This case report describes a 39-year-old woman presenting with an abdominal wall mass that exhibited cyclical pain correlated with her menstrual cycle. Magnetic resonance imaging identified a hemorrhagic multiloculated mass within the rectus abdominis muscle, leading to a preoperative diagnosis of endometrioma. Histologic analysis of the resected specimen confirmed the presence of ectopic endometrial tissue in the abdominal wall. This paper is centrally about endometriosis — specifically the rare presentation of extra-pelvic endometrioma in the abdominal wall.
