Abstract Background Skin cancer incidence is rising, and people of color often present with more advanced, aggressive disease due to delayed detection. Identifying effective educational tools in community screening settings may improve early detection by increasing awareness of risk factors and promoting self-skin examinations. This study compared the effectiveness of a video versus a written pamphlet in educating patients about skin cancer, risk factors, and self-skin examination techniques at a community health event. Methods 72 participants recruited from two free skin cancer screening events, held in Gainesville, Florida, in November 2023 and April 2024, were randomized to receive educational information via video or pamphlet. Participants completed demographic questionnaires and six pre- and post-survey questions. Educational materials and surveys were available in English and Spanish. Results 44.4% (n = 32) of participants identified as people of color. Surveys were completed in English by 87.5% and in Spanish by 12.5%. Video participants demonstrated significant improvements in knowledge and self-examination confidence across all six questions, with paired response analysis showing improvements in 5/6 questions. Pamphlet readers exhibited significant improvement in 4/6 questions in both overall and paired comparisons. Conclusions The video format demonstrated a slight advantage over the pamphlet in enhancing knowledge and self-examination confidence. Providing educational materials in multiple formats at free screening events may improve skin cancer awareness, particularly in minority populations.

Skin Cancer Screening Public Health Patient Education Early Detection of Cancer Florida Self-Examination Skin Neoplasms Introduction Skin cancer rates are steadily rising, especially among people of color. 1 The prevalence of skin cancer in these populations tends to be lower, but late-stage presentation with more aggressive histologic subtypes is more common, contributing to poorer outcomes [ 1 , 2 ]. Delayed detection has been attributed to a presumed lack of education on skin cancer presentation, low screening rates, and healthcare access barriers [ 3 – 5 ]. Several studies have trialed methods, including flyers, videos, and lectures, to increase awareness of skin cancer screening in minority populations and have shown their success in improving understanding and awareness in those populations [ 5 – 9 ]. Additionally, free skin cancer screening events that target minority and underserved populations provide a viable location for distributing such educational materials. There has been no study to date comparing the effectiveness of a flyer versus a video in both English and Spanish for delivering education on these topics. This study aimed to evaluate the effectiveness of a video versus a flyer in educating patients on skin cancer, risk factors, and self-skin examinations. Methods Participants were recruited and consented at CHOMP Melanoma, a free skin cancer screening event, in Gainesville, Florida, in November 2023 and April 2024. The Institutional Review Board at the University of Florida deemed this study IRB exempt. Seventy-two participants were randomized to either watch a video or read a flyer with materials adapted from the American Academy of Dermatology’s (AAD) infographic on performing self-skin examinations [10]. Participants completed a demographics and skin screening questionnaire (Tables 1 and 2) and a pre-survey (Tables 3 and 4) before receiving a free skin cancer screening. After receiving their skin examination, they answered post-survey questions (Tables 3 and 4). Pre- and post-survey questions were adapted from a validated questionnaire from Chung et al. and aimed to determine participants’ familiarity with melanoma, risk factors, and how to perform a self skin examination [5]. Educational materials and surveys were available in both English and Spanish. People of color included participants who self-identified as American Indian or Alaskan Native, Asian/Pacific Islander, Black or African American, Hispanic, or multiple ethnicity. Data analysis was performed using R (version 4.3.1; R Core Team, 2023) with a significance value set to p < 0.05. Pooled results were compared using Fisher’s exact test. Paired individual pre- and post-survey responses were compared using either the Wilcoxon signed-rank test or McNemar’s Chi-squared test, as indicated in Table 5. Of 72 participants, 44.4% (n = 32) self-identified as people of color, with 26.4% as Hispanic, 8.3% as Asian/Pacific Islander, 5.6% as multiple ethnicities, 2.8% as Black or African American, and 1.4% as American Indian or Alaskan Native. 31.9% (n = 23) identified as Hispanic/Latino. The survey was completed in English by 87.5% (n = 63) of participants and in Spanish by 12.5% (n = 9) of participants. Most commonly, respondents reported an average household income of $ 30,000 or less (n = 25, 34.7%). A majority reported completing 2–4 years of college (n = 37, 51.4%). A total of 20.8% (n = 15) of participants were uninsured and 30.6% (n = 22) had public insurance. Participants’ past skin examination and screening histories are reported in Table 2. The majority of participants do not perform self-screening skin examinations (n = 50, 69.4%). Out of the participants who do perform self-screening examinations (n = 22, 30.6%), they most commonly report performing these monthly (n = 9, 40.9%). The majority of participants, 83.3% (n = 60), had never been told by a healthcare provider to perform self-skin examinations, and 90.3% (n = 65) had not seen a healthcare provider for evaluation of a skin lesion in the past six months. Tables 3 and 4 summarize the pooled pre- and post-survey responses for participants who read the flyer and those who watched the video, respectively. Among flyer readers, four out of six questions showed statistically significant improvement, while video viewers demonstrated significant improvement across all six questions. Table 5 presents a statistical comparison of paired individual responses between the two groups. When analyzing individual-level data, significant improvements were observed in four questions for the flyer group and five questions for the video group. Discussion This study demonstrated that both written flyers and digital video formats improved patient knowledge and confidence regarding melanoma awareness and self skin examinations, with the video format moderately outperforming the flyer format. Pooled results were compared to investigate overall sample trends, and individual paired responses were compared to show individual-level changes pre- and post-survey after the written flyer or video educational intervention. In both pooled and individual-paired analyses, the flyer and video improved participant answers post-survey, with the video only slightly outperforming the flyer. These results are consistent with previous literature showing that flyers or videos can both improve patient awareness of skin cancer, with videos tending to outperform written materials [ 7 – 9 ]. However, the authors are not aware of a study that compares written versus video educational materials in both English and Spanish for skin cancer and screening awareness to date. This free skin cancer screening event in Gainesville, Florida, served people of color and disadvantaged populations. According to the US Census Bureau, in the year 2023, an estimated 50.7% of Floridians identified as people of color. 11 Our study closely matches these statistics, with 44.4% of participants identified as people of color. Most of the participants (34.7%) also reported a household income of less than $ 30,000, which qualifies as more than 200% below the poverty line, suggesting many of the participants had lower socioeconomic status [ 12 ]. However, the majority of participants (51.4%) reported their highest education level as 2–4 years of college, which is usually associated with higher socioeconomic status, but may be skewed by the event being associated with a large university, where some participants were students who were not yet employed. In addition, the majority of participants (51.4%) reported having no insurance or public insurance. Lastly, the majority of participants had not been told to perform self skin examinations (83.1%), taught how to perform them (81.9%), nor regularly perform skin examinations (69.4%), highlighting the importance of education on this topic in our community. Educational materials were successfully integrated into the workflow of this free skin cancer screening event, highlighting the ability to deliver such education at public health events. Specifically, educational materials were presented to patients while they waited to be screened by a dermatologist, and surveys were administered while patients waited to be roomed with the intake forms. This ensured the clinic flow and length of stay of patients were minimally impacted while maximizing patient education and use of time. A limitation of this study is the inability to determine the significance of additional physician education during the free screening appointment on influencing post-survey responses, as there was no control group who did not receive an educational intervention. Another limitation is the small overall sample size, particularly the limited number of participants who completed the materials in Spanish. The CHOMP Melanoma free skin cancer screening event successfully targeted minority and underserved populations, and this study demonstrated that both written and digital educational materials can be employed to educate patients on skin cancer and screening. Future directions include optimizing materials for equal effectiveness and investigating participant satisfaction with both education modalities. Conclusion In conclusion, both flyer and video formats showed enhancement of patient knowledge and confidence, with the video format slightly outperforming the flyer format in both pooled and individual pre- versus post-survey analyses, highlighting the importance of using public health outreach events as educational opportunities.
