It has been considered primarily a postmenopausal condition, due to the known effects of low estrogen on bone mass loss in females. However, recognition of the burden of osteoporosis in the male population has grown in the past decades, associated with an increase in fragility fractures in the population, likely due to the worldwide longevity, a more sedentary lifestyle, and an increased prevalence of risk factors. The aim of the study was to evaluate osteoporosis in males and its causative factors in Damascus Hospital. Methods : A cross-sectional study was conducted in Damascus Hospital (Al-Mujtahid), during the period from 20/6/2024 to 15/8/2024. Patient files were studied in the Department of Endocrinology, between 2013 and 2024, and the number of patients studied was 73 patients diagnosed with osteoporosis. Results : Our study included 73 patients diagnosed with osteoporosis, aged 60-97 with a mean age of 77 years. The majority of patients were rural residents (61.6%). Chronic diseases suffered by patients were: hypertension (23.3%), followed by diabetes (15.1%). More than half of the patients were smokers (74%). A small percentage (8.2%) consumed alcohol. The majority of patients (67.1%) were not physically active. The common primary causes were: family history (35.6%), vitamin D deficiency (32.9%), lack of physical activity (15.1%), advanced age (9.6%). Secondary causes were: medications (42.5%), then underlying medical conditions (9.6%). Conclusion: Osteoporosis in men is caused by a complex interaction between hormone deficiency, medication use, lifestyle choices, and genetic factors. Understanding these causes is critical to developing effective prevention and treatment strategies. Our study found that the most important causes of osteoporosis were long-term steroid use and a family history of osteoporosis. For males, the same thresholds as for females are recommended, although the definition of density is not as well standardized as in postmenopausal women. [2] Although the prevalence of osteoporosis is higher in females, males are at higher risk of death after fracture. This applies to both vertebral and nonvertebral fractures.[3][4] Despite the higher mortality and morbidity in males, most randomized controlled trials of osteoporosis treatment include only postmenopausal females, creating a challenge in the management of osteoporosis in males. Despite hormonal and biomechanical advantages over females, approximately one-third of men will experience one or more osteoporotic fractures in their lifetime. Osteoporosis in men can be classified into two categories, primary and secondary. Secondary causes account for approximately 50% of cases in males.[5] However, the primary cause may be underestimated due to less active diagnostic searches in patients without risk factors. Statistical analysis: Patient data was tabulated and entered into the computer into the Excel program, and then the Statistical Package for the Social Sciences (SPSS) version (26) was used to analyze this data, and the following statistical methods were relied upon in the analysis: Descriptive statistics: It consists of finding the relative frequency distributions of the categorical study variables. Results Our study included 73 patients diagnosed with osteoporosis, aged 60–97 with a mean age of 77 years (Table 1 ). (Table 4 ) shows that more than half of the patients take medications at a rate of 38 (52.1%), compared to 35 (47.9%) who do not. These medications were as follows: alpha blockers 18 (24.7%), metformin 12 (16.4%) (Table 5 ). A small percentage of 6 (8.2%) drink alcohol, compared to 76 (91.8%) who do not drink alcohol (Table 7 ). Yes 24 32.9 no 49 67.1 Total 73 100 Family history By studying the family history of the patients, we found that 44 (60.3%) of them had a family member with a history of osteoporosis (Table 9 ). (Table 10 ) shows that a family member had suffered fractures or loss of height, and the lowest percentage had suffered fractures at 33 (45.2%). Yes 33 45.2 no 40 54.8 Total 73 100 Medication use : We found that most of the patients were taking medications for the treatment of osteoporosis, with a percentage of 59 (80.8%) (Table 11 ). The medications were as follows: Forteo (Teriparatide) at a rate of 35 (47.9%), and Tymlos (Abaloparatide) at a rate of 24 (32.9%) (Table 12 ). (Low sex hormones; estrogen and testosterone). 3 4.1 nothing 32 43.8 Total 73 100 Discussion Osteoporosis in men is a multifactorial condition, influenced by a combination of genetic, hormonal and lifestyle factors. Scientific studies have identified several major causes that contribute to the development of osteoporosis in men. One major cause is hypogonadism, which is characterized by low levels of testosterone. Testosterone plays a crucial role in maintaining bone density, and its deficiency can lead to significant bone loss [ 6 , 7 ]. Age-related decline in testosterone levels is a common phenomenon in men, which partly explains the increased incidence of osteoporosis in older males [ 8 ]. Another important cause is the use of glucocorticoids, either due to chronic conditions that require long-term steroid treatment or endogenous Cushing's syndrome [ 6 ]. Glucocorticoids interfere with bone remodeling by inhibiting osteoclast function and enhancing osteoclast activity, leading to decreased bone formation and increased bone resorption [ 7 ]. This mechanism is well documented in clinical studies and is a major risk factor for secondary osteoporosis in men [ 8 ]. In our study, steroid use was the most important secondary cause of osteoporosis. Lifestyle factors also play a crucial role in the development of osteoporosis. Excessive alcohol consumption and smoking have been consistently associated with decreased bone density [ 6 ]. Alcohol interferes with calcium balance in the body and affects the production of hormones necessary for bone health. On the other hand, smoking has been shown to reduce bone mass and increase the risk of fractures [ 7 ]. The percentage of our patients who smoked was (74%). Physical inactivity also exacerbates the risk, as regular weight-bearing exercise is essential to maintain bone strength. [ 8 ] Genetic predisposition is another important factor. Studies have shown that men with a family history of osteoporosis are more likely to develop this condition [ 6 ]. The percentage of patients with a family history of osteoporosis was (60%). Genetic factors affect bone density and the rate of bone loss, making some individuals more susceptible to osteoporosis [ 7 ]. In addition, some chronic diseases such as diabetes and rheumatoid arthritis are associated with an increased risk of osteoporosis in men [ 8 ]. The percentage of diabetic patients in our study was approximately (20%). Conclusion Osteoporosis in men is caused by a complex interaction between hormone deficiencies, medication use, lifestyle choices, and genetic factors. Understanding these causes is crucial to developing effective prevention and treatment strategies. Our study found that the most important causes of osteoporosis were long-term steroid use and a family history of osteoporosis. Patient files were studied in the Department of Endocrinology, between 2013 and 2024, and the number of patients studied was 73 patients diagnosed with osteoporosis.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e: Our study included 73 patients diagnosed with osteoporosis, aged 60-97 with a mean age of 77 years. The majority of patients were rural residents (61.6%). Chronic diseases suffered by patients were: hypertension (23.3%), followed by diabetes (15.1%). More than half of the patients were smokers (74%). A small percentage (8.2%) consumed alcohol. The majority of patients (67.1%) were not physically active. The common primary causes were: family history (35.6%), vitamin D deficiency (32.9%), lack of physical activity (15.1%), advanced age (9.6%).
