Abstract
Ultrasound measurement of endometrial thickness correlates well with real endometrial thickness. It turns ultrasound the gold standard method to screen endometrium pathology. Patients with pelvic organ prolapse would have endometrial thickness assessment difficult due to externalization of the uterus, especially when its relocation is not possible. A 65 years-old woman presented with a request for transvaginal ultrasound for assessment of endometrial thickness as she would undergo vaginal hysterectomy as treatment of total genital prolapse. Due to significant genital prolapse swelling, it was not possible uterus relocation to do transvaginal ultrasound exam. The exam was performed with a linear probe of 7.5 MHz, that has been coated with latex glove and then with aqueous gel. The probe was passed directly over the herniated surface generating images suitable for endometrial assessment. The endometrial thickness (A) was 1.1 mm consistent with atrophic endometrium. The technique used for the measurement of the endometrium was through the sagittal image of the uterus. The patient had vaginal hysterectomy the day after the exam. The pathology report of the surgical specimen consisting of uterus showed atrophic endometrium measuring 1 mm thickeness with endometrial cavity not evaluable. A technique with linear probe, that was not yet described in the literature, was performed. There was agreement between ultrasound and pathology measurement that turns the linear probe approach a good option to endometrium evaluation in cases of female pelvic organ prolapse.
