It is a predictor of physical and psychological effects on human health. This study aims to determine the prevalence and associated factors of burnout among nurses in Birtamod Municipality in Nepal. Methodology: Between September and November 2022, a cross-sectional study was conducted among 427 randomly selected nurses working in 14 different hospitals in Birtamod Municipality. To assess the prevalence of burnout, a validated Copenhagen Burnout Inventory tool was used. Nurses voluntarily provided data on sociodemographic, work-related, and perceived stress. Bivariate and binary logistic regression were applied to identify the associated burnout factors. Results: Out of 427 nurses, 401 completed the survey. The overall prevalence of burnout was 36%. Personal, work-related, and client-related burnout was 53.8%, 35.4%, and 26.6% respectively. Living with family (β=2.295, p=0.011), working in the pediatric ward (β=3.231, p=0.004) and perceived stress (β=4.054, p=0.026) were associated with personal burnout. Similarly, working in the pediatric ward (β=4.426, p=0.001) and perceived stress (β=2.451, p=0.020) were significantly associated with work-related burnout. Working in the pediatric ward (β=4.856, p=0.002), working for more than 8 hours a day (β=1.750, p=0.030) and perceived stress (β=3.671, p=0.009) were associated with client-related burnout. Conclusion: The burden of burnout reported in the present study among nurses indicates it to be an important mental health issue in Nepal. Perceived stress and work department are major predictors of three burnout subordinates. Burnout among nurses is preventable using effective interventions and preventive strategies. Burnout Nurses Psychological condition Nepal Developing countries Figures Figure 1 Figure 2 Introduction Burnout is a psychological term commonly used for the experience of long-term exhaustion, frustration, anger, and depression. It is associated with feelings of difficulty and hopelessness while dealing at work ( 1 ). Burnout is known to be a significant issue in the current working environment as a result of a substantial increase in its prevalence over the past decade ( 2 ). There is a significant difference between the prevalence of burnout among different regions. The total prevalence of burnout among nurses globally was 11.23–30%, and it was 52% in Asian countries ( 3 – 5 ). Some proven factors influencing the development of burnout are high job stress, workload, shift at work, marital status, working hours, and lack of organizational support ( 6 – 8 ). Other associated factors for the development of burnout are inadequate staffing, time pressure, role conflict, poor leadership, and job insecurity ( 9 ). High job stress was associated with the development of burnout ( 6 ). Burnout affects physical, psychological, and work-related aspects of life. It predisposes to physical conditions like type 2 diabetes, coronary heart disease, musculoskeletal pain, tiredness, and headaches, along with psychological illness ( 10 ). In a healthcare setting, evidence shows a correlation between working conditions and service outcomes such as quality of service delivery, patient safety, sickness absenteeism, low commitment towards the organization, job dissatisfaction, and turnover ( 11 , 12 ). The psychological outcomes of burnout were symptoms of depression, insomnia, use of psychological medications, and hospitalization for mental disorders ( 10 ). Healthcare Workers (HCWs) are constantly subjected to emotionally debilitating pressure during the complex delivery of care and treatment to patients, which puts them at risk of developing occupational burnout ( 13 ). Among HCWs, nurses play a major role as a part in service delivery and are reported to experience higher psychological illness along with a higher prevalence of burnout ( 4 , 14 – 16 ). Identification and characterization of work-related barriers and other determining factors of burnout are essential to ensure the development of appropriate health-promoting approaches and sustain their health. Some international studies conclude a fact on the relation between region, place of work, and burnout ( 17 , 18 ). Few studies on job satisfaction, level of stress, depression, and burnout have been done in Nepal. For instance, moderate to high job stress and burnout are prevalent among healthcare professionals in Nepal ( 19 – 21 ). However, sparse evidence has been explored on burnout among nurses around this area of the country. Therefore this cross-sectional study focuses on studying burnout among nurses at Birtamode municipality. Furthermore, this study also explores if burnout is associated with sociodemographic, work-related factors, and level of stress among the study participants. Hospital settings having better workplace environments for health care workers was seen to prevail lower prevalence of burnout ( 7 ). Evidence from this study provides the basis for promoting the physical and mental health of healthcare workers and aids in declining the increasing trend of burnout and thus improving work performance and service delivery. Methods This is a cross-sectional sectional designed to collect data on magnitude of burnout and determine its associated factors among nurses working at Birtamod municipality. Study participants Data were collected from registered nurses with at least 1 year of work experience. The respondents were Auxiliary Nurse Midwifery (ANM), Proficiency Certificate Level in Nursing (PCL), Bachelor in Nursing (BN), B.Sc. Nursing and Masters in Nursing (MN). Data collection was done from September to November 2022. Sample size and sample procedure The sample size was determined by using the Cochrane formula (n = Z²pq/d²). In a study conducted in Nepal using CBI to estimate burnout among medical students and residents, the prevalence was found to be 48.8% ( 21 ). Hence, the prevalence of burnout among nurses has been assumed to be 50%. So, p = 0.50, q = 0.50, d = allowable error, taken as 5% and 95%CI, initial n = 384.16%, assuming non-response rate as 10%, sample size of study (n) is 427. Therefore, a total of 427 nurses were included in this study. The list of nurses was obtained from all fourteen hospitals around the municipality. 641 was the sampling frame for the study. Using simple random sampling 427 samples were selected. We communicated with the Medical Director, Human Resources head, matron, and department head of each hospital. An explanation was made of the research, its purpose, and its processes, emphasizing that participation was voluntary. The participants were asked to sign the consent form. Questionnaires were distributed on morning and evening shifts to meet the nurses as per guidance from the department head. Nurses with night shit were met in the morning before shift handover. The completed questionnaires were collected on the same day, a precise checking was done to ensure there were no omissions or errors. The same process was continued in each hospital. Follow-up was made to meet those on leave on the day of data collection. Data collection A self-administered questionnaire comprising four sections was used for data collection. The first section comprises structured questionnaires of sociodemographic data of participants. The sociodemographic variable includes age group, religion, marital status, family status, living arrangement, educational level, monthly income, and work experience. The second section includes of structured question comprising of the workplace environmental profile of participants. Workplace environment variables comprise types of hospitals, employment contracts, working hours per day, working department, job title, and support from colleagues. The third and fourth section includes a validated tool for determining perceived stress and burnout using the Perceived Stress Scale(PSS) and Copenhagen Burnout Inventory (CBI). CBI is a highly validated tool as a measure of burnout in three dimensions: Personal burnout, work-related burnout, and client-related burnout. The six-item personal burnout measures the level of physical and mental exhaustion by an individual. The seven items of work-related burnout assess the degree of physical and psychological exhaustion from individual work. The six-item client-related burnout estimates the amount of physical and psychological exhaustion while dealing with clients ( 22 ). PSS was used to measure the level of stress among nurses. It comprises a set of 10 items scored on a 5-point Likert scale ( 23 ). Analysis Collected data were entered in Microsoft Excel and exported to SPSS V.25 for statistical analysis. The descriptive data were summarized in terms of frequency and proportions. To determine the relationship between dependent and independent variables, bivariate analysis was carried out using Pearson's Chi-Square at 95%CI and 5% level of significance, that is, p-value < 0.05. The variables found to be significant in bivariate analysis were considered for multivariate analysis using binary logistic regression to determine the adjusted effect of each factor on the dependent variable. Before multivariate regression analysis, the multicollinearity between the independent variables was tested using the Variance Inflation Factor (VIF) test, highest VIF was recorded to be 2.564, thus no collinearity was seen among the independent variables. The Hosmer–Lemeshow test for goodness of fit was also performed. Result The research instrument was distributed to the sample of 427 nurses where 401(n = 401) were filled and returned, giving a response rate of 93.91%. Sociodemographic characteristics All the participants were female and the majority (79.1%) belonged to the Hindu religion. Age of participants were divided into 5 groups, about half (48.6%) of the participants were in the age group 20–24 years of age. Nearly one-third of nurses were married (33.9%) of them, 16.7% had children. More than half of the participants (63.6%) were living with their families. Only 10.2% were university graduates. Likewise, the median monthly income of respondents was Rs.17,950. The majority of participants (94.0%) were working as a temporary employee. More than half (56.9%) of the participants were working for longer than 8 hours a day. Compared to other departments Intensive care unit(ICU) holds a high ( 18.5%) enrollment of the sample for the study, low (2.7%) from High-density unit(HDU). Over half (52.9%) of the participants were titled a job as a nurse. Almost the majority(98.8%) of participants were getting support from their colleagues at their workplace. More than three-fourths (77%) of the participants reported a moderate level of perceived stress. Participants reported with high (6%) level of perceived stress (Fig. 1 ). Prevalence of burnout It has been found that 36% of nurses were suffering from burnout. In addition, 53.9% experienced personal burnout, 35.4% work-related burnout, and 26.7% client-related burnout. Similarly family status, educational level, monthly income, types of hospital, work department, job title, and perceived stress as associated factors with work-related burnout Accordingly, factors that showed association with client-related burnout were educational level, monthly income, work department, job title, employment contract, working hour and stress. (online supplemental table 1,2 3) Adjusted relationship of explanatory variables with Burnout Binary logistic regression was performed on variables that were found significant at a 5% level of significance in bivariate analyses. The adjusted relationships of independent variables with three subscales of burnout were assessed at 95% CI in the binary logistic regression. Findings showed that living arrangements, those who live with family were 2.3 times (AOR = 2.295, CI = 1.205–4.357) more likely to develop personal burnout compared to those who live alone. The analysis also showed that those who worked in the surgical department were 2 times (AOR = 2.024, CI = 1.040–3.938) and those working in the pediatric ward 3.2times(AOR = 3.231, CI = 1.445–7.228) more likely to develop personal burnout than those working in medical and neuro ward. Similarly, participants with high perceived stress were 4.6 times (AOR = 4.054, CI = 1.183–13.901) more likely to develop personal burnout compared to those with low perceived stress. Monthly income did not show a statistically significant relation with personal burnout. Similarly, nurses experiencing moderate stress were 2.4 (AOR = 2.451, CI = 1.154–5.205) times and those experiencing high stress were 3.7 (AOR = 3.728, CI = 1.118–12.336) times more likely to develop work-related burnout compared to those experiencing low stress. The variables that did not show significant association with work-related burnout were monthly income, educational level, types of hospital, and job title. Likewise, those having moderate stress were 3.7 times (AOR = 3.671, CI = 1.377–9.789), and those with high stress were 6.3 times (AOR = 6.319, CI = 1.578–25.311) likely to develop client-related burnout compared to low stress. Nurses whose working hour was more than 8 hr per day were almost 2 times (AOR = 1.750, CI = 1.055–2.903) more likely to develop client-related burnout compared to those working less than 8hr a day. The variables that did not show a significant association with client-related burnout are monthly income, employment contract, educational level, and job title. The result exhibited a prevalence of burnout among nurses to be 36%. Our study determined the prevalence of burnout score in 3 domains; where 53.8% of respondents had personal burnout, 35.4% had work-related burnout and 26.6% had client-related burnout. The prevalence in this study is comparable to the 33.3% reported in Singapore, and 37.6% in India ( 24 , 25 ). Compared to many studies, our study reported a lower prevalence of burnout. For example, the prevalence was 59.4% in Portugal ( 26 ) and 69% in India ( 27 ). Similarly, 76.5% of nurses experienced burnout in Nepal ( 28 ). The highest-reported subcategory in this study was personal burnout. This was supported by the study conducted in Nepal ( 29 ) and Saudi Arabia ( 30 ). Work-related burnout was the subscale with the highest average score ( 26 ). The disparity in result might be due to differences in the study area as a single-centered study, to time of the study, the difference in the composition of the sample, difference in the measuring scale. Binary logistic regression in this study unveiled, working in the pediatric department and perceived stress as correlated factors for all three subordinates of burnout. In line with other studies, our study showed a significant association of perceived stress with personal(p-0.007), work-related (p-0.000), and client-related burnout (p-0.000) ( 27 , 31 , 32 ). A systematic review in low-middle-income countries (LMIC) determined the correlation between higher job stress with high levels of burnout ( 6 ). Elements like emergencies, unsupportive relationships at the workplace, high job demands, and uncooperative patients and visitors were often regarded as generating workplace stress. Oftentimes, the most useful coping strategies for minimizing stress at the workplace were self-controlling, planful problem-solving, and seeking social support ( 33 ). Another important correlate for burnout in our study was the work department. Correlation of the pediatric department with all subordinates of burnout and surgical unit with personal burnout. A similar result was unveiled by the study in Ethiopia and Nepal ( 8 , 31 ). This was supported by another study where those posted in ICU duty experienced the highest level of burnout ( 34 ). Furthermore, our study revealed working for longer hours(> 8 hours) a day had an association with client-related burnout. A result of a study in Uganda concluded that nurses who worked for less than 10 hours a day were 31% less likely to develop burnout ( 35 ). Another study’s findings showed that those working for more than 12 hours of shift a day were classified as a predictor for the development of burnout ( 12 ). A systematic review and meta-analysis reported, that working for longer hours influenced in development of burnout among nurses ( 36 , 37 ). Similar findings were shown in a study in Portugal and Saudi Arabia where the longer time spent in the working institution, the higher the burnout ( 26 , 30 ). Additionally, findings of this study concluded that living with family had higher odds of developing personal burnout than those living single. Family responsibility can create stress that can ultimately permeate the pressure in the workplace. This impacts the work performance and service effectiveness of healthcare professionals ( 38 ). Studies among healthcare workers showed that those who experience complex and challenging situations, along with family demands, were more likely to experience burnout ( 38 , 39 ). However, the report of a study conducted in Nepal differs from the findings of our study ( 31 ). Limitations and recommendations This study uses validated screening tools to understand burnout and perceived stress. The sampling technique and high response rate are the fair strong aspects of this study. Despite these, there are some limitations to consider. The information on burnout is self-reported by the respondents thus there could be some chances of reporting bias. The cross-sectional nature of the study makes it difficult to establish temporal correlations, restricting our capacity to draw causal conclusions regarding the relationship between dependent and independent factors; as a result, we were only able to identify the associated characteristics rather than the cause and effect of burnout. The results of the study will add insight into burnout and will work as a baseline in the exploration of other factors related to this issue. This study highlights the need to evaluate and develop interventions to improve the well-being of nurses with a focus on tackling stress at work. Conclusion This is one of the few studies in Nepal exploring burnout among nurses around the eastern part of the country. To conclude, this study has provided an important contribution to the risk profile of burnout among nurses working in different hospitals within Birtamod Municipality. Burnout was prevalent among 36% of nurses. Nurses experiencing perceived stress, particularly ones working in the pediatric department were especially found to have an association with three subordinates of burnout. The study identified living with family and working in the surgical department as contributors to personal burnout. Working for more than eight hours a day was a contributor to client-related burnout. Age of participants were divided into 5 groups, about half (48.6%) of the participants were in the age group 20\u0026ndash;24 years of age. Nearly one-third of nurses were married (33.9%) of them, 16.7% had children. More than half of the participants (63.6%) were living with their families. Only 10.2% were university graduates. Likewise, the median monthly income of respondents was Rs.17,950. The adjusted relationships of independent variables with three subscales of burnout were assessed at 95% CI in the binary logistic regression.\u003c/p\u003e \u003cp\u003eFindings showed that living arrangements, those who live with family were 2.3 times (AOR\u0026thinsp;=\u0026thinsp;2.295, CI\u0026thinsp;=\u0026thinsp;1.205\u0026ndash;4.357) more likely to develop personal burnout compared to those who live alone. The analysis also showed that those who worked in the surgical department were 2 times (AOR\u0026thinsp;=\u0026thinsp;2.024, CI\u0026thinsp;=\u0026thinsp;1.040\u0026ndash;3.938) and those working in the pediatric ward 3.2times(AOR\u0026thinsp;=\u0026thinsp;3.231, CI\u0026thinsp;=\u0026thinsp;1.445\u0026ndash;7.228) more likely to develop personal burnout than those working in medical and neuro ward. Similarly, participants with high perceived stress were 4.6 times (AOR\u0026thinsp;=\u0026thinsp;4.054, CI\u0026thinsp;=\u0026thinsp;1.183\u0026ndash;13.901) more likely to develop personal burnout compared to those with low perceived stress. It is a predictor of physical and psychological effects on human health. This study aims to determine the prevalence and associated factors of burnout among nurses in Birtamod Municipality in Nepal.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethodology:\u003c/strong\u003e Between September and November 2022, a cross-sectional study was conducted among 427 randomly selected nurses working in 14 different hospitals in Birtamod Municipality. To assess the prevalence of burnout, a validated Copenhagen Burnout Inventory tool was used. Nurses voluntarily provided data on sociodemographic, work-related, and perceived stress. Bivariate and binary logistic regression were applied to identify the associated burnout factors.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e Out of 427 nurses, 401 completed the survey. The overall prevalence of burnout was 36%. Personal, work-related, and client-related burnout was 53.8%, 35.4%, and 26.6% respectively. Living with family (β=2.295, p=0.011), working in the pediatric ward (β=3.231, p=0.004) and perceived stress (β=4.054, p=0.026) were associated with personal burnout. Similarly, working in the pediatric ward (β=4.426, p=0.001) and perceived stress (β=2.451, p=0.020) were significantly associated with work-related burnout. Working in the pediatric ward (β=4.856, p=0.002), working for more than 8 hours a day (β=1.750, p=0.030) and perceived stress (β=3.671, p=0.009) were associated with client-related burnout.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion: \u003c/strong\u003eThe burden of burnout reported in the present study among nurses indicates it to be an important mental health issue in Nepal. Perceived stress and work department are major predictors of three burnout subordinates.
