Objectives The aim of this work was to determine the out-of-pocket expenditures and coping strategies employed by families of patients with CVD admitted to selected public and private hospitals in Yemen. Methods This cross- sectional descriptive study was carried out on 400 patients, admitted cardiovascular patients and selective outpatients came to treat in outpatient clinics at cardiovascular admission rooms and outpatient clinics in selected public and private hospitals. Data were collected through questionnaires covering socio-demographic details, medical history, lifestyle habits, cardiovascular disease characteristics, comorbidities, surgical history, and financial capacity for medical care. Results There was a significant relation between patients’ ability to afford medical expenses and type of hospital admitted to, age, place of residence, educational level, socio- economic factors except number of family members, diagnostic cardiac catheterization (P < 0.05). There was a significant relation between (open heart surgery and being late to take necessary health measures) and (duration of the disease, age, sex, and health habits) (P < 0.05). There was a significant relation between number of methods used to cover expenses and place of residence, job ad health insurance (P < 0.05). Conclusions Preventing CVD complications involved early detection, improved screening, and increased awareness of risk factors. Strengthened healthcare systems, public education, especially in rural areas and the implementation of comprehensive health insurance were key to improving outcomes and reducing the economic burden. Clinical Trial Number: Not Applicable Coping Strategies Financial Burdens Cardiovascular Diseases Public and Private Hospitals Sana`a City Introduction Cardiovascular Diseases (CVD) are a group of disorders of the heart and blood vessels which include coronary heart disease, cerebrovascular disease, rheumatic heart disease, congenital heart disease, peripheral arterial disease, deep vein thrombosis and pulmonary embolism. They are the leading cause of death globally. A report published by WHO revealed that these health conditions are responsible for approximately 17.9 million deaths worldwide every year, which translates to about one-third of global mortality [ 1 ]. In addition to the impacts of CVD on public and population health, Heart conditions impose physical, social, financial and health related quality of life limitations on individuals. These conditions result in an economic burden and impact on society due to expenditures on healthcare treatment, productivity losses from employment impacts, costs of providing formal and informal care, and lost wellbeing [ 2 ]. At patient and household levels, CVDs, like every chronic illness, have the potential to cause severe financial hardship due to the increase in the demand for medical services and a consequent rise in household health expenditures, especially in low- and middle-income countries (LMICs) [ 2 ]. The burden of chronic health expenditure born by patient households in these countries may be very high, particularly where out-of-pocket payments for health care are common. One such country where out-of-pocket payments are especially high is Yemen. To the best of our knowledge the financial impact of CVD on households in this country has not been researched. With a sizable economic impact in a country where there is no stable income nor a public health system strong enough to smoothen all hardships faced by patients and their households different coping strategies are identified in order to adapt health care payments. The aim of this work was to determine coping strategies for financial burdens in families with CVD in patient attending in selected public hospital, to study the relationship between the demographic factors and the coping strategies for financial burdens in families with cardiac diseases and to evaluate the impact of kind of cardiovascular disease and disease duration on the coping strategies for financial burdens in families with cardiac diseases. Patients and Methods This cross- sectional descriptive study was carried out on 400 patients, admitted cardiovascular patients and selective outpatients came to treat in outpatient clinics at cardiovascular admission rooms and outpatient clinics in selected public and private hospitals (Al-Thawra hospital, General Military hospital, University of Science and Technology hospital, Azal hospital and Lebanon hospital). The research was done from February to March 2023 approved by the Faculty of Medicine and Health Science ethical committee and was carried out following the guidelines and regulations of the University Research Ethical Committee/UREC at UST, Yemen (approval code: 0025/1444). An informed written consent was obtained from the patient or relatives of the patients. Exclusion criteria were patients visited outpatient clinics for the first time or patients haven't diagnosed yet. Data Collection Tools and Techniques This study done by used questionnaire which most of its variables were taken from previous studies [3]. This questionnaire consists of four sections: Section A: Socio-demographic data include short patient information as: age, gender, marital status, type of residence, education level, occupational status, type of housing, number of rooms in the house, size of family, number of employees in family, monthly income level. Section B: Information about health status it was statement to know type of cardiovascular disease , duration of it , risk factors as smoking and Khat chewing , Associated chronic disease as hypertension , diabetes mellitus and hyperlipidemia ,the number of hospitalization in the last year , number of family members who suffer from chronic diseases, type of procedure either utilization or unmet which consist of five questions which include : diagnostic cardiac catheterization ,therapeutic cardiac catheterization , open heart surgery ,valve replacement or repair , installation of pacemakers. Section C: Information about the financial burden statement if there was health insurance, if can able to afford the medical expense or late in taking any necessary health procedures and expressing the reason. Section D: Strategies to face the financial burden statement to know what was the method used to cover expenses that include 9 options, was it from their own pocket, rely on savings, borrow, mortgage of assets, sale of assets, family and friends support, benefactors support, governmental support / organizations support / exemption from fees, Zakat support. Data collection A self-administrated questionnaire was designed in Arabic to facilitate data collection, the questionnaire was distributing and data collected from participant randomly validation of questionnaire was achieved before starting the data collection through 35 questionnaires as pilot study. Pilot study: A pilot study was done on 35 patients and it was conducted to examine the structure, content, and clarity of the questionnaire which was select the sample randomly, they were excluded from study. Sample Size Calculation: Sample size was calculated by using OpenEpi, version 3.01 (http://www.openepi.com ) with a knowledge assumption of 50%. The confidence interval and absolute error of the margin were 95 and 5%, respectively. 400 of cardiovascular patients were recruited in this study. Four hundred different types of CVD who admitted in cardiovascular admission rooms or attended to outpatient clinics at selected public and private hospitals during the study application period (in period between 23 February to 11 march 2023). 210 patients were recruited from two public hospitals (157 patients from Al-Thawra hospital and 53 patients from General Military hospital) and 190 patients recruited from three private hospitals (80 patients from Lebanon hospital, 58 patients from University of Science and Technology hospital, and 52 patients from Azal hospital). Statistical analysis Statistical Package for Social Science (SPSS) version 25 program was used for data processing. Data was analyzed and expressed as mean, median and standard deviations. For descriptive statistics and p-value according to categorical variables: those with only 2 categories use Fisher’s Exact test, and those with more than 2 categories use chi-square test and values were considered significant when p values were equal to or less than 0.05. The majority of patients were advised to do diagnostic cardiac catheterization (53.0%), but only 49% were able to do it. The second most recommended procedure was therapeutic cardiac catheterization (30.8%), but only 26.8% were able to do it. There was no significant relation between (patients’ ability to afford medical expenses and sex, marital status, therapeutic cardiac catheterization, open-heart surgery, valve replacement or repair and installation of pacemakers) and between (being late to take necessary health measures and diagnostic cardiac catheterization, therapeutic cardiac catheterization, valve replacement or repair and installation of pacemakers). There was a significant relation between open heart surgery and being late to take necessary health measures (P = 0.047). Discussion Cardiovascular disease is one of the most important topic now days as a part of chronic diseases. It gains its importance through the different theories and studies that have been established around its effect on patient life even by its cost and economic impact on patient daily life or its effect on the general budged. About risk factors smoking is consider one of the most dangerous risk factors in CVD, in our research only one third of targeted people were smokers, while two third is not. This result was similar to the National Health and Nutrition Examination Survey which showed that 61% of the total sample never smoke [ 4 ]. In the opposite to our study positive relation between family history of cardiovascular system and the chance of developing those diseases which reach to two doubles in Kerala 2021 [ 5 ]. While in our study this relation was negative whereas less than half of sample size has a family history with CVD. The exact roll of genetic and family history with most of cardiovascular disease was not clearly understood, but mostly all researches done in this topic give strong positive relation between family history of cardiovascular disease and the chance of developing the same disease in number of a new generation [ 6 , 7 ]. About procedure in our research more than half of sample size were passing through some important procedure ether diagnostic or therapeutic such as diagnostic or therapeutic catheterization, open heart surgery, valve replacement or repair and lastly installation of pacemakers, cardiac catheterization was the most common one and that was similar to study done in UK [ 8 ]. The reason of chosen this maneuver that its saver and more adequate than other methods, whereas the side effect of health and budget of the other methods was worst. In our results unemployed were the majority of patient, followed by self-employed and lastly retired. Such a distribution explained the reason of chosen this topic. And similar to this result a study done in Sudan in 2019, give a similar results [ 9 ]. Type of housing, number of rooms in the house and income level, all of these factors were included in our research, even though none of them give a sure indicator about those family income or financial stats. In our results most of cardiovascular disease patients have their own house which were about to third of the sample size. With number of rooms rich to more than 4 in most of them. About income the answer of more than half of participants were barely enough, followed by third with the answer not enough and only quarter of then believes that they have enough income. And this results were against a result of study done by Adrianna et al. [ 10 ] in Ukraine world health survey. Rely on saving, receiving help from family and friends were also helpful ways, the last method used between those families was charitable support from zakat. This result was similar to study done by Micheal et al. [ 11 ] in Vietnamese population and another study done by Adrianna et al.[ 10 ] in Global Health organization and Scopus on 22 October 2018, which also shows a coping methods using for fitting against chronic disease as cardiovascular disease, which were similar to those we discuss in our research. Regarding to cardiovascular patient ability to afford the medical expenses two third of patient answer by no while only one third are able. Being late to take any necessary health measures as also related issue, whereas also two third of the total sample size were an able to pay for them [ 12 ]. In our results only 7% of the targeted sample have medical insurance, while all the others did not have .This result was in contrast with many other studies where Important work has been done to show how measures to expand insurance coverage in LMICs, especially to the poorest families and those with children and older adults, can achieve the greatest improvements in financial protection [ 13 , 14 ]. Limitations of the study included that lack of prior studies on the topic, especially in Yemen and Arab countries. Difficulty obtaining hospital permission to distribute questionnaires. Patient reluctance or fear to complete questionnaires. Cross-sectional design may cause selection bias and limited representation. Inability to track families over time, requiring potentially inaccurate assumptions about financial burdens. Challenges in capturing changes in financial burdens over time. Difficulty identifying multifactorial factors affecting coping strategies with financial burdens. Inability to assess the impact of social, economic, and cultural factors on coping strategies. Conclusions Families with CVD face a heavy financial burden, with many resorting to borrowing to cover the associated costs. Those visiting government-funded hospitals vs. private hospitals have differing financing methods, highlighting disparities in access to healthcare and financial support. Addressing the high costs and financial barriers associated with chronic illnesses was critical, and policymakers should take steps to increase access to affordable treatments, implement effective insurance policies, and provide financial assistance to those in need. Government and organizational bodies should adopt a comprehensive approach to addressing the financial strain associated with medical costs so that financial obstacles do not hinder access to care. Data were collected through questionnaires covering socio-demographic details, medical history, lifestyle habits, cardiovascular disease characteristics, comorbidities, surgical history, and financial capacity for medical care.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eThere was a significant relation between patients’ ability to afford medical expenses and type of hospital admitted to, age, place of residence, educational level, socio- economic factors except number of family members, diagnostic cardiac catheterization (P \u0026lt; 0.05). There was a significant relation between (open heart surgery and being late to take necessary health measures) and (duration of the disease, age, sex, and health habits) (P \u0026lt; 0.05). There was a significant relation between number of methods used to cover expenses and place of residence, job ad health insurance (P \u0026lt; 0.05).\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusions\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003ePreventing CVD complications involved early detection, improved screening, and increased awareness of risk factors.
