However, the impact of age on the clinicopathological features and prognosis of colorectal mucinous adenocarcinoma (MAC) remains underexplored. Materials and Methods Clinicopathological data of CRC patients who underwent curative surgery and were pathologically diagnosed with MAC across 23 hospitals in China from 2016 to 2021 were collected. Data from MAC patients in the Surveillance, Epidemiology, and End Results (SEER) database were also collected. The clinicopathologic features and prognosis of MAC in patients in different age groups were analysed. Results The clinicopathological features of MAC patients in different age groups significantly differed. In both the SEER cohort and the Chinese cohort, patients > 60 years old were more likely to be female, have larger tumours, and have a lower pathological N category and TNM category (P < 0.05). In the SEER cohort, significant differences were observed in OS among the young, middle-aged, and elderly patient groups (65.2% vs. 70.7% vs. 66.2%, P < 0.01). In the Chinese cohort, the OS rates were similar (82.9% vs. 89.1% vs. 80.0%, P = 0.06). There was no significant difference in DFS among the young, middle-aged, and elderly groups (80.0% vs. 85.6% vs. 79.2%, P = 0.20). Conclusion In the SEER cohort, patients < 60 years old had more cases with advanced TNM stages, more patients with tumours located in the left colon and rectum, and a higher proportion of male patients. The clinicopathological features were similar among the three groups, except that the young population aged 18–40 years had more advanced TNM stages at diagnosis in the Chinese cohort. Patients in the 40–60 years age group had worse OS than those in the > 60 age group. Young MAC patients should receive more attention because of their aggressive features and poor diagnosis. Figures Figure 1 Figure 2 Figure 3 Figure 4 INTRODUCTION CRC is the third most commonly diagnosed cancer worldwide [1]. MAC accounts for approximately 10–20% of all CRC cases [2]. The World Health Organization defines MAC as a lesion in which more than 50% of the lesion is composed of pools of extracellular mucin containing malignant epithelium [3]. In terms of prognosis, a meta-analysis including 56 studies with a total of 803,157 patients revealed that MAC is an adverse prognostic factor for OS in CRC patients (HR 1.04; 95% CI: 1.03–1.04, P < 0.01) [4]. Recent studies have shown that MACs have distinct genetic backgrounds and clinicopathological and prognostic features compared with those of CRCs [5], and an increasing number of studies have suggested that MACs should be treated as a single tumour. The prognostic significance of primary different age groups in CRC has garnered considerable interest. Several studies have suggested that young patients with CRC have different clinicopathological features and are more likely to present with advanced disease than older patients are [6–7]. Nonetheless, the impact of different age groups on the clinicopathological features and prognosis of MAC remains unclear. Therefore, we conducted this study to provide more precise screening methods and comprehensive postoperative treatment for patients with MAC of different age groups. To evaluate the impact of age on MAC outcome, we stratified the patient cohort into three age groups: Group 1, 18–40 years; Group 2, 41–60 years; and Group 3, >60 years. The clinicopathological characteristics and prognostic features were compared among the different groups. MATERIALS & METHODS Study Population The clinicopathological data of CRC patients who underwent curative surgery and were pathologically diagnosed with MAC across 23 hospitals in China from 2016 to 2021 were collected. Data from MAC patients in the SEER database from 2000 to 2021 were also collected. In this study, all MAC patients were included, whereas those with multiple primary tumours, multiple CRCs, mucus components < 50%, stage IV MAC, and insufficient clinical and/or pathological information were excluded (Fig. 1 ). In the Chinese cohort, data, including sex, age, tumour location, tumour size, perineural invasion (PNI), lymphovascular invasion (LVI), T stage, N stage, and TNM stage, were collected from clinical records. Follow-up Method Histories and physical examinations were given every 3 to 6 months for 2 years and then every 6 months for a total of 3 years. Follow-up included physical examinations, tumour marker assessments, computed tomography (CT) scans, and colonoscopies to detect postoperative recurrence. CT scans were typically performed every six months for five years, whereas colonoscopies were performed in the first three years. Data Selection Patients were categorized into three groups based on patient age: (1) Group 1, 18–40 years; (2) Group 2, 41–60 years; and (3) Group 3, >60 years. Data on age, sex, tumour size, LVI, and PNI at presentation were recorded for each patient according to the Union for International Cancer Control (8th edition) tumour‒node‒metastasis (TNM) classification. Follow-up data were obtained through patient interactions in the hospital or via telephone communication. Study Endpoint The endpoint of this study was survival time. Deaths from any cause, recurrence or metastasis were considered events. Patients still alive at the last follow-up, with or without disease, were censored. Survival time was measured from the date of surgery to the date of death or the last known follow-up. OS was defined as the period from surgery to death from any cause, and disease-free survival (DFS) was defined as the period from surgery to recurrence or any death from any cause. Statistical Analysis Chi-square analysis, Fisher's exact test, or Kruskal‒Wallis ANOVA was used to assess differences in categorical variables where appropriate. Kaplan‒Meier curves were generated to assess survival, which was compared via log-rank tests. Univariable and multivariable survival analyses with Cox regression models were conducted to identify prognostic factors. Hazard ratios (HRs) are reported with 95% confidence intervals (95% CIs). The adjusted variables included sex, age, tumour location, LVI, PNI, tumour size, and TNM stage. A two-sided P value ≤ 0.05 was considered statistically significant. All of the statistical analyses were conducted via SPSS software (version 25.0 for Windows, IBM SPSS Statistics, IBM Corporation, Armonk, NY). RESULTS Study Cohort Characteristics and Clinicopathological Features A total of 4,166 patients in the SEER cohort and 2,431 patients in the Chinese cohort met the inclusion criteria. The SEER cohort comprised 263 (6.3%) patients in Group 1, 1521 (36.5%) in Group 2, and 2382 (57.2%) in Group 3. In the Chinese cohort, 184 (7.6%) patients were in Group 1, 955 (39.3%) were in Group 2, and 1292 (53.1%) were in Group 3 (Table 1 ). Compared with those in Group 1/2, patients in Group 3 were more likely to be female, have larger tumours, and belong to a lower pathological T category, N category and TNM category (P < 0.05) (Table 2 ). In the Chinese cohort, similar trends were noted, with Group 1 patients also being more likely to have a higher pathological N category and TNM category than Group 2 and Group 3 patients (all P < 0.05) (Table 3 ). Furthermore, the positive rate of LVI in Group 1 patients was higher than that in Group 3 patients. (Table 3 ). These findings indicate that MAC tumours of different ages have different clinicopathological characteristics. Overall, younger patients are more likely to be diagnosed at an advanced stage and have more aggressive clinicopathological features. Prognosis of MAC Patients by Age To further elucidate the differences among the different groups, the diagnosis of the included patients were depicted. The 3-year OS rates for MAC patients were 84.8% in the Chinese cohort and 68.8% in the SEER cohort. For patients in the Chinese cohort, the 3-year OS rates for Group 1, Group 2 and Group 3 were 82.9%, 89.1%, and 80.0%, respectively (P = 0.06) (Fig. 2 A). There was no significant difference between Group 1 and Group 2 or between Group 1 and Group 3 (P = 0.26 and 0.92) (Fig. 2 B and 2 C), but there was a significant difference between Group 2 and Group 3 (P = 0.02) (Fig. 2 D). The trends in the DFS rates were similar (Group 1 vs. Group 2 vs. Group 3, 80.0% vs. 85.6% vs. 79.2%, P = 0.20) (Fig. 3 ). In the SEER cohort, the 3-year OS rates for Group 1, Group 2 and Group 3 were 65.2%, 70.7%, and 66.2%, respectively (P < 0.01)(Fig. 4 A). There was no significant difference between Group 1 and Group 2 or between Group 1 and Group 3 (P = 0.07 and 0.83) (Fig. 4 B and 4 C), but there was a significant difference between Group 2 and Group 3 (P = 0.02) (Fig. 4 D). The prognosis of MAC differ among different age groups in Eastern populations but are similar in the Chinese cohort. Notably, the 40–60 years age group had worse OS than the > 60 years age group in both cohorts. Factors Affecting OS and DFS in MAC Patients Univariate and multivariate analyses were conducted to assess the factors associated with the prognosis of MAC. In the Chinese cohort, LVI (HR = 2.741, 95% CI = 1.751–4.382, P < 0.01), PNI (HR = 1.911, 95% CI = 1.2–3.043, P < 0.01), and advanced TNM stage (HR = 3.524, 95% CI = 2.043–6.079, P < 0.01) were identified as risk factors for poor OS (Table 4 ). Multivariate analysis further revealed that advanced TNM stage (HR = 2.742, 95% CI = 1.531–4.909, P < 0.01) was an independent risk factor for poor OS (Table 4 ). In the SEER cohort, different age groups (P < 0.01), tumour locations (P < 0.01), tumour sizes ≥ 5 cm (HR = 1.375, 95% CI = 1.227–1.541, P < 0.01), and advanced TNM stages (HR = 1.928, 95% CI = 1.73–2.148, P < 0.01) were identified as independent risk factors for poor OS (Table 4 ). Multivariate analysis further revealed that tumour location (P = 0.01), tumour size ≥ 5 cm (HR = 1.393, 95% CI = 1.242–1.562, P < 0.01) and advanced TNM stage (HR = 1.907, 95% CI = 1.709–2.127, P < 0.01) were independent risk factors for poor OS (Table 4 ). In terms of DFS, in the Chinese cohort, a tumour size ≥ 5 cm (HR = 1.327, 95% CI = 1.076–1.495, P = 0.02) and advanced TNM stage (HR = 1.597, 95% CI = 1.142–2.207, P < 0.01) were significant independent risk factors (Table 5 ). TNM stage remains the most important factor affecting the OS and DFS of patients with MAC. However, more attention should be given to MAC patients with tumours located in the right colon or with a tumour size larger than 5 cm, and close follow-up is necessary. DISCUSSION Our results demonstrate that age is not an independent prognostic factor for survival. However, disparities in patient cohorts make accurate comparisons difficult. For instance, some studies included cohort patients with preexisting factors predisposing them to CRC, such as a hereditary gastrointestinal cancer syndrome [10]. These factors are likely to lead to an overestimation of the incidence of CRC in the young population. Similarly, there is a lack of consensus on the definition of ‘young-onset CRC’, with researchers using various age cut-offs including 35, < 40 or < 50 years. In the O’Connell review, 37 (67%) of the included studies defined ‘young onset’ as patients under the age of 40 years [10]; hence, we used this definition. With respect to clinicopathological features, our results revealed that left-sided cancer and the rectum were the most common sites of MAC in all three groups. Overall, in patients under the age of 60 years (Groups 1 and 2), left-sided tumours and the rectum accounted for more than 70% of all cases of MAC. These findings are consistent with previously published studies that reported a predilection of CRC for the left colon and rectum in younger patients [11]. Furthermore, our results revealed that the 18–40-year-old group presented with advanced disease (stage III) compared with the other two groups. These findings are similar to those of the review by O’Connell et al., in which the authors demonstrated that 66% of patients under the age of 40 presented with advanced disease [10]. Similar findings have been reported in other recent studies [12–13]. Some authors have suggested that the higher rate of advanced disease observed in younger patients could be attributed to delayed diagnosis because, first, younger patients are more likely to dismiss red-flag bowel symptoms. In addition, physicians are less likely to refer young patients for endoscopic investigations because their symptoms are benign [14]. Our studies suggest that although young patients are more likely to have advanced disease, their outcome is similar to that of older patients. This is because younger patients have better performance status and are therefore more likely to tolerate more aggressive treatment. We acknowledge several limitations to this study. For instance, factors such as a patient’s comorbidities, weight and body mass index and family history of MAC were not assessed in this study. These are clinically relevant variables that could affect survival outcomes. Another limitation is the small sample size, especially in the 18–40-year age group. This limited our ability to perform stage-dependent stratification of OS and DFS between the three groups. The SEER database lacks data on whether patients have LVI or PNI and the time period from surgery to recurrence or death from any cause, which imposes limitations on the research. CONCLUSION In the SEER cohort, patients <60 years old were more likely to have advanced TNM stages, more tumours located in the left colon and rectum, and a higher proportion of male patients. The clinicopathological features of Chinese patients in various age groups are highly similar. However, the young population aged 18–40 years has more advanced TNM stages at diagnosis. Middle-aged patients have a poor outcome and should receive more attention during follow-up. Declarations Financial support and sponsorship none. However, disparities in patient cohorts make accurate comparisons difficult. For instance, some studies included cohort patients with preexisting factors predisposing them to CRC, such as a hereditary gastrointestinal cancer syndrome [10]. These factors are likely to lead to an overestimation of the incidence of CRC in the young population. Similarly, there is a lack of consensus on the definition of \u0026lsquo;young-onset CRC\u0026rsquo;, with researchers using various age cut-offs including 35, \u0026lt; 40 or \u0026lt;\u0026thinsp;50 years. These findings are consistent with previously published studies that reported a predilection of CRC for the left colon and rectum in younger patients [11]. However, the impact of age on the clinicopathological features and prognosis of colorectal mucinous adenocarcinoma (MAC) remains underexplored.\u003c/p\u003e\u003ch2\u003eMaterials and Methods\u003c/h2\u003e\u003cp\u003eClinicopathological data of CRC patients who underwent curative surgery and were pathologically diagnosed with MAC across 23 hospitals in China from 2016 to 2021 were collected. Data from MAC patients in the Surveillance, Epidemiology, and End Results (SEER) database were also collected. The clinicopathologic features and prognosis of MAC in patients in different age groups were analysed.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e\u003cp\u003eThe clinicopathological features of MAC patients in different age groups significantly differed. In both the SEER cohort and the Chinese cohort, patients\u0026thinsp;\u0026gt;\u0026thinsp;60 years old were more likely to be female, have larger tumours, and have a lower pathological N category and TNM category (P\u0026thinsp;\u0026lt;\u0026thinsp;0.05). In the SEER cohort, significant differences were observed in OS among the young, middle-aged, and elderly patient groups (65.2% vs. 70.7% vs. 66.2%, P\u0026thinsp;\u0026lt;\u0026thinsp;0.01). In the Chinese cohort, the OS rates were similar (82.9% vs. 89.1% vs. 80.0%, P\u0026thinsp;=\u0026thinsp;0.06). There was no significant difference in DFS among the young, middle-aged, and elderly groups (80.0% vs. 85.6% vs. 79.2%, P\u0026thinsp;=\u0026thinsp;0.20).\u003c/p\u003e\u003ch2\u003eConclusion\u003c/h2\u003e\u003cp\u003eIn the SEER cohort, patients\u0026thinsp;\u0026lt;\u0026thinsp;60 years old had more cases with advanced TNM stages, more patients with tumours located in the left colon and rectum, and a higher proportion of male patients. The clinicopathological features were similar among the three groups, except that the young population aged 18\u0026ndash;40 years had more advanced TNM stages at diagnosis in the Chinese cohort.
