This paper reports a single case of ileocecal (cecum and ileum region) endometriosis in a 32-year-old nulliparous woman presenting with epigastric pain and suspected bowel obstruction. She had a history of right ovarian surgery for endometriosis and worsening bowel obstruction symptoms around menstruation, leading surgeons to suspect adhesive bowel obstruction due to intestinal endometriosis; during ileocecal resection, a submucosal tumor was sampled for intraoperative rapid aspiration cytology. Cytology showed clusters of endometrial-gland–like cells with characteristic stromal patterns, while histology demonstrated endometrial glands and stroma in submucosa, muscularis, and subserosa, and immunohistochemistry supported ER and PgR positivity in glandular components and CD10 positivity in stromal components; the authors emphasize the importance of detailed screening given endometriosis can occur in various organs.
