Abstract

Abstract Background: Incidence of Hepatocellular carcinoma (HCC) has increased over the past few decades to become the most common primary hepatic malignancy. Most patients with HCC have advanced unresectable disease, resulting in limited treatment options with curative intent and poor survival outlooks. Results: In this study on Egyptian patients, hepatocellular carcinoma (HCC) primarily occurred in the sixth decade of life, often alongside liver cirrhosis. No significant gender or smoking differences were found between HCC and cirrhotic groups, suggesting smoking isn't a major HCC risk factor. Diabetes mellitus (DM) and hypertension (HTN) were more common in HCC patients, reflecting their roles in cirrhosis and HCC development. Serum alpha-fetoprotein (AFP) levels were significantly higher in HCC patients. Neuropilin-1, a novel marker, showed a significant increase in HCC patients compared to cirrhotic ones, with a strong correlation to liver function markers (AST, ALT, AFP, albumin) and Barcelona and Child scores. Neuropilin-1 demonstrated higher sensitivity (96.67%) and specificity (80%) for HCC detection compared to AFP, highlighting its potential as a superior biomarker for diagnosing HCC. Conclusion: Serum neuropilin 1 level was significantly higher in patients with HCC regardless presence of combined conditions as DM and HTN or viral markers and mildly elevated in patients with liver cirrhosis compared to HCC group. Neuropilin 1 is more sensitive and more specific than AFP. Using neuropilin 1 will improve the diagnostic field and can help in early detection of HCC in surveillance programs together with AFP.

HCC exacts a heavy disease burden and is a leading cause of cancer-related death in many parts of the world and is the fourth most common cause of cancer-related death overall worldwide (1). Alpha-fetoprotein (AFP) remains the most widely used serum biomarker for the diagnosis and prognosis of hepatocellular carcinoma (HCC). The role of AFP has become controversial due to nearly 30% of early-stage HCC patients being AFP negative (2). While NRP1 has been extensively studied and characterized, NRP2 has received fewer studies. Nonetheless, the significance of both NRPs in cancer, including HCC, has recently been highlighted due to their significant association with crucial cellular and molecular mechanisms involved in tumor progression ( 3 ). The role of Neuropilin 1 in the progression of HCC is not fully comprehended..NRP1 Expression has been detected in liver sinusoidal endothelial cells, in hepatic stellate cells, in the heap 129 hepatoma cell line, and was found to be increased following partial hepatectomy, under shear stress conditions and in liver cirrhosis although these data point out the important role of Neuropilin 1 in liver function and possible implication in liver carcinogenesis ( 4 ). METHODS Patients: This study had been carried out on 60, age range more than 18 year selected from internal medicine and hepatology outpatient clinics and inpatient wards at Ain shams university hospital from January 2024 to June 2024, after informed consent were taken from the patients Subjects were divided as follow: Group 1 : include 30 HCC diagnosed by alpha fetoprotein and imaging. Group 2 : include 30 matched cirrhotic patients without HCC divided according to child -Pugh scoring system. Methodology: Type of Study : Case control study Proposed Study Design: Comparative cross-sectional study Study Setting: After obtaining approval from the Research Ethical Committee of Ain Shams University, this study will be conducted in the Internal medicine and Hepatology department and outpatient clinic and clinical pathology lab at Ain Shams University Hospitals under supervision of Dr Mariem Elsaid lecturer clinical pathology at Ain Shams university. Study Period : 6 months Design: Study will be conducted on 60 Egyptian patients (30 HCC patients, 30 liver cirrhosis without HCC) Study Population: Cirrhotic Egyptian Patient Treated in Ain Shams University. Selection criteria for cases: Inclusion Criteria: Age > 18 years old and patients diagnosed with liver cirrhosis with or without HCC. Exclusion Criteria: age < 18 years old, pregnant, female, patients diagnosed with malignancy other than HCC and patients who refuse to participate in the study. Sample Size: 60 patients will be divided into 2 groups: Group A (Case): 30 patients with liver cirrhosis and Hepa-tocellular carcinoma. Group B (Control): 30 patients with liver cirrhosis with-out Hepatocellular carcinoma Ethical Considerations: A signed written consent will be taken from all participants included in the study. All data are confidential; patients will never be mentioned by name in any published paper. The Scientific Research Ethics Committee of the Faculty of Medicine of Ain Shams University will approve the study before commencing work. Amount of blood needed for research: 2 cm from each person. There will be no leftovers or storage of samples after doing research. Study Tools and procedures: All patients will be subjected to the following: Full medical history and full clinical examination including general and local examination. Laboratory assessment : CBC, PT, PTT, INR. Liver profile: AST, ALT, S. Albumin, T. Bilirubin, Bilirubin, ALP,Gamma GT. Kidney function tests: BUN, Creatinine. -Electro- Lytes: Na, K. HBsAg, HCV Ab. Alpha-feto protein (AFP) and Neuropilin 1. Radiological assessment: Pelviabdominal ultrasound, confirmation of any suspicious hepatic focal lesion by Triphasic CT with contrast or Dynamic MRI with contrast, statistical Analysis of all data to measure sensitivity and specificity of serum Neuropilin 1 diagnosing HCC and all personal protective equipment will be available and strict precautionary measures will be taken to ensure maximum safety for all participants and working staff. Principle of the assay: This kit is an Enzyme-Linked Immunosorbent Assay (ELISA). The plate has been pre-coated with Human NRP1 antibody. NRP1 present in the sample is added and binds to antibodies coated on the wells. And then biotinylated Human NRP1 Antibody is added and binds to NRP1 in the sample. Then Streptavidin-HRP is added and binds to the Biotinylated NRP1 antibody. After incubation unbound Streptavidin-HRP is washed away during a washing step. Substrate solution is then added, and color develops in proportion to the amount of Human NRP1. The reaction is terminated by addition of acidic stop solution and absorbance is measured at 450 nm. Statistical Methods: data was collected, revised, coded and entered to the Statistical Package for Social Science (IBM SPSS) version 27. The quantitative data were presented as mean, standard deviations and ranges when parametric and median, inter-quartile range (IQR) when data found non-parametric. Also, qualitative variables were presented as numbers and percentages. The comparison between groups regarding qualitative data was done by using Chi-square test and/or Fisher exact test when the expected count in any cell found less than 5. The comparison between two independent groups with quantitative data and parametric distribution was done by using independent t-test while with non-parametric distribution were done by using Mann-Whitney test. The comparison between more than two groups regarding quantitative data and non-parametric distribution was done by using Kruskall-Wallis test . Spearman correlation coefficients were used to assess the correlation between two quantitative parameters in the same group. Receiver operating characteristic curve (ROC) was used to assess the best cut off point with its sensitivity, specificity, positive predictive value, negative predictive value and area under curve (AUC) of the studied marker. The confidence interval was set to 95% and the margin of error accepted was set to 5%. So, the p-value was considered significant as the following: P-value > 0.05: Non-significant (NS). P-value < 0.05: Significant (S). P-value < 0.01: Highly significant (HS). Results This study was carried out on 60 participants divided into 2 group: 30 patients cirrhotic with Hepatocellular Carcinoma. 30 cirrhotic patients without hepatocellular Carcinoma selected from Ain shams university hospitals, after informed consent were taken from the patients All patients were sex and age matched with average age of 61years old. HCC patients had significantly higher CHILD score with p value = 0.025 p value = 0.017., MELD score with p value = 0.017, AFP level with P value < 0,001, Neuropilin 1 level with P value < 0,001 Than cirrhotic patients. there was statistically significant positive correlation between: Neuropiln 1 and (AST, ALT, MELD) among all cases, neuropilin1 and (ALT) among cirrhotic group and neuropilin 1 and (ALT, AST, Barcelona score) among HCC group While there was statistically significant negative correlation between: Neuropiln 1 and (Albumin) among All cases There was no statistically significant correlation between: neuropiln 1 and (age, Hb, WBC, platelet, BUN, CR, total bilirubin, Direct bilirubin, NA, K, INR, child score) among ALL cases, neuropilin1 and (age, TLC, Hb, Platelet, BUN, CR, Direct bilirubin, AST,INR,CHILD, MELD,NA,K, total bilirubin) among cirrhotic group and neuropilin 1 and (age, TLC, Hb, Platelet, BUN, CR, Direct bilirubin, Total bilirubin, INR, CHILD,MELD,NA,K) among HCC group. The cut off point for neuropilin 1 level to differentiate between cases with HCC and cirrhotic cases was found > 10,7with sensitivity of 96,67%, specificity of 80% and area under curve (AUC) of 96,1%, AFP > 5,6 with sensitivity of 80%, specificity of 76,67% and AUC of 82,1%. DISCUSSION This study was carried out on 60 subjects, age range (28-91) year selected from Ain shams university hospitals, after informed written consent were taken from the patients, classified into 2 groups: Group A (HCC);30patients cirrhotic with hepatocellular carcinoma Group B (cirrhotic): 30 patients cirrhotic without hepatocellular carcinoma. In our study, we found that the serum AFP was significant increase in HCC patients compared to cirrhotic, this is in agreement with many several reports one of them is (5) that consider AFP in high serum levels as diagnostic for HCC combined with triphasic CT abdomen or dynamic MRI and many guidelines advised use of serum AFP combined with abdominal ultrasound for screening of cirrhotic patients for early detection of HCC as showed that AFP is still reliable marker in HCC. Regarding serum neuropilin1, which is the new marker addressed in this study, show that there was significant increase in neuropilin1 level in HCC group compared to cirrhotic with p value ˂ 0,001 agree with (6) that show in cross-sectional studies examining the association of neuropilin 1 with HCC, neuropilin 1 level was significantly higher in patients with HCC than in those cirrhotic. In our study we found that There was no statistically significant correlation between neuropilin 1 and (platelet, INR) among both HCC group and cirrhotic group against with (7 ) that found that negatively statistically significant correlation between neuropilin1 and platelet and positively statistically significant correlation between neuropilin1 and INR. In our study we found that there was no statistically Significant correlation between neuropilin 1 and (age, TLC, Hb, Creatinine) that agree with (6) and we found that no statistical correlation between neuropilin1 and bilirubin that agree with (8). In our study we found that there was a significant correlation between neuropilin 1 and (AST, ALT, AFP, albumin) that agree with (9) that found that there was significant correlation between neuropilin 1 and (AST, ALT, AFP, albumin). In our study there was positive significant correlation between MELD score and serum neuropilin level which agree with (7) and there was no significant correlation Between neuropilin1 level and CHILD score which agree with (6). In our study there was significant correlation between Barcelona score and neuropilin 1 which agree with (10) that was found Elevated serum neuropilin1 were detected in tumor stages B/C according to Barcelona staging system. Our study shows the sensitivity of serum neuropilin1 in detection of HCC was 96.67 % with specificity 80% with AUC 0.961 at cut off value > 10.7 ng / ml in compare with (6) That show serum neuropilin1 could detect HCC with sensitivity was 93.3% and specificity 80 % with AUC of 0.0842 at cut off value 4030 pg / ml. AFP in our study shows sensitivity in detection of HCC 80% with specificity 76,67% so AFP in our study show less sensitivity and specificity in detection of HCC than Neuropilin1 which agree with (9) that show serum NRP1 is a better diagnostic marker than AFP with an area under receiver operating curve of 0,971, compared with 0,862 for AFP. The limitations in the study include: 1 -small number of patients in our study. 2- No available investigation to exclude other malignancy like mammogram, colonoscopy. Conclusion Serum neuropilin 1 level was significantly higher in patients with HCC regardless presence of combined conditions as DM and HTN or viral markers and mildly elevated in patients with liver cirrhosis compared to HCC group. Neuropilin 1 is more sensitive and more specific than AFP. Using neuropilin 1 will improve the diagnostic field and can help in early detection of HCC in surveillance programs together with AFP. Most patients with HCC have advanced unresectable disease, resulting in limited treatment options with curative intent and poor survival outlooks\u003cstrong\u003e.\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e In this study on Egyptian patients, hepatocellular carcinoma (HCC) primarily occurred in the sixth decade of life, often alongside liver cirrhosis. No significant gender or smoking differences were found between HCC and cirrhotic groups, suggesting smoking isn't a major HCC risk factor. Diabetes mellitus (DM) and hypertension (HTN) were more common in HCC patients, reflecting their roles in cirrhosis and HCC development. Serum alpha-fetoprotein (AFP) levels were significantly higher in HCC patients. Neuropilin-1, a novel marker, showed a significant increase in HCC patients compared to cirrhotic ones, with a strong correlation to liver function markers (AST, ALT, AFP, albumin) and Barcelona and Child scores. Neuropilin-1 demonstrated higher sensitivity (96.67%) and specificity (80%) for HCC detection compared to AFP, highlighting its potential as a superior biomarker for diagnosing HCC.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion: \u003c/strong\u003eSerum neuropilin 1 level was significantly higher in patients with HCC regardless presence of combined conditions as DM and HTN or viral markers and mildly elevated in patients with liver cirrhosis compared to HCC group.
