This study sought to analyze the impact of maternal anemia on anemia in children in this age group in Tanzania. Methods: Data was extracted from the Tanzania TDHS-MIS 2022, conducted from March to July. A total of 2,500 children aged between 6 and 59 months were included in the final analysis. The analysis considered both univariate and multivariate odds ratios (COR and AOR, respectively), with a 95% CI, as determinants of anemia. Data was analyzed using R software. Results: The overall prevalence of anemia in children aged 6 to 59 months was 70.16% (95% CI = 68.32; 71.95), with 40.8% of cases classified as mild, 56.4% moderate, and 2.8% as severe. This prevalence was higher in children < 24 months (75.1%). In the model adjusted for other factors, maternal anemia emerged as a potential indicator for anemia in this age group, with an AOR of 1.48 (95% CI: 1.18; 1.87) for mild anemia, 1.71 (95% CI: 1.32; 2.21) for moderate, and 2.14 (95% CI: 1.08; 4.26) for severe anemia. Furthermore, children under 24 months, males, with a recent history of diarrhea, in families with more than two children, and with mothers aged between 20 and 29 years, were more likely to develop anemia. On the other hand, children who received drugs for intestinal parasitosis in the last six months were less likely to have anemia. Conclusion: This study highlights the high prevalence of child anemia in Tanzania, especially in children under 24 months. Maternal anemia emergedas a significant potential predictor for child anemia. Targeted interventions that address both maternal and child health are crucial for reducing the prevalence of anemia and improving child health outcomes. This condition represents a global public health challenge, affecting 1.62 billion (24.8%) people worldwide [ 2 ]. It is most prevalent among preschool-aged children under five years [ 3 , 4 ], impacting not only human health but also social and economic progress [ 5 ]. Globally, 293.1 million (47.4%) children under five are anemic [ 6 ], and 67.6% of these children live in Africa [ 2 ]. In Tanzania, about 65.4% of children aged between 6 and 59 months were anemic according to the Tanzania Demographic and Health Surveys (TDHS) 2015 report [ 7 ]. Childhood anemia significantly impacts mental, physical, and social development in children, both short and long term [ 8 ]. It can be associated with abnormalities in immune function, inadequate motor and cognitive development, poor school performance, and reduced productivity throughout life, resulting in decreased earnings prospects and negatively impacting national economic growth [ 8 , 9 ]. Furthermore, it is associated with increased morbidity and mortality, especially in Africa, where resources to determine the underlying etiology remain scarce [ 10 ]. Various studies have been conducted in the sub-Saharan African region to identify risk factors associated with anemia in children aged between 6 and 59 months, yet the effect of maternal anemia in this child age group has received little attention [ 10 , 11 ]. This study aims to determine the prevalence of anemia in this age group, analyze the impact of maternal anemia on this outcome, and explore other possible predictive factors. Methods Study Design and Data Collection A Demographic and Health Survey and Malaria Indicators Survey (TDHS-MIS) was conducted in Tanzania in 2022, from March to July. The goal was to provide comprehensive estimates for the entire country, including urban, rural, and Zanzibar areas, excluding institutional populations such as hospital patients, hotel guests, military personnel in barracks, camps, hostels, and prisons. Sampling Technique and Sample Size Participants were selected through a two-stage approach: clusters were chosen based on the enumeration areas of the 2012 Census, and then 26 households were selected from each cluster. Before the survey, a household listing was conducted in all selected areas [ 12 ]. A total of 2500 children aged between 6 and 59 months, whose anemia levels were determined, were included, as shown in Fig. 1. Outcome Variable To diagnose anemia in children aged 6 to 59 months, blood samples were collected with consent from guardians and the portable HemoCue device was used to measure hemoglobin. According to WHO criteria, hemoglobin below 11.0 g/dl indicates anemia, classified as severe below 7.0 g/dl, moderate between 7.0 g/dl and 9.9 g/dl, and mild between 10.0 g/dl and 10.9 g/dl [ 13 ]. A The dependent variable was coded dichotomously, assigning 1 for anemia diagnosis and 0 for absence. Predictor Variables Potential explanatory variables were selected based on previous studies [ 14 – 16 ]. Variables were divided into individual and household factors. Child-related variables included sex, age in months (categorized into 6–23 months and 24 months or more), birth weight (less than 2.5 kg considered low birth weight), wasting (weight for height less than − 2SD from the reference population median) [ 17 ], m thinness (weight for age less than − 2SD from the reference population median) and nutritional status (malnourished if standard deviation is less than − 2, normal if between − 2 SD and 2 SD, and overweight if greater than 2 SD) [ 17 ]. Diagnosis of anemia, presence of high fever (possible infection), recent history of diarrhea, and treatment for parasitic infections in the last six months. Variables related to the child’s mother and household included anemia (anemic or not), educational level (no education, primary, secondary, and higher education), economic activity (working or not), current breastfeeding (yes or no), interval between previous births (< 24, 24–35, 36–47, and more than 47 months), birth order (< 5 and 5 or more), BMI (underweight, overweight, obesity, and normal weight), anemia (no anemia, mild, moderate, severe), household size (less than 5 and 5 or more individuals), number of children in the household (less than 3 and 3 or more), gender of the household head, water source (improved or not), and type of sanitation (improved or not). Data Analysis Descriptive analysis was used for qualitative data, expressing relative and absolute frequencies. The chi-square test detected significant differences between observed and expected frequencies. Correlation, using Cramér's V coefficient, assessed the multicollinearity of explanatory variables in logistic regression models. The robustness of the models was confirmed by the generalized variance inflation factor (GVIF). To assess the impact of each characteristic on anemia in children aged 6 to 59 months, logistic regression models were employed to estimate Odds Ratios (ORs) and 95% confidence intervals (CI). Initially, a univariate analysis was conducted to examine the association between independent variables and child anemia. Subsequently, all variables were included in a multivariate model for a more comprehensive analysis. The final multivariate model was obtained using the backward variable selection method, based on the p-value. Results with p < 0.05 were considered statistically significant. The data analysis was performed using R software version 4.3.2 ( https://www.r-project.org/ ) and RStudio version 2023.12.0 + 369 ( https://posit.co/download/rstudio-desktop/ ). Results According to Fig. 1, the study included 2500 children aged between 6 and 59 months. Most of the children were male (60.6%) and aged between 6 and 23 months (62.7%). Most mothers were between 20 and 39 years old (51.2%), had primary education (52.9%), and approximately 56.6% breastfed during the research. Regarding anemia, it was observed that mild anemia was more common (20.7%), followed by moderate (17.5%) and severe (2.2%) among mothers. Most households had fewer than three children in this age range, according to Table 1 . The overall prevalence of anemia among children aged 6 to 59 months was 70.16% (n = 1754; 95% CI = 68.32; 71.95), with 40.8% having mild anemia, 56.4% moderate, and 2.8% severe cases. According to Table 1 , the prevalence of anemia was higher in the age group of 6 to 23 months (75.1%; p < 0.001), in boys (72.5%; p = 0.009), in children with a recent history of diarrhea (77.6%; p = 0.003), and who have not received medication for intestinal parasitosis in the last six months (73.3%, p < 0.001). A higher rate was also observed among children whose mothers were aged between 15 and 19 years (75.7%) and those aged between 20 and 29 years (71.8%; p = 0.005). Conversely, this rate was significantly higher in children whose mothers were breastfeeding (73.9%; p < 0.001), in households with more than 2 children (74.3%; p = 0.005), and in mothers with low BMI (77.8%; p = 0.008). Moreover, a higher rate of anemic children was observed when mothers also had anemia (75.9%; p < 0.001). As presented in Table 2 , the univariate model identified the following risk factors associated with anemia in children aged 6 to 59 months: male sex, age between 6 and 23 months, low BMI for age, recent history of diarrhea, mothers aged between 15 and 19 years or between 20 and 29 years, breastfeeding phase, low maternal BMI, households with three or more children, and presence of mild, moderate, or severe anemia in mothers. Conversely, deworming in the last six months, recent maternal employment, and maternal overweight were inversely associated with anemia in children aged 6 to 59 months. After adjusting the model for potential confounders, the final multivariate model revealed that maternal anemia was a strong indicator of anemia in children aged 6 to 59 months, showing a significant increase in the odds of mild (AOR = 1.48; 95% CI: 1.18; 1.87), moderate (AOR = 1.71; 95% CI: 1.32; 2.21), and severe anemia (AOR = 2.14; 95% CI: 1.08; 4.26). Furthermore, it was observed that children under 24 months (AOR = 1.72; 95% CI: 1.43; 2.07), boys (AOR = 1.27; 95% CI: 1.06; 1.51), children with a recent history of diarrhea (AOR = 1.43; 95% CI: 1.06; 1.92), children whose mothers were aged between 20 and 29 years (AOR = 1.48; 95% CI: 1.03; 2.12), and children living in households with three or more children (AOR = 1.31; 95% CI: 1.02; 1.67) had a higher chance of developing anemia. On the other hand, children who received drugs for intestinal parasitosis in the last six months were less likely to have anemia, with an AOR of 0.82 (95% CI: 0.69–0.99), as shown in Table 2 , model 3. In some regions of Tanzania, the prevalence of anemia in children aged 6 to 59 months was alarmingly high, reaching up to 84.6% [ 20 ]. However, in other regions, the rates were lower, such as 37.9% in the northeast district of Rondo [ 21 ], and in Zanzibar, with rates of 76.1%, 70.2%, and 65.4% [ 7 ]. Compared to other African countries, such as Nigeria (68.1%) [ 14 ], Togo (70.9%) [ 8 ], and Mozambique (80.3%) [ 22 ], Tanzania showed converging rates, although lower than those of Ghana [ 16 , 23 ], Ethiopia [ 15 , 24 ], Zimbabwe [ 25 ], and Namibia [ 26 ]. Among these affected children, 40.8% had mild anemia, 56.4% moderate anemia, and 2.8% severe anemia. These numbers reflect similar trends in Ghana [ 16 ] and Togo [ 8 ], while lower rates were found in Ghana [ 27 ] and in the South and Southeast Asian regions [ 28 ]. In Mozambique, the severe anemia in this age group was about twice as high (7.0%) [ 22 ]. While the WHO considers a prevalence of anemia above 40% as a serious public health problem [ 1 , 2 ], most African countries, including the context of this study, face rates well above this threshold. It is crucial to improve existing policies and implement specific interventions, such as geographic, behavioral, community, and individual strategies, to drastically reduce these rates [ 23 ]. In this study, several factors were identified as predictors of anemia in children aged 6 to 59 months, after adjustments for potential confounding variables. An important contribution of this study in relation to previous research [ 10 , 18 , 29 ] is the evidence that maternal anemia and communities with a high prevalence of anemic women have adverse effects on the occurrence of anemia in children in this age group. It was observed that children whose mothers had mild, moderate, and severe anemia showed, respectively, increases of (AOR = 1.48; 95% CI: 1.18; 1.87), (AOR = 1.71; 95% CI: 1.32; 2.21), and (AOR = 2.14; 95% CI: 1.08; 4.26) in the odds of developing anemia. A compelling explanation is that anemic mothers often reside in unfavorable socioeconomic environments, where they face challenges in securing adequate nutrition for both them and their children. This lack of vital nutrients, such as iron, can trigger anemia [ 30 ]. Additionally, in the first months after birth, mothers and children share the same social environment, which directly influences their dietary patterns and lifestyle [ 10 ]. Furthermore, low levels of essential minerals, such as iron, zinc, and folate, along with vitamins A and B12 in the breast milk of anemic mothers, can significantly impact the hemoglobin levels in breastfeeding children. Additionally, children aged between 6 and 23 months (AOR = 1.72; 95% CI: 1.43; 2.07) had a higher chance of developing anemia compared to children over 23 months of age. This finding is in line with several previous studies [ 21 , 23 , 24 , 26 ]. After six months of age, children begin to introduce complementary foods into their diet. However, up to two years of age, there may be difficulty in the intake of these foods, especially due to the lack of socioeconomic conditions to acquire them, which can result in nutrient deficiency and increase the likelihood of anemia [ 31 ]. Moreover, infections caused by contaminated food and water can result in gastrointestinal symptoms, such as diarrhea, vomiting, and mouth ulcers, impairing the ability to ingest and absorb iron and other essential nutrients, which can contribute to anemia. Male children in this study had a higher probability of developing anemia compared to female children (AOR = 1.27; 95% CI: 1.06; 1.51). This trend is consistent with several other findings in previous studies [ 10 , 23 , 26 ]. This might be explained by the fact that male children tend to have a higher growth rate, which results in an increased demand for iron in the body [ 32 ]. However, this increased need for iron may not be fully met by the diet, leading to a higher prevalence of anemia in boys [ 33 ]. Consistent with previous studies [ 31 , 34 , 35 ], a recent history of diarrhea (AOR = 1.43; 95% CI: 1.06; 1.92) emerged as a predictor of anemia in this age group. This association can be explained by the fact that diarrhea leads to a decrease in body fluids, resulting in the loss of important nutrients, such as iron, due to poor absorption in the gastrointestinal tract. In this study, it was observed that children born to mothers aged between 20 and 29 years (AOR = 1.48; 95% CI: 1.03; 2.12) and those belonging to families with three or more children per household (AOR = 1.31; 95% CI: 1.02; 1.67) had a higher risk of anemia. This finding is in line with previous studies [ 31 , 36 ]. This may be explained by the fact that younger mothers may have less experience in care and proper feeding practices, while families with many children may face additional challenges in providing a balanced and nutritionally adequate diet for each member [ 31 ]. Younger mothers may also be in a phase of anatomical and physiological formation, which can result in a lower reserve of nutrients. These mothers face the challenge of sharing these nutrients with their children. Treatment for parasitic infections in children in the last 6 months was shown to be a significant protective factor (AOR = 0.82; 95% CI: 0.69; 0.99). Similar associations were found in some previous studies [ 37 , 38 ]. These findings highlight the importance of addressing the presence of intestinal parasites in child health. Intestinal parasites can impair iron levels by feeding on the blood from the intestinal wall, causing bleeding and reducing the absorption of essential nutrients [ 39 ]. Furthermore, they cause gastrointestinal disorders, such as diarrhea or dysentery. This can lead to anemia in children due to loss of appetite, competition for micronutrients, and interference in the absorption of vitamin A, which is essential for hemoglobin synthesis. The limitations of this study include the cross-sectional nature of the data, which precludes establishing causal relationships between exposures and child anemia. Additionally, self-reported DHS data may introduce recall bias. Despite these limitations, the results still offer valuable insights for the planning and monitoring of child anemia at the national level, given the representativeness of the study data, which used nationally representative survey data. These results are consistent with previous studies that investigated the prevalence and risk factors of anemia in children aged 6 to 59 months. Conclusion The high prevalence of anemia in children aged 6 to 59 months, especially those under 24 months and males, is alarming, underscoring its importance as a public health concern in Tanzania. Maternal anemia emerged as a potent predictor of child anemia. Additionally, recent history of diarrhea, along with the lack of treatment for intestinal parasitosis in children, emerged as significant predictors of child anemia. These results emphasize the urgent need for targeted interventions to reduce anemia, prioritizing improvements in nutrition and health for both mothers and children, as well as improvements in the implementation of preventive strategies against infections and parasitosis. This study sought to analyze the impact of maternal anemia on anemia in children in this age group in Tanzania.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods: \u003c/strong\u003eData was extracted from the Tanzania TDHS-MIS 2022, conducted from March to July. A total of 2,500 children aged between 6 and 59 months were included in the final analysis. The analysis considered both univariate and multivariate odds ratios (COR and AOR, respectively), with a 95% CI, as determinants of anemia. Data was analyzed using R software.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003eThe overall prevalence of anemia in children aged 6 to 59 months was 70.16% (95% CI = 68.32; 71.95), with 40.8% of cases classified as mild, 56.4% moderate, and 2.8% as severe. This prevalence was higher in children \u0026lt; 24 months (75.1%). In the model adjusted for other factors, maternal anemia emerged as a potential indicator for anemia in this age group, with an AOR of 1.48 (95% CI: 1.18; 1.87) for mild anemia, 1.71 (95% CI: 1.32; 2.21) for moderate, and 2.14 (95% CI: 1.08; 4.26) for severe anemia. Furthermore, children under 24 months, males, with a recent history of diarrhea, in families with more than two children, and with mothers aged between 20 and 29 years, were more likely to develop anemia. On the other hand, children who received drugs for intestinal parasitosis in the last six months were less likely to have anemia.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion:\u003c/strong\u003eThis study highlights the high prevalence of child anemia in Tanzania, especially in children under 24 months.
