Abstract
The strategy for integrating the latest technologies into minor gynecological surgeries is aimed at optimizing the work of shortstay units (SSU) and improving the efficacy of diagnosis and treatment of complex gynecological pathologies. Objective. To improve the quality of diagnosis and treatment of patients in SSU by introducing the “integrated digital minor operating room” (IDMOR) into practice. Patients and methods. The analysis included the treatment outcomes of 612 patients aged 19 to 46 years hospitalized at SSU from January to March 2025 for elective intrauterine surgery using the IDMOR principle – a combination of intraoperative 2D/3D ultrasound with hysteroscopy. A new approach was developed for the treatment of Müllerian duct anomalies (26; 4.2%), intrauterine synechiae (38; 6.2%), uterine myoma (125; 20.4%), and various endometrial pathologies (348; 57%). Results. The original system, developed for performing minor gynecological surgeries at SSU by combining the advanced diagnostic capabilities of 3D ultrasound and hysteroscopy, made it possible not only to diagnose but also to simultaneously correct such rare and complex pathologies as uterine malformations (T-shaped uterus, intrauterine septum), intrauterine synechiae, including Asherman’s syndrome. The safety of hysteroscopic myomectomy was improved, and it became possible to diagnose such a rare malformation as a unicornuate uterus with its detailed classification. In patients with infertility, tubal patency was assessed simultaneously with hysteroscopy, or the cause of occlusion was determined. Difficult-to-access peritoneal or deep infiltrating endometriosis types were diagnosed against the postoperative hydroperitoneum. Conclusion. The development of a new principle of surgery in minor operating room in accordance with current requirements and trends in the provision of highly qualified care allows for the simultaneous diagnosis and treatment of simple or complex nosologies without loss of time or reduction in the quality of treatment.
