Methods : A cross-sectional survey was conducted among 549 newly admitted clinical medicine graduate students at a medical university in Anhui Province using the Symptom Checklist-90 (SCL-90) and the Frost Multidimensional Perfectionism Scale. Data were analyzed using descriptive statistics, correlation analysis, and multiple linear regression. Results : The average SCL-90 score was 1.49±0.41, significantly higher than the national norm ( P <0.05). Dimensions such as anxiety, depression, phobia, and psychosis scored higher than the norm, while interpersonal relationships and hostility scored lower ( P <0.05). Perfectionism dimensions, including concern over mistakes, parental expectations, personal standards, and doubts about actions, were positively correlated with SCL-90 scores ( P <0.05). Multiple regression analysis indicated that being female, living in rural areas, and having perfectionist traits such as concern over mistakes and doubts about actions were significant predictors of higher SCL-90 scores ( P <0.05). Conclusion : Medical graduate freshmen have certain psychological health problems, and students with gender as females, family residence in rural areas, and perfectionist personality traits such as concern over mistakes and doubts about actions should be the focus of psychological health intervention. Perfectionism Mental health Medicine Graduate student Introduction Medical graduate education holds a significant position within higher education, with clinical medical graduate students being cultivated as advanced, research-oriented, and applied talents capable of engaging in clinical scientific research and practice. These students confront intricate medical knowledge, clinical practice, and research demands, all while shouldering substantial academic pressures and challenges. Amidst this fiercely competitive academic milieu, the mental well-being of clinical medical graduate students has garnered widespread concern. This study delves into the mental health status of clinical medical graduate freshmen, with a specific emphasis on how perfectionism impacts their psychological state. Given that clinical medical graduate students frequently strive for flawless academic achievements and clinical skills, they are more susceptible to the influence of perfectionism, which can trigger a cascade of mental health issues. Focusing on the Clinical Medical College of a medical university in Anhui, this research examines and analyzes the mental health conditions of 549 newly enrolled master's and doctoral students in 2021. By exploring the mental health status and its influencing factors among medical graduate students, the study aims to offer a scientific foundation for enhancing their mental well-being and fostering comprehensive development. Materials and Methods Subjects Cluster sampling was used to select 588 clinical medicine postgraduate students enrolled in a medical university in Anhui Province in 2021 as the study subjects. A questionnaire survey was conducted from September to October 2021, yielding 549 valid responses with a 93.4% effective rate. The sample comprised 235 males (42.8%) and 314 females (57.2%), including 122 doctoral students (22.2%) and 427 master's students (77.8%), with a mean age of 24.32 ± 3.07 years. 1.2 Methods 1.2.1 Mental Health Status The mental health status was assessed using Derrogis' Self-Rating Symptom Scale (SCL-90) [5] , which includes 90 items across 10 dimensions: somatization, compulsion, interpersonal sensitivity, depression, anxiety, hostility, phobia, paranoia, psychosis, and others (reflecting eating and sleep status). The scale uses a 5-point scoring system: 1 for "none," 2 for "mild," 3 for "moderate," 4 for "severe," and 5 for "very severe." Higher scores indicate more severe psychological issues. In this study, the scale showed a high Cronbach's alpha coefficient of 0.954, demonstrating strong internal consistency and reliability. 1.2.2 Perfectionism Personality The Frost Multidimensional Perfectionism Scale [6] , revised by Zi Fei et al, was employed in this study. This 27 items scale assesses five dimensions: concern about mistakes, personal standards, orderliness, parental expectations, and doubts about actions. The scale uses a 5-point scoring method, from 1 to 5 representing completely disagree to completely agree. The higher the score, the higher the corresponding perfectionism tendency. In this study, the Cronbach's alpha coefficient of this scale was 0.741. 1.2.3 General Information Survey A self-edited questionnaire was administered to gather data on gender, age, height, weight, training type, family residence, whether respondents are only children, the marriage status, etc. Quality Control The research was supervised by a teacher specializing in Epidemiology and Health Statistics. The investigation team consisted of graduate student counselors from various grades. The survey used a self-management approach, with on-site distribution of questionnaires. Respondents provided informed consent, and completed questionnaires were checked on-site for any obvious omissions. Statistical Analysis Database establishment and data entry were performed using EpiData 3.1. Data analysis was conducted with the SPSS 21.0 software package. Descriptive analysis was performed on the perfectionism dimensions scores. The T-test was used to compare the SCL-90 scores with the norm, and Pearson correlation analysis was performed between the perfectionism personality dimensions and the SCL-90 dimensions. Multivariate regression analysis was used for factor analysis, with a significance level of α=0.05. Results 2.1 Comparison of Psychological Status among Clinical Medical Graduate Students and Norms The average score of the Symptom Checklist-90 (SCL-90) for 549 clinical medicine graduate students was 1.49 ± 0.41, which is significantly higher than the normative average score of 1.44 ± 0.43. Dimensions such as depression, anxiety, phobia, and psychosis scored significantly higher than the norm, while the dimensions of interpersonal relationships and hostility scored significantly lower than the norm, also with statistical significance. Collinearity diagnosis was also performed, and the VIF value was 1.885, indicating that there was no multicollinearity in the multiple linear regression. The analysis results showed that the regression coefficients of age, training type, being the only child, body mass index, marital status, and the dimensions of Orderliness, Parental Expectations, and Personal Standards in perfectionism were not statistically significant. However, being female, living in rural and township areas, the dimensions of Concern over Mistakes and Doubts about Actions in perfectionism had significant positive predictive effects on the total score of SCL-90 (all P <0.05). The specific results are shown in Tables 4 and 5. The mental health issues among graduate students are concerning, which is consistent with the research results of Luo Jinyan [7] , but inconsistent with the research results of Zhang Min [8] and Yao Bin [9] . Psychologist Lazarus [10] believes that when the stimulation from the individual's internal and external environment exceeds their coping ability and resources, stress will occur, which can easily lead to reactions such as depression and anxiety. Graduate freshmen face a new learning, living, and working environment with higher academic goals and career planning, which can easily lead to cumulative stress, resulting in a imbalance between body and mind, and a series of emotional, behavioral, cognitive, and physiological manifestations. Currently, especially for professional degree graduate students, they enter a 33-month clinical rotation upon enrollment, which can easily lead to anxiety and fear, especially in recent years with the continuous emergence of residents' suicide incidents, it is essential for society and schools to pay more attention and to strengthen mental health education among graduate students. The urgency of enhancing mental health education for graduate students cannot be overstated. The perfectionism personality trait is an important factor affecting the mental health of medical graduate students [4] . This study conducted a correlation analysis between each dimension of perfectionism and various dimensions of mental health. The results showed that, with the exception of the organization dimension, worries about mistakes, parental expectations, personal standards, and doubts about actions were positively correlated with the SCL-90 mental health scores. Students with perfectionist personality traits are more likely to have mental health problems, which might be related to their tendency to experience negative emotions and underestimate their abilities and achievements. Graduate students with high perfectionism tendencies are more likely to experience psychological symptoms than those with low perfectionism tendencies. The study discovered that factors affecting the mental health of medical graduate students include gender, family residence in rural town areas, and the perfectionistic personality traits of concern over mistakes and doubts about actions. Most graduate students are between the ages of 24-26 when they begin their studies. Due to the conflict between women's self-pursuit and society's positioning of women, they need to find a balance between their education, family, and social relationships, facing more mental stress [11] ; this may also be related to women's genetic and biological susceptibility [12] . The general place of family residence can reflect the students' origin and family economic conditions, and they may face specific expectations and pressures from their families and society, such as early marriage and family responsibilities. These expectations and pressures may conflict with their personal development and career aspirations, increasing the risk of mental health issues. Additionally, perfectionists are prone to excessive concern about their mistakes and failures, and whether their actions meet the required standards. This worry and doubt can lead to persistent anxiety and stress. They may set excessively high standards for themselves, being overly sensitive to any shortcomings or errors, which can result in continuous self-pressure and negatively impact their mental health. Conclusion The mental health issues among the medical graduate freshmen in the School of Medicine at Anhui Medical University are of concern. Research findings suggest a need to pay attention to the mental health of new graduate students in clinical medicine, and to formulate corresponding intervention measures targeting factors such as gender, students from rural and township areas, and those with a perfectionist personality, in order to improve the mental health level of clinical medicine graduate students and promote their comprehensive development. However, there are limitations to this study, such as the limited sample to one medical college which may not be representative, and the cross-sectional design which cannot determine causality. Future research should focus on conducting cohort studies and experimental research to further explore perfectionism and other potential mental health influencing factors, to track and observe the changes in mental health of graduate students during their studies, and to evaluate the effectiveness of intervention measures. By exploring the mental health status and its influencing factors among medical graduate students, the study aims to offer a scientific foundation for enhancing their mental well-being and fostering comprehensive development.

Materials and Methods

Subjects

Cluster sampling was used to select 588 clinical medicine postgraduate students enrolled in a medical university in Anhui Province in 2021 as the study subjects. mass index, and marital status, as well as the dimensions of perfectionism, were included as independent variables, and the total score of SCL-90 was treated as the dependent variable in a multiple linear regression analysis.

However, being female, living in rural and township areas, the dimensions of Concern over Mistakes and Doubts about Actions in perfectionism had significant positive predictive effects on the total score of SCL-90 (all P<0.05). research results indicate that the overall score of the Symptom Checklist-90 (SCL-90) for medical graduate freshmen is significantly higher than that of the general population, with dimensions such as anxiety, depression, phobia, and psychosis scoring higher than the national adult norm. The mental health issues among graduate students are concerning, which is consistent with the research results of Luo Jinyan[7], but inconsistent with the research results of Zhang Min[8] and Yao Bin[9]. Psychologist Lazarus[10] believes that when the stimulation from the individual's internal and external environment exceeds their coping ability and resources, stress will occur, which can easily lead to reactions such as depression and anxiety.

The perfectionism personality trait is an important factor affecting the mental health of medical graduate students[4].

Due to the conflict between women's self-pursuit and society's positioning of women, they need to find a balance between their education, family, and social relationships, facing more mental stress [11]; this may also be related to women's genetic and biological susceptibility[12].

Conclusion

The mental health issues among the medical graduate freshmen in the School of Medicine at Anhui Medical University are of concern.

Declarations

This study was carried out in strict accordance with the principles of the Declaration of Helsinki. Data were analyzed using descriptive statistics, correlation analysis, and multiple linear regression.

Results: The average SCL-90 score was 1.49±0.41, significantly higher than the national norm (P<0.05). Dimensions such as anxiety, depression, phobia, and psychosis scored higher than the norm, while interpersonal relationships and hostility scored lower (P<0.05). Perfectionism dimensions, including concern over mistakes, parental expectations, personal standards, and doubts about actions, were positively correlated with SCL-90 scores (P<0.05). Multiple regression analysis indicated that being female, living in rural areas, and having perfectionist traits such as concern over mistakes and doubts about actions were significant predictors of higher SCL-90 scores (P<0.05).

Conclusion: Medical graduate freshmen have certain psychological health problems, and students with gender as females, family residence in rural areas, and perfectionist personality traits such as concern over mistakes and doubts about actions should be the focus of psychological health intervention.
