ 3 ] and is the leading cause of death and disability among Chinese adults.[ 4 ] It is also the leading cause of death and disability among Chinese adults. Lack of physical activity increases the risk of cardiovascular disease.[ 2 ] Regular aerobic exercise training reduces the medium- and long-term risk of cardiovascular death by 25%.[ 5 ] The risk of cardiovascular death is reduced by 25% in the medium and long term. There is sufficient evidence that regular exercise can improve cognitive deficits and promote physical rehabilitation after stroke, and exercise-based behavioral change is an important component of secondary stroke prevention.[ 6 ] Exercise-based behavioral change is an important component of secondary stroke prevention. The guidelines recommend that[ 7 – 8 ] Stroke patients should engage in 150–300 minutes of moderate-intensity exercise, or 75–150 minutes of vigorous-intensity physical activity, or an equivalent combination of moderate-intensity and vigorous-intensity aerobic physical activity per week. Studies have shown that[ 9 – 10 ] that about 78.76% of stroke patients had insufficient physical activity and only 17% of stroke patients met the internationally recommended standards for physical activity. Therefore, it is important to identify the factors influencing different levels of physical activity in stroke patients to promote the improvement of their physical activity levels and to fully utilize the added benefits of physical activity. The health promotion model (HPM) proposed by Pender states that[ 11 ] the selection and maintenance of health behaviors depend mainly on three aspects: personal traits and experiences, behavior-specific cognitions and emotions, and behavioral outcomes, of which behavior-specific cognitions and emotions include perceived behavioral benefits/barriers, perceived self-efficacy, interpersonal influences, social influences, and permissive activity schedules, which are considered modifiable factors affecting health behaviors and are also the targets of the HPM research.. Some studies have found[ 12 – 13 ] that perceptions of motor behavior are important factors influencing motor exercise behavior, and that patients' subjective perceptions of motor behavior drive their practice behaviors, and that understanding the barriers to motor behavior in stroke patients is essential for promoting their physical activity and for implementing motor exercise programs to improve their health outcomes. Current research on the perception of exercise behavior is mostly based on scale scores to determine the level of individual exercise perception and does not take into account the heterogeneous characteristics of exercise behavior perception problems in stroke patients. Categorizing individuals according to their perceived categories of motor behaviors can differentiate risk factors for people with different characteristics to propose more targeted interventions. Latent profile analysis (LPA) is a statistical method that explains the association between exogenous indicators through intermittent latent variables, this association is estimated by latent category variables and thus maintains its local independence, and its classification accuracy is high, which is widely used in the field of psychology[ 14 ]. In this study, we used a latent profile-based analysis to understand the latent categories of perceived motor behaviors in stroke patients and to explore the relationship between different perceived categories of motor behaviors and physical activity levels, intending to provide an effective basis for targeted interventions to improve the problem of physical activity levels in stroke patients. 2 The study 2.1 Participants This study was a cross-sectional survey, and stroke patients hospitalized in a tertiary-level hospital in Henan Province from February 2023 to May 2023 were selected by convenience sampling method. Inclusion criteria: age ≥ 18 years old, and conscious, with basic communication and expression skills; meet the diagnostic criteria of stroke in China[ 15 – 16 ] and confirmed by imaging examination; 6 months ≥ 2 months from the onset of the last stroke; informed consent. Exclusion criteria: the presence of cognitive dysfunction; serious mental illness; comorbidity with other serious diseases with contraindications to exercise unsuitable for exercise. This study was approved by the Ethics Committee of Xinxiang Medical College (XYLL-20220219) and ensure that: (i) identifying the licensing committee approving the experiments, including any relevant details; (ii) confirming that all experiments were performed in accordance with relevant guidelines and regulations. 2.2 Instruments It was designed by the researcher, including age, gender, education level, marital status, occupational status, type of stroke, coexisting chronic disease status, degree of BADL, and knowledge about physical activity in stroke patients, including: is there any understanding that physical activity includes occupational physical activity, transportation physical activity, household physical activity, and leisure time physical activity? Is it understood that moderate and high-intensity physical activity can reduce risk factors for stroke, such as hypertension, diabetes, dyslipidemia, and heart disease, and prevent the recurrence of stroke? Are you aware of the types of moderate and vigorous physical activity? Are aware of the duration, intensity, and frequency of physical activity recommended by the World Health Organization for patients with chronic diseases? Are aware of the techniques used by stroke patients when performing physical activity? Perceived Exercise Behavior Scale was compiled by Sechrist et al [ 17 ] and translated into Chinese by Zheng Jing, it was used to assess patients' perceived value of exercise and expectations of exercise outcomes. There were 43 entries with 2 dimensions, of which 29 perceived exercise benefits were factor analyzed to produce 5 exercise benefit factors, namely: life promotion, physical performance, psychological outlook, preventive health care, and social interaction; and 14 perceived exercise barriers were factor analyzed to produce 4 exercise barrier factors, namely: exercise environment, family resistance, physical exertion, and time expenditure. Exercise benefit perception entries were scored on a Likert 4-point scale: a score of 1 indicates complete disagreement and a score of 4 indicates complete agreement. Perceived exercise barrier entries were scored on a reverse scale. The total score on the scale ranges from 43 to 172, with higher scores indicating a stronger perception of exercise benefits and a weaker perception of exercise barriers by the patient. The Cronbach's alpha coefficients for the two subscales in this study were 0.825 and 0.718, respectively, and the Cronbach's alpha coefficient for the total scale was 0.772. International Physical Activity Questionnaire-short form-Chinese version (IPAQ-S) was developed by the International Physical Activity Measurement Group[ 18 ] and was Chineseized by our scholar Qu Ningning[ 19 ], was used to investigate the health-related exercise behaviors of stroke patients in the past week. The questionnaire consists of seven questions, including four parts: high-intensity physical activity, moderate-intensity physical activity, walking, and sitting still. According to the International Physical Activity Questionnaire Manual[ 20 ], the amount of each physical activity per week (MET-min/week) = MET value × physical activity time (min) × number of activities per week, and the metabolic equivalence criterion for METs corresponding to high-intensity physical activity was 8 for, the metabolic equivalence criterion for METs corresponding to moderate-intensity physical activity was 4 for, the metabolic equivalence criterion for METs corresponding to walking was 3.3 for, the metabolic equivalent criterion for METs corresponding to sitting was 1 for meditation. According to the classification criteria of the manual, the total physical activity energy consumption of individuals was categorized into three levels: physically active (≥ 3000 MET-min/week ), moderately physically active (≥ 600 MET-min/week), and physically inactive (< 600 MET-min/week). 2.3 Data collection First obtain the consent of the hospital and departments and other relevant departments, before the survey, using a unified guide to explain to patients the purpose, significance, filling out the method of this study, after obtaining the informed consent of the patients, the questionnaire was issued, and the patients were told to independently fill out the exercise before this hospitalization, and if there are reading and writing difficulties by the researcher according to the patient's expression to fill out on behalf of the patient, the patients fill out the process of the questionnaire to answer the questions promptly, and The questionnaires were collected on site. A total of 308 questionnaires were distributed in this study, and 299 valid questionnaires were returned, with a valid recovery rate of 97.08%. 2.4 Date analysis Epidata 3.1 software was used for single-person double-entry data, and Mplus 8.3 software was used for latent profile analysis (LPA) to explore the potential patterns of motor behavior perception in stroke patients. Specifically, the nine dimensions in the motor behavior perception questionnaire were used as observational variables, and the results of the latent profile analysis of one to four categories were extracted for summarization and comparison, and the results were compared according to the model fit indexes: akaike information criterion (AIC), bayesian information cirterion (BIC), sample-corrected BIC (aBIC), information entropy index (entropy value), likelihood ratio test (lo-mendell-rubin Likelihood, LMR), and Bootstrap-based likelihood ratio test (BLRT). ratio test (BLRT), to determine the fitness of the model and select the optimal potential class. Among them, AIC, BIC, and aBIC are statistical descriptions of the fitness of the potential profile model, and the lower the value indicates the better the model fitting effect; the entropy index is an evaluation index of the model categorization accuracy, which takes a value between 0 and 1, and the value > 0.8 indicates that the categorization accuracy rate is more than 90%, and the closer the value is to 1 indicates that the model categorization accuracy is higher; the LMR and BLRT are used for comparing the K categories and K-1 categories, if the P value is statistically significant, it indicates that the K-category model is significantly better than the K-1 category model. Statistical analyses were performed using SPSS 25.0 software, and measurements were expressed as mean ± standard deviation or median (interquartile spacing), and counts were described as frequencies and percentages. Comparisons between groups were made using the Wilcoxon rank-sum test, the Kruskal-Wallis test, and the rank-sum test for multiple-group hierarchical data. Ordered multicategorical logistic regression analyses were performed with categorical variables from general demographic information as independent variables, potential categories of perceived motor behavior as independent variables, and the 3 physical activity levels as dependent variables. The test level α = 0.05. 3 Results 3.1 General information and perceived motor behavior score and physical activity score of stroke patients A total of 299 stroke patients were investigated. Among the valid respondents, the ages of participants ranged between 19 and 86 ,with a mean value of 55.67 years (SD 10.75), 60.87% were males (N = 182), 39.13% were females (N = 117),86% were married (N = 257), 14% were unmarried, divorced or widowed (N = 42),64.21% were employed (N = 192), 35.79% were retired or unemployed (N = 107), 85.62% were ischemic (N = 256), 14.38% were hemorrhagic (N = 43), 90.30% were moderate or lower functional deficits (N = 270), 9.70% were moderate or higher functional deficits (N = 29). Other socio-demographic and clinical characteristics of the sample appear in Table 2 . The results of model fitting are shown in Table 1 . Combining the fit indices and practical significance of the models, the 3-category model was selected in this study as the best categorization result for perception of motor behavior in stroke patients. Meanwhile, the average probability of belonging to each potential category for the 3 categories of stroke patients was C1: 95.5%, C2: 94.9%, and C3: 94.9%, all of which were > 90%, which indicated that the results of the 3-category potential profile analysis were highly reliable. The scores of the 3 potential categories of perception of motor behavior of stroke patients were plotted according to their answers to the questions on 9 dimensions, as shown in Fig. 1 . The 3 categories were named according to their scoring characteristics, and C1 had lower scores on all the subdimensions of perception of motor benefits, and scores on all the subdimensions of perception of motor impairments were significantly higher than those of the other groups compared to C2 and C3, and the score on the 1st subdimension of perception of motor impairments (exercise environment) was highest Therefore, this group of stroke patients was named high perceived barriers-satisfaction with the exercise environment group ,which accounted for 10.6% of the total subjects. The trend of C2 scores was similar to that of C1 and C3, and the scores of this group were in the middle of C1 and C3, and the score of Time Expenditures was the lowest among the dimensions of Perceived Impairment. The group was named medium perceived benefits of exercise-worrying about consuming time group, which accounted for 48.6% of the total number of subjects. C3 scored high on all dimensions of Perceived Benefits and low on all dimensions of Perceived Benefits, but scored low on the 5th subdimension of Perceived Benefit Dimension (Social Interaction), which was named high perceived benefits-worry about social interaction group according to the characteristics of the scores, which accounted for 40.8% of the total number of subjects. 3.3 Univariate analysis of perceived latent categories of motor behavior in stroke patients The difference between the three potential categories of perceived motor behavior in stroke patients was statistically significant ( P < 0.05) for the five questions on knowledge related to physical activity, as shown in Table 2 . 3.4 Distribution of Physical Activity Levels in Patients with Different Perceived Categories of Motor Behavior The difference in the distribution of the three categories of perceived motor behavior in stroke patients across physical activity levels was statistically significant ( P < 0.05), as shown in Table 3 . Ordered multicategorical logistic regression analyses were performed with general information and perceived categories of motor behavior of stroke patients as independent variables. Parallel line test, χ 2 = 39.086, P = 0.252, so the ordered multicategorical logistic regression analysis model could be used. The results showed that the likelihood ratio χ 2 = 194.729, P < 0.001. The level of motor exercise in stroke patients was statistically significant in terms of perceived categories of motor behavior and age, as shown in Table 4 . In this study, three categories of exercise behavior perception in stroke patients were identified using latent profile analysis, and the scores of each category in different dimensions had significant variability, suggesting that the exercise behavior perception of stroke patients has obvious categorical characteristics. The exercise behavior perception of stroke patients can be classified into the " high perceived barriers-satisfaction with the exercise environment group ", " medium perceived benefits of exercise-worrying about consuming time group ", and " high perceived benefits-worry about social interaction group ". The medium perceived benefits of exercise-worrying about consuming time group comprised 48.6% of stroke patients, indicating that nearly half of the stroke patients had moderate exercise benefit perception, and those attributed to this category showed the presence of a lack of time in the perception of movement disorders. Similar to the results of the Security Xia et al.[ 13 ] similar to the results of the study, the reason for analyzing this may be that stroke is a chronic disease of the elderly, and the scientific popularization of exercise for the prevention of chronic diseases in our country is remarkable, and the concept of exercise for the prevention of disease has been widely recognized, but most of the people in this category are young and middle-aged married patients in the workplace, and this kind of group is the main bearer of the function of the family as well as the "backbone of the workplace", and their limited time is very limited. However, most of the patients in this category were married and working, and this group is the main bearer of family functions and the "backbone" of the workplace, and their limited time is more inclined to work and maintain family intimacy, which makes them think that exercise is a waste of time. Stroke patients in the high perceived benefits-worry about social interaction group accounted for 40.8% of patients, and the overall level of perceived benefits of exercise was high in this group, with the highest scores in the physical performance dimension and low scores in the social interaction dimension. This may be due to the fact that patients in this group had a higher level of familiarity with knowledge related to exercise and thus were more likely to intuitively perceive the positive aspects of exercise. Some studies have shown that[ 21 – 22 ] that the decline in activity ability of the elderly and the reduction of the opportunity to contact new things will produce a sense of social alienation and the sense of social alienation is positively correlated with physical activity, and the group of elderly stroke patients are many, and most of them are retired or unemployed, and there is a certain degree of limb dysfunction and a sense of shame in the patients after the stroke, which makes them feel that they are a burden to the family, and the negative impact of this disease will increase the sense of social alienation and make their social participation Decrease. Stroke patients in the high perceived barriers-satisfaction with the exercise environment group accounted for 10.6% of the total, and compared with the other 2 categories, those who belonged to this category had higher scores on all dimensions of perceived movement disorders, with the highest scores on the exercise environment subdimension. The reason for this may be that with the gradual improvement of China's public service system, a large number of exercise facilities, and the development of green space for patients to participate in sports and exercise to provide the basic conditions, a convenient exercise environment is the promotion of physical activity for stroke patients, suggesting that this category of stroke patients should look for the specific causes of their movement disorders, and health care workers can improve their perceptions of movement disorders through peer support, the demonstration of successful cases of combined The health care workers can improve their knowledge of exercise through peer support, presentation of successful cases, and psychological intervention to increase the perceived benefits of exercise in stroke patients.[ 23 ] The benefits of exercise can be improved through peer support and psychosocial interventions. 4.2 Physical activity levels of stroke patients differ across perceived categories of motor behavior, Perceived levels of motor behavior are positively correlated with physical activity levels The results of this study showed that the difference in physical activity levels among the 3 categories of perceived motor behavior in stroke patients was statistically significant ( P < 0.05). The high perceived benefits-worrying about social interactions group had a greater proportion of stroke patients who were moderately physically active and physically active, which is in line with the results of previous studies on other chronic diseases[ 24 – 25 ]. The theoretical model of health promotion suggests that [24] that perceived benefits or barriers are predictors of health behaviors and that physical activity levels are positively associated with perceived exercise behaviors. In this study, only 10.7% of stroke patients were physically active, with the highest number of patients in the high perceived benefits-worrying about social interactions group being at the most active level of physical activity at 87.5%, whereas the physical activity levels of stroke patients in the high perceived barriers-satisfaction with the exercise environment group were characterized by insufficient and moderate levels of physical activity. It has been suggested that[ 26 ] that patients with adequate levels of physical activity after stroke have a lower risk of adverse health outcomes, and that adequate physical activity can maximize the role of exercise in preventing stroke recurrence and improving prognosis. Therefore, it is suggested that healthcare professionals should focus on stroke patients in the high perceived barriers-satisfaction with the exercise environment group and provide them with personalized exercise interventions from the perspective of exercise psychology to overcome barriers to exercise, promote benefit discovery, and improve physical activity levels. 4.3 Age and Perceived Motor Behavior as Influences on Physical Activity Levels in Stroke Patients The results of this study showed that age was a hindering factor for physical activity level (OR = 0.390), which was higher in elderly stroke patients than in young and middle-aged stroke patients, which is consistent with the results of the Li Yan et al.[ 27 ] The results of the study were consistent with the fact that the older the age of the patients with chronic diseases in our community, the higher the level of physical activity, but in contrast to the shikawa-Takata K et al.[ 28 ] study results. Analyzing the reason, it may be due to the fact that most people over 60 years old in China are retired or jobless, have more time to participate in household chores or leisure time physical activities, and the elderly in China pay more attention to preventive health care, most of the elderly groups to participate in group physical activities, such as tai chi, square dance, etc., to strengthen the body and enrich the life. It is suggested that when healthcare professionals intervene in physical activity for stroke patients, they should strengthen the health guidance for young and middle-aged stroke patients, improve their health awareness, and promote the transformation of physical activity to a high level. Stroke patients with higher levels of perceived motor behavior had higher levels of physical activity (OR = 0.188–0.275), and this result further confirms the applicability of the health promotion theoretical model in physical activity health behavior change[ 29 ] that stroke patients make trade-offs between perceived barriers to exercise and benefits of exercise, and that final behavioral perceptions influence patients' physical activity levels. The results of one study have shown[ 30 – 32 ] that about 90% of stroke patients in the Chinese community felt a sense of shame and 72.5% experienced prejudice from the outside world, and that there was a significant correlation between shame and social participation, and that intervening in the sense of shame of stroke patients could improve their positive attitudes and increase their exercise adherence. This is consistent with the cognitive behavioral theory model[ 33 ] As changes in an individual's internal cognition can affect changes in behavior, healthcare professionals can use cognitive-behavioral therapy to intervene in the physical activity of stroke patients by correcting the misperceptions of stroke patients, improving the perception of the benefits of exercise, and clarifying the goals of action to promote the level of physical activity. 4.4 Limitations The present study was limited to 1 tertiary hospital in Henan Province, which is limited in scope, and a large-sample multicenter study can be conducted in the future; the present study was a cross-sectional study, which failed to assess the relationship between the perceived characteristics of motor behaviors and the physical activity level of stroke patients in different rehabilitation cycles, and a longitudinal study can be carried out in the future to gain a deeper understanding of the characteristics of the perceived characteristics of motor behaviors in stroke patients in different periods as well as the potential transformation of the perceived characteristics of motor behaviors. on their physical activity level and compliance. 5 Conclusion In this study, the perception of exercise behavior of stroke patients was classified into three categories: "high perceived barriers-satisfaction with the exercise environment group", "medium perceived benefits of exercise-worrying about consuming time group", and "high perceived benefits-worrying about social interactions group" through potential profile analysis.. The perceived level of exercise behavior in stroke patients was positively correlated with the level of physical activity, and special attention needs to be paid to the patients in the high perceived barriers-satisfaction with the exercise environment group. Unlike previous studies, this study clarifies the characteristics of perceived exercise behavior of different stroke patients and its impact on physical activity level, so that researchers can focus on the changes in patients' psychological trajectories from the perspective of positive psychology according to the patients' perceived level of exercise behavior and their age and take more targeted interventions to promote the improvement of physical activity level in stroke patients. Latent profile analysis identified distinct categories of perceived exercise behavior and ordered logistic regression analyzed the impact of these categories and demographic factors on physical activity levels.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e\u003cp\u003ePerceived exercise behavior was categorized into three groups: high perceived barriers but satisfaction with the exercise environment (10.6%), moderate perceived benefits but concerns about time consumption (48.6%), and high perceived benefits but worries about social interaction (40.8%). A positive correlation was found between perceived exercise behavior and physical activity levels. Age and perceived exercise behavior category significantly influenced physical activity (both P\u0026thinsp;\u0026lt;\u0026thinsp;0.05).\u003c/p\u003e\u003ch2\u003eConclusion\u003c/h2\u003e\u003cp\u003eStroke patients' perceived exercise behavior falls into three latent categories and serves as a modifiable factor affecting physical activity.
