Having specific risk values for robust health variables that can be measured through easy-to-use, inexpensive, feasible and reliable tools would make it possible to overcome the usual lack of resources in mental health services. Aims : To provide reference values for levels of physical fitness (a more powerful predictor of disease, disability and mortality than smoking, hypertension, hypercholesterolaemia and diabetes) evaluated through low-resource tools in people with severe mental disorders. Method : In total, 305 people (83 women) with severe mental disorders aged between 20 and 82 years were evaluated on a set of components of physical fitness (aerobic fitness, strength, flexibility, agility and balance) using the Senior Fitness Test battery. Results : Reference values were provided by quartiles according to sex and age (20–39 years, 40–59 years, > 60 years). Women had significant lower performance than men in the aerobic fitness test (p <0.001; Cohen’s standardized effect size [d]: -0.83, 95 % confidence interval [CI]: -0.54, -1.12) and men had significant lower performance in the lower limb flexibility test (p <0.001; d: -0.55, CI: -0.83, -0.26). Performance in all tests was lower as age increased (standardized regression coefficients [β] ranged from -0.10 to -0.40). Conclusions : This study provides the first reference values for physical fitness components specific to adults with severe mental disorders, helping identify individuals at potential health risk and prioritize therapeutic interventions. The main contribution was to provide the first reference values of a set of physical fitness components specific for adults with severe mental disorders. Sports Medicine and Kinesiology Psychiatry Physical Medicine & Rehab Early diagnosis health risk physical fitness reference values severe mental disorders Introduction Severe mental disorders (SMDs), such as schizophrenia, depression, anxiety and bipolar disorder, are a leading cause of illness [ 1 ], disability and premature death from all causes [ 2 ] worldwide. In 2010, the global economic burden of SMDs was comparable to that of cardiovascular diseases and higher than that of cancer, chronic respiratory diseases, and diabetes, and is expected to double by 2030 [ 3 ]. Therefore, reducing the growing burden associated with SMDs is a global priority [ 4 ]. One factor that could reduce the burden associated with SMDs is to improve the detection of people with SMDs at health risk in order to prioritize the application of therapeutic interventions. The evidence accumulated for more than 30 years [ 5 ] maintains that low physical fitness – an aspect consistently more common in people with SMDs compared to the population without SMDs [ 6 , 7 ] – is a more powerful predictor of disease, disability and mortality than traditional risk factors such as smoking, hypertension, hypercholesterolaemia and type 2 diabetes mellitus. In fact, scientific support for the prognostic use of physical fitness is so robust that scientific-health institutions, such as the American Heart Association, advocate its routine evaluation as a vital clinical sign [ 8 ]. Unfortunately, physical fitness is not usually assessed in people with SMDs, probably due to the lack of resources (material, temporal and personal) in mental health services, which is even more acute in countries with low income. One aspect that could facilitate the assessment of physical fitness in mental health services is to have reference values of easy-to-use, inexpensive, feasible and reliable assessment tools for people with SMDs that allow the level of physical fitness to be interpreted for those with similar characteristics. To our knowledge, the only reference values available for physical fitness levels in people with SMDs come from a study that assessed aerobic fitness, using the 6-minute walk test, in 237 people (101 women) with schizophrenia aged 22–70 years [ 9 ]. Given that physical fitness includes components additional to aerobic fitness, such as muscular strength, flexibility, agility and balance, the objective of this work was to provide reference values for levels of physical fitness components (aerobic fitness, muscular strength, flexibility, agility and balance) evaluated through low-resource tests in people with SMDs. We hypothesize that men and young people will have higher levels than women and older people for most physical fitness components. Methods Participants and procedure This cross-sectional study was part of the PsychiActive project, which included an integrated approach to positive health behaviour and healthy lifestyle for people with SMDs from Spain. The protocol was approved by the XXX and XXX Ethics Committee (1674-N-17). All participants gave their informed written consent prior to enrolling in the study and after receiving information about the aims and protocol. There was no compensation for participation. From July 2014 to February 2022, a convenience sample of people with SMDs was recruited from 19 different outpatient mental healthcare settings in Southern Spain. Project pamphlets and posters were placed in waiting rooms and interested people contacted the researchers directly or through staff members. People with a range of ICD-10 SMD diagnoses (see Table 1 ) established by experienced psychiatrists were included if they were aged 20 years or older and were stable on antipsychotic medication during the last month. Those with clinical instability, substance abuse or any other condition contraindicating participation were excluded. Participants were required to undergo fitness and anthropometric measures. Data analysis was conducted in September 2023 and included participants who performed at least one of the fitness tests. Measures Physical fitness Physical fitness was assessed through the Senior Fitness Test battery, which allows different components of physical fitness to be assessed in a standardized manner using easy-to-use, inexpensive, feasible and reliable assessment tools [ 10 ]. In brief: Aerobic fitness was assessed with the 6-minute walk test, which consists of walking around a 46-m rectangular space, with the objective of covering the greatest distance possible in 6 minutes. Lower body muscular strength was assessed with the 30-second chair stand test, which consists of getting up and sitting down on a chair as many times as possible, with arms crossed on the chest. A valid repetition was considered when a full body extension was performed. Upper body muscular strength was assessed with the arm curl test, which consists of performing the greatest number of elbow flexion–extensions for 30 seconds with a dumbbell (3.6 kg for men and 2.3 kg for women) while sitting on a chair. A valid repetition was considered when a complete elbow flexion was performed. Lower body flexibility was assessed with the chair sit-and-reach test, which consists of moving the hands together towards the tips of the toes, trying to reach and/or surpass them if possible. The elbows must be extended and the knee of the foot on which the test is performed must be flexed. Upper body flexibility was assessed with the back scratch test, which consists of trying to touch and/or extend the tips of the fingers on the back. One hand is placed behind the shoulder on the same side with the palm down, and the other behind the back with the palm up. Agility/dynamic balance was assessed with the 8-foot up-and-go test, which consists of getting up from a chair, walking 2.44 m, turning 180º and walking back to sit on the same chair in the shortest time possible. The tests were performed in the following order, with at least 1 minute of rest between attempts and/or 3 minutes between tests to reduce the influence of fatigue: back scratch test; chair sit-and-reach test; arm curl test; 8-foot up-and-go test; 30-second chair stand test; and 6-minute walk test. All participants performed several trials before recording the results to ensure correct execution. The best of two attempts for each test was used in the analysis, except for the 30-second chair stand test and the 6-minute walk test, which were performed only once. Explanation, supervision and support during execution of all the tests were carried out by the same experienced physical exercise professional (J.B.A.). Sociodemographic and clinical data Weight and height (used to derive body mass index) were measured following standard procedures with a scale (TANITA BC-420; Tanita, Tokyo, Japan) and a wall-mounted stadiometer, respectively. Age, sex, and diagnoses were obtained from the participants’ medical records. Statistical analysis Between-sex comparisons of physical fitness were performed using Welch’s unpaired t -test, with adjustments for multiple comparisons using the Bonferroni method by dividing the significance level of 0.05 by the number of comparisons; in this case, 0.05 x 6 − 1 = 0.0083. The associations between physical fitness and age were explored using regression analysis adjusted for sex. The physical fitness reference values were displayed by sex and age group (20–39, 40–59 and > 60 years; according to the previous study in people with SMDs [ 9 ]) and categorized into 25th, 50th and 75th percentiles as low, average and high performance groups, respectively. Data were analysed using SPSS Statistics for Windows, Version 27.0 (IBM Corp.; Armonk, NY, USA), with statistical significance set at α = 0.050. Between-sex comparisons were assessed using an approach based on the standardized effect size, with Cohen’s d adjusted by correcting for small sample bias, also known as Hedges’ g , and the associated 95% confidence intervals determined using a free online effect size calculator [ 11 ]. Cohen’s d was calculated as follows: the difference between the mean values for both groups divided by the pooled standard deviation for both groups. The formulae for estimation of Cohen’s d and the associated 95% confidence intervals are available on GitHub [ 12 ]. Cohen’s d ≥ 0.50 was considered to be a minimally important difference [ 13 ]. Results The sample included 305 people with SMDs aged 20–82 years. The sociodemographic characteristics and average physical fitness values for all participants and by sex are presented in Table 1 and Table 2 , respectively. The association between physical fitness and age is shown in Table 3 . In general, performance on all fitness tests was lower as age increased. Discussion The main contribution of the study was to provide reference values of a set of physical fitness components for adults with SMDs attending outpatient mental healthcare settings. Men had higher performance than women in the cardiorespiratory fitness test and women had higher performance in the lower limb flexibility test. Performance on all tests was lower as age increased. The results for the influence of sex and age on physical fitness agree with those reported in previous research on people with SMDs [ 6 ] other pathologies and on healthy people [ 5 , 14 ], suggesting the detection of health risk profiles due to low physical fitness in people with SMDs by taking into account sex and age, which could be improved by comparing the results for accessible diagnostic tests against specific reference data stratified by sex and age, as we propose in this work. Our values in the 6-minute walk test, considered as low performance (i.e. lower than the 25th percentile values) for men and women in the age ranges 20–39 and 40–59 years, are higher than those provided by the only previous study on physical fitness reference values for people with SMDs [ 9 ]. This fact may be due to differences in the procedure followed in the 6-minute walk test. Huang's study [ 9 ] included a straight course (out and back) with 180º turns that, compared to the continuous course (rectangular) with 90º turns in our study, could involve greater decelerations in each turn, making it difficult to maintain a high walking speed during the test and therefore achieving a lower result, as was found in a study of people with severe emphysema [ 15 ]. Additionally, participants in Huang's study [ 9 ] were instructed to “walk as fast as possible”, which, compared to the “walk as far as possible without jogging or running” in our study, could lead to “earlier fatigue and possible excessive cardiac stress”, as pointed out by the American Thoracic Society Statement in the guidelines for the 6-minute walk test [ 16 ]. The latter represents a call to attention for researchers and professionals in the mental health area to carefully and consistently apply the procedures described in the literature in order to reduce any influence of the evaluator on the performance of participants and facilitate comparison of results between studies employing similar methods. The difference in performance for the 6-minute walk test compared to Huang's study [ 9 ] may also be due to the geographical situation in which the research was carried out (Spain vs. Taiwan), as suggested by a study in healthy adults from seven different countries who performed the 6-minute walk test following American Thoracic Society guidelines [ 17 ], as well as the lower age and higher average height (in men) of our participants, as suggested by Huang's own study [ 9 ]. On the other hand, the fact that the participants in our study were recruited from outpatient settings and have a higher average body mass index value than the participants in Huang's study [ 9 ] who were recruited mostly (72%) from inpatient settings, seems to contradict the findings of a meta-analysis on moderators of the 6-minute walk test in adults with SMDs [ 18 ]. However, the authors of the meta-analysis [ 18 ] propose further research to clarify whether the clinical setting is a considerable moderator of the 6-minute walk test, and point out that including cross-sectional studies prevents a causal relationship to be established between physical fitness and body mass index; this is questioned in numerous studies [ 19 ], including a meta-analysis of people with SMDs, whose discussion is worth reading [ 6 ]. Given that we are not aware of additional studies on reference values for the physical fitness of people with SMDs, the rest of the discussion will focus on comparing our results with those of similar studies developed in other populations. In all tests of the Senior Fitness Test battery (except for the 6-minute walk test), our values that were considered as low (i.e. lower than the 25th percentile values) in women aged 20–59 years are higher than the values considered as low (i.e. lower than the 20th percentile values) in Spanish women with fibromyalgia aged 35–45 years [ 20 ]. For the 6-minute walk test, our values that were considered as low in women aged 20–39 years and 40–59 years are higher than the values considered as low in Spanish women with fibromyalgia aged 35–45 years and 45–55 years, respectively [ 20 ]. The particular characteristics associated with SMDs and fibromyalgia could explain all these differences and reinforce the need to have reference data from specific populations to interpret the results using more sensitive tools. On the other hand, the values considered as low (i.e. lower than the 25th percentile values) in the 6-minute walk test of men and women aged 40–59 years in our study are lower than those that were considered low (i.e. lower than the 20th percentile values) in healthy older Spanish aged 65–69 years (for men) and 70–74 years (for woman) [ 21 ], suggesting premature physical aging of people with SMDs according to Zechner [ 22 ] which could partially explain why people with SMDs have a life expectancy of up to 32 years lower compared to a healthy population [ 2 ]. Additionally, our values considered as low (i.e. lower than the 25th percentile values) in the 6-minute walk test of men and women aged 20–59 years are lower than the criterion-referenced fitness standards for maintaining physical independence in healthy older Americans aged 70–84 years [ 23 ], which could partially explain why diagnoses linked to SMDs, such as depression and anxiety, were two of the 25 main causes of disability worldwide in 2019 [ 24 ]. Considering the latest comments, there appears to be a need to promote adequate levels of aerobic fitness in people with SMDs. A meta-review by the European Psychiatric Association [ 25 ] includes clinical practice recommendations to improve aerobic fitness in people with SMDs by using physical activity. Limitations This work has several strengths. Physical fitness was assessed through valid tests that require few resources to apply, developed by the same evaluator following standardized protocols and who created free-access video tutorials to facilitate reading comprehension and their application in mental health services. The work provides the first reference values in people with SMDs on components of physical fitness additional to those available in aerobic fitness [ 9 ], such as strength, flexibility, agility and balance, which allow a more complete assessment of physical fitness for developing more individualized therapeutic interventions for this population. Participants have different types of SMDs, as commonly occurs in mental health services, thus increasing the applicability of the results in real-world settings. The work also has limitations: the non-representative convenience sample, composed mainly of men and those under 60 years of age, limits generalization of the results; and the cross-sectional design precludes causal relationships. Conclusion The main contribution of the study was to provide reference values for a set of fitness components in adults with severe mental disorders that could facilitate the detection of people with potential health risk and therefore with priority needs to start therapeutic interventions. Implication for practice This work allows the scientific community and mental health professionals to improve the detection of people at health risk to prioritize the application of therapeutic interventions through easy-to-use, inexpensive, feasible and reliable tools would make it possible to overcome the usual lack of resources in mental health services.
