Abstract
OBJECTIVE:
\nTo evaluate the role of non
\n-
\ndescent vaginal hysterectomy in
\nadvancing gynaecological
\npractice and to study the safety and feasibility of performing vaginal
\nhysterectomy for non
\n-
\nprolapsed uterus as primary route in benign gynaecological condition
\n.
\nMETHODS:
\nA prospective study of 120 cases was conducted at the department of obstetrics and
\ng
\nynaecology of PES institute of medical sciences and research from January 2012 to December
\n2014 120 patients planned for hysterectomy for a wide range of benign indications like fibroid
\nuterus, AUB, adenomyosis were chosen for non
\n-
\ndescent vaginal hysterect
\nomy. Data regarding
\nage, parity, uterine size, and estimated blood loss, length of operation, intra
\n-
\noperative and post
\n-
\noperative complications and hospital stay were recorded.
\nRESULTS:
\nA total of 120 cases were
\nselected for non
\n-
\ndescent vaginal hysterectomy
\n. Among them 113(94.16%) cases successfully
\nunderwent non
\n-
\ndescent vaginal hysterectomy. Majority were aged 40
\n-
\n45 years (53.33%) with 8
\nnullipara and 21primipara. Commonest indication was fibroid (58%) and largest uterine size was
\n16 weeks. Different morc
\nellation techniques were used in more than 10weeks sized uterus.
\nAdnexal surgeries were
\nperformed in 11 cases without much difficulty. In uncomplicated cases
\naverage blood loss was 200 ml and operating time was 60minutes. Most of the patients were
\ndischarg
\ned by 4
\nth
\npost
\n-
\noperative day, 7 patients were converted to abdominal route due to
\nvarious difficulties, 3 patients had bladder injury and 1 patient was subjected to laparotomy due
\nto hemoperitoneum post operatively.
\nCONCLUSION:
\nProper training and proper c
\nase selection
\ncan lead a gynecologist to consider the vaginal approach as the standard route for hysterectomy
\nand good patient compliance
