CHANGE IN THE LEVEL OF HORMONAL ACTIVITY OF THE ENDOMETRIUM DEPENDING ON THE HORMONAL BACKGROUND OF PATIENTS WITH ENDOMETRIOS

Abstract

The article presents the results of the study of the operative material of the endometrium of 25 patients with endometriosis and the level of progesterone in their blood. The state of progesterone receptors and the ratio of their isoforms depending on the level of hormones in patients with endometriosis was studied. In 6 % of patients who had severe clinical manifestations of endometriosis, there was a long-term violation of the menstrual cycle, a pronounced pain syndrome accompanied by bleeding, pain in the lower back, pain during urination. In the main group of studied patients (83 %), minor clinical manifestations of endometriosis were determined, namely: minor pain syndrome during the menstrual cycle, absence of pain during urination, pregnancy attempts in 27 % were positive, although 58 % of pregnancies ended in spontaneous termination. In 6 % of patients, it was determined that a moderate reaction was detected in the cells of the epithelium and stroma in a quarter of the field of vision (++), there was infertility in the anamnesis and minor disturbances of the menstrual cycle. There were no violations on the part of the neighboring organs (bladder and rectum). During the study of biomaterial for the presence of progesterone receptors, it was found that in 11 % of patients with a severe course of endometriosis, the level of progesterone receptors in the endometrium was very low according to the clinical course - a weak reaction (+) was determined, while in patients with a mild course, a pronounced expression of progesterone receptors was determined - pronounced reaction in epithelial and stroma cells-+++. When examining the level of progesterone in the blood of this group of patients, a significant decrease in the level of progesterone was determined (0.49±0.18ng/ml), which is 9.5 times lower than the normal values of healthy women. During the research, it was found that in 83 % of the observations, a pronounced reaction was detected in the cells of the epithelium and stroma (+++). The ratio of PR-A to PR-B isoforms was 2 to 1, that is, an increase in PR-A isoforms and a decrease in the number of PR-B isoforms was still observed. The level of progesterone in the blood of patients was on average 0.65±0.15 ng/ml, which is 7.2 times lower than the normal values of healthy women.
