The Hospital Survey on Patient Safety Culture 2.0 (HSOPSC 2.0), developed by the Agency for Healthcare Research and Quality, has been widely used internationally; however, its psychometric properties have not been fully validated in mainland China. Methods A cross-sectional survey was conducted among medical staff in a tertiary public hospital in Zhejiang Province, China. A total of 560 valid questionnaires were analyzed. Internal consistency reliability was assessed using Cronbach’s α and split-half reliability. Construct validity was evaluated through item–dimension correlations, exploratory factor analysis (EFA), and confirmatory factor analysis (CFA). Results The overall Cronbach’s α coefficient of the questionnaire was 0.942, with dimension-specific α values ranging from 0.657 to 0.914. The overall split-half reliability coefficient was 0.862. The Kaiser–Meyer–Olkin value was 0.938, and Bartlett’s test of sphericity was significant (p < 0.001), indicating suitability for factor analysis. EFA extracted ten factors explaining 74.424% of the total variance. CFA results demonstrated acceptable to good model fit after item adjustment (χ²/df = 2.890, GFI = 0.923, RMSEA = 0.058), showing improved structural validity compared with the original model. Conclusions The Chinese version of the HSOPSC 2.0 demonstrates good reliability and validity and is suitable for assessing patient safety culture in Chinese hospitals. This validated instrument may support healthcare institutions in identifying safety culture gaps and guiding targeted patient safety improvement strategies. Patient Safety Culture HSOPSC Reliability Validity Figures Figure 1 Figure 2 Background Patient safety is a persistent theme in the development of hospitals and an important foundation for efforts to deepen the reform of the medical and health system. Patient safety culture refers to a combination of common cognitions, concepts, attitudes, values, and behaviors exhibited by medical personnel in the process of ensuring patient safety [ 1 – 2 ]. Regular assessments of patient safety culture are beneficial given their ability to enable medical institutions to identify and correct deficiencies pertaining to patient safety management promptly. In 2004, the Agency for Healthcare Research and Quality (AHRQ) publicly released the Hospital Survey on Patient Safety Culture (HSOPSC) 1.0, which has been widely used by research institutions and scholars worldwide [ 3 – 5 ]. In 2019, AHRQ published a new version of the Hospital Survey on Patient Safety Culture. This article translates the 2019 version, which is known as HSOPSC 2.0, and investigates medical staff working for a comprehensive public tertiary hospital in Zhejiang Province with the goal of collecting data that can be used to test the reliability and validity of the questionnaire. The aim of this research is to provide a reference for domestic medical institutions seeking to apply this questionnaire to the tasks of identifying weak links in hospital safety culture construction and promoting patient safety management. Research Subjects and Methods Research Subjects In a comprehensive public tertiary hospital in Zhejiang Province, China, a stratified random sampling method was used to recruit 1000 employees, including 316 physicians, 453 nursing personnel, 121 medical technicians, and 110 administrative and logistics staff, to participate in this research. A total of 1000 questionnaires were distributed, and 564 questionnaires were returned. Following review and the exclusion of blank questionnaires as well as those that featured incomplete responses or obvious logical errors, a total of 560 valid questionnaires were obtained, for a valid response rate of 56%. Research Methods Research Tools HSOPSC 2.0. This questionnaire includes questions pertaining to the following topics: (1) employees’ positions/titles and background questions; (2) a total of 10 measurement dimensions, including the communication of errors, openness to communication, handoffs and information transfer, hospital management support for patient safety, organizational learning and continuous improvement, patient safety event reporting, reactions to errors, staffing and workload, department leadership support for patient safety, and teamwork (these dimensions are associated with a total of 32 items); (3) the number of adverse event reports and an evaluation of the level of patient safety that characterizes the corresponding department. Participants respond to the items included in the questionnaire on a five-point Likert scale that includes the following options: "Strongly Disagree/Never," "Disagree/Rarely," "Neutral/Sometimes," "Agree/Often," and "Strongly Agree/Always." These options are associated with scores ranging from 1 to 5 points. Questionnaire Localization and Cultural Adaptation The English version of this questionnaire was translated and modified in line with the usage guide provided by the AHRQ. The specific process employed in this context was as follows. (1) The author first translated the English version of the questionnaire with the goal of ensuring that the translation was semantically and contextually consistent with the original version. (2) A researcher with experience working with patient safety culture surveys reviewed the original version and the translation and made relevant corrections to any incorrect or unclear expressions. (3) These first two steps were repeated until the two researchers reached a consensus, thus leading to the development of a preliminary draft, following which two senior experts in the field of hospital quality management were asked to evaluate whether the item design and language expression were appropriate and consistent with the national conditions. (4) After the preliminary draft was modified on the basis of the experts' opinions, a pilot survey was conducted by reference to 30 employees drawn from the fields of clinical medicine, nursing, hospital management, and human resource management; in this context, the participants' feelings during the survey were recorded, as were any doubts they experienced, following which corrections were made in a timely manner until the questionnaire was well accepted and understood by the participants. On this basis, the final draft of the translation was established. Online Survey Method First, the questionnaire was entered into the Questionnaire Star platform, following which a link to the patient safety culture survey was sent to the recruited employees via the hospital's short message service (SMS) platform, thus allowing employees to complete and submit the survey voluntarily. The survey was conducted in 2023. Method of Statistical Analysis The data exported from Questionnaire Star, which were in Excel format, were encoded and processed, and then subsequently subjected to statistical analysis using SPSS 20.0 and AMOS 22.0 software. Split-half reliability and Cronbach’s α coefficient were used to evaluate the reliability of the questionnaire, while Pearson correlation analysis, t-tests, and both exploratory and confirmatory factor analysis were employed to assess its validity. A special note is needed: in the current study, the 564 valid respondents corresponded to a participant-to-item ratio of approximately 17.6:1 (564 participants/32 items), which far exceeded the recommended minimum range of 5–10:1. This indicated that the sample size was statistically sufficient to support the robustness of factor analysis and other subsequent analytical procedures. Thus, despite the modest overall response rate, the statistical power of the present study was sufficiently robust to ensure the validity of the research findings. Results and Analysis Basic Information Concerning the Surveyed Subjects As Table 1 indicates, among the 560 surveyed employees, nurses accounted for the majority (at 47.5%), followed by doctors (at 31.4%). The sample included fewer medical technicians and administrative and logistics personnel, who accounted for 11.4% and 9.7% of the total, respectively. In terms of participants’ titles, those with intermediate-level titles or below accounted for 84.7% of the total, whereas those with senior titles accounted for 12.3%. Participants’ tenure in their specific hospitals and departments was concentrated in the range of 1–10 years; namely, individuals with this level of tenure accounted for approximately 70% of the total. A total of 84.1% of the employees included in this research frequently engaged in direct contact with patients, whereas 15.9% did not frequently engage in direct contact with patients. The results revealed that the overall Cronbach’s α coefficient of the questionnaire was 0.942, and the Cronbach’s α coefficients pertaining to the 10 dimensions included in the questionnaire ranged from 0.657 to 0.914. The overall split-half reliability coefficient of the questionnaire was 0.862. Since 6 dimensions contained an odd number of items, these dimensions were further divided into "equal-length split-half reliability coefficients" and "unequal-length split-half reliability coefficients" [ 6 ]. Overall 0.942 0.862 0.862 Validity Test of the Hospital Patient Safety Culture Survey 2.0 Content Validity A single-sample Kolmogorov–Smirnov (K-S) test was conducted to explore the total scores obtained by summing the scores associated with the 32 items included in the questionnaire. The results of the K-S test revealed a value of 1.320, with P = 0.061 > 0.05, thus indicating that the total scores on the questionnaire were normally distributed. Pearson correlation coefficients were used to evaluate the content validity of the questionnaire. The correlation coefficients between each item and the corresponding dimension ranged from 0.667 to 0.950, with P < 0.001 (Table 3 ). The correlation coefficients between the scores of each dimension and the total score ranged from 0.475 to 0.833, with P < 0.001 (Table 4 ), thus indicating strong correlations. Table 5 presents the results of a principal component analysis, which revealed a total of 10 common factors following variance-maximizing orthogonal rotation, with a cumulative variance contribution rate of 74.424%; items that exhibited factor loadings greater than 0.45 were entered into the factor matrix. Overall, the items contained in each common factor identified by the exploratory factor analysis were generally consistent with the original questionnaire, albeit with some discrepancies: Items A3, B3, C2, C4, and E2 are now collectively assigned to a new dimension, and the "Organizational Learning and Continuous Improvement" and "Department Leadership's Support for Patient Safety" dimensions from the original questionnaire are now merged into a single dimension. The data were imported with the goals of verifying and revising both models. The model fit results indicated that, in comparison with Model M0, Model M1 exhibited reduced values with respect to the chi-square to degrees of freedom ratio, root mean squared residual (RMR), and root mean square error of approximation (RMSEA) as well as improved relative fit indices and parsimonious fit indices; all fit indices thus exhibited further improvement. The results revealed that, with the exception of working hours, the scores on the patient safety culture survey obtained by participants exhibited significant differences (P < 0.05) across the groups divided according to job position, title, and frequent direct contact with patients. The core dimensions of patient safety culture proposed by AHRQ were retained, while minor modifications reflected contextual and cultural adaptations to the Chinese healthcare setting. Similar variations in factor structures have been reported in validation studies conducted in other countries, suggesting that such adaptations are a common and acceptable feature of cross-cultural psychometric validation. The Hospital Patient Safety Culture Survey 2.0 exhibits good reliability Reliability reflects the stability and dependability of a questionnaire, which is usually measured by reference to Cronbach’s α coefficient, split-half reliability, and test-retest reliability [ 7 ]. In light of the busy schedules of medical staff and the long questionnaire collection period, this study did not examine test-retest reliability. The test results regarding reliability reveal that the overall Cronbach's α coefficient of the questionnaire was 0.942, and the Cronbach’s α coefficients of the other dimensions ranged between 0.657 and 0.914, thus indicating a high level of internal consistency. The overall split-half reliability coefficient of the questionnaire was 0.862, and the split-half coefficients of the other dimensions ranged from 0.609 to 0.890, thus indicating that the questionnaire exhibited good reliability. The Hospital Patient Safety Culture Survey 2.0 exhibits good validity Validity refers to the extent to which a measurement tool can accurately measure what it is intended to measure. This notion is used mainly to evaluate the effectiveness of a questionnaire, including in terms of content validity, structural validity, and discriminant validity. Content validity refers to the appropriateness of the content that the questionnaire aims to measure and its consistency with the actual content that it measures. The hospital patient safety culture questionnaire used in this study was developed on the basis of a mature questionnaire developed in another national context; the original questionnaire exhibited a high level of content validity. The questionnaire was translated, modified, and culturally adjusted according to strict standards and procedures, and the results of a preinvestigation confirmed that the Chinese version of the questionnaire achieved high levels of acceptance and understanding among medical staff. The results of the content validity test revealed that, with the exception of the reporting of patient safety incidents, the scores pertaining to each dimension of the questionnaire and the total score were strongly correlated (0.706 < r < 0.833), thus indicating that the questionnaire exhibited good content validity [ 8 ]. Structural validity refers to the degree of consistency between the actual measured structure of the questionnaire and the theoretical structure; it is thus the most important indicator of validity. In this research, an exploratory factor analysis was conducted to extract 10 common factors that were associated with a cumulative variance contribution rate of more than 70%. Most dimensions and items were consistent with those pertaining to the original questionnaire, but some dimensions exhibited changes. A review of the translated or adapted versions of such scales in China and other countries revealed similar research results, such that some dimensions and items exhibited relatively low correlations with each other in comparison with the original English version of the tool [ 9 – 10 ]. The results of a confirmatory factor analysis revealed that the adjusted structural equation model exhibited a degree of freedom ratio of 2.890 0.90; furthermore, the RMR and RMSEA were less than 0.08, and the other fit indicators were also ideal. In particular, these indicators were better than those associated with the adjusted structural equation model, thus indicating that the questionnaire exhibited good structural validity [ 11 – 12 ]. Discriminant validity refers to the ability of a questionnaire to distinguish among different known characteristics of a given population. The test results revealed significant differences in patient safety culture scores among medical staff who exhibited different positions, titles, and numbers of weekly working hours as well as based on whether the participants often engaged in direct contact with patients (P 0.05). These findings differ from those reported by Zhou Xueyang [ 13 ] and Sun Huijun [ 14 ] but are consistent with those reported by Jia Ying Lei [ 15 ], Tran [ 16 ], Manzo BF [ 17 ], etc., thus suggesting that the questionnaire exhibited good discriminant validity. The Hospital Patient Safety Culture Survey 2.0 exhibits a better structure after the items are adjusted The results of an exploratory factor analysis revealed that Items "B1. The department regularly reviews work processes to determine whether changes are needed to improve patient safety," "B2. The department assesses the effectiveness of improvements to patient safety," "E1. My leaders take suggestions made by staff regarding ways of improving patient safety seriously," and "E3. My leaders take action to address patient safety concerns" can be categorized into one dimension, while Items "B3. The same patient safety events occur repeatedly in the department" and "E2. My leaders expect us to speed up work even by taking shortcuts when we are busy" as well as Items "A3. Disrespectful behavior occurs among staff in this department," "C2. After an error event is reported, it seems that only the person who made the mistake receives attention, whereas the event itself is not viewed as important," and "C4. The department lacks support for staff involved in patient safety events" form a new dimension. The results of the confirmatory factor analysis revealed that the adjusted model exhibited a better fit than did the adjusted model. Items B1, B2, E1, and E3 all indicate that the department/leaders will implement measures to improve patient safety. In terms of content, it is reasonable to categorize these four items as part of a single dimension. Item B3 focuses on the frequency of patient safety events, and Item E2 focuses on the department's work methods; thus, it is reasonable that these two items are not grouped together with Items B1, B2, E1, and E3. Items B1, B2, E1, and E3 were originally associated with the dimensions of "Organizational Learning and Continuous Improvement" and "Department Leadership Support for Patient Safety"; however, following the merging process, they are collectively named "Department Continuous Improvement of Patient Safety." As for items A3 (which was originally associated with the "Teamwork" dimension), C2, and C4 (which were originally associated with the "Response to Errors" dimension),the content reflected by the items indeed exhibited a certain correlation with the original dimensions; however, this content focused more on reflecting the department's interpersonal style as well as the corresponding attitudes toward employees and patient safety events, thus resembling the connotation of Items B3 and E2. The correlation coefficients between these five items and the original dimensions were 0.585, 0.813, 0.668, 0.758, and 0.779, and the corresponding correlation coefficients with the current dimensions were 0.768, 0.756, 0.780, 0.812, and 0.693. Overall, the correlation coefficients with the current dimensions are thus greater. Following group discussion and a review of the relevant literature [ 18 – 19 ], the new dimension was named "Department Safety Climate." Accordingly, the logical structure reflected by the research data presented in this paper is largely consistent with that of the original questionnaire, and the content and structure of the questionnaire are better than those of the original questionnaire after some items were adjusted. Limitation This study adopted a single-center, cross-sectional design with participants solely recruited from medical staff of a public tertiary general hospital in Zhejiang Province, and test-retest reliability was not evaluated, which to some extent restricts the sample’s representativeness and the generalizability of the findings. Despite these limitations, the sample size of this study is sufficient, with a participant-to-item ratio far exceeding the recommended minimum of 5–10:1 for scale validation, and the sample covers diverse occupational structures among hospital employees, which meets the basic criteria for scale validation. In summary, a survey of 560 hospital employees provided preliminary verification that the translated Chinese version of the Hospital Survey on Patient Safety Culture 2.0 (HSOPSC 2.0) exhibits high reliability and validity, with credible measurement results—indicating its applicability as a tool for assessing hospital patient safety culture. Notably, due to cultural disparities between China and Western countries, further refinements to the questionnaire’s dimensions and items warrant validation through more extensive empirical investigations [ 20 – 22 ]. For instance, incorporating an "inapplicable" option into a 6-point Likert scale would enhance the practicality and contextual adaptability of the Chinese HSOPSC 2.0, ensuring it better aligns with the actual work scenarios of Chinese healthcare professionals. Practically, this validated Chinese version offers hospital managers and policymakers a standardized instrument to assess patient safety culture, pinpoint organizational gaps in patient safety, and monitor the efficacy of safety improvement interventions. Its routine implementation is poised to facilitate data-driven patient safety management and underpin continuous quality improvement efforts in Chinese healthcare institutions. The results of the content validity test revealed that, with the exception of the reporting of patient safety incidents, the scores pertaining to each dimension of the questionnaire and the total score were strongly correlated (0.706\u0026thinsp;\u0026lt;\u0026thinsp;r\u0026thinsp;\u0026lt;\u0026thinsp;0.833), thus indicating that the questionnaire exhibited good content validity [\u003cspan citationid=\"CR8\" class=\"CitationRef\"\u003e8\u003c/span\u003e].\u003c/p\u003e \u003cp\u003eStructural validity refers to the degree of consistency between the actual measured structure of the questionnaire and the theoretical structure; it is thus the most important indicator of validity. In this research, an exploratory factor analysis was conducted to extract 10 common factors that were associated with a cumulative variance contribution rate of more than 70%.
