MÉTODO: Realizamos um estudo observacional, transversal, prospectivo de 27 pacientes do sexo feminino, com suspeita clínica de endometriose. Todas as pacientes foram submetidas à realização de exames de ressonância de alto e baixo campo em serviço privado, no mesmo dia, seguindo protocolo pré-estabelecido nos dois aparelhos. Os exames foram avaliados quanto à presença ou ausência de focos de endometriose nos ovários, endometriomas e focos peritoneais, alterações tubárias, adenomiose, alteração fibrocicatricial de fundo de saco posterior, alteração dos ligamentos útero-sacros, alteração de septo reto-vaginal, reto-sigmóide e bexiga. As leituras foram realizadas por um médico radiologista, iniciando pelos exames do baixo campo, seguidos dos exames do alto campo, sendo por fim os dois exames confrontados para avaliação da conspicuidade do baixo campo. As imagens foram comparadas usando como padrão ouro a ressonância de alto campo. A análise estatística baseou-se nos cálculos de prevalência, acurácia, sensibilidade, especificidade, valor preditivo negativo e valor preditivo positivo. RESULTADOS: Das 27 pacientes com suspeita clínica de endometriose, identificamos 18 (66,7%) com lesões indicativas de endometriose no exame de alto campo, sendo que em 14 destas (77,8%) o diagnóstico também foi feito pelo exame de baixo campo. Todos os endometriomas, lesões tubárias e focos de endometriose maiores do que 7 mm identificados pelo exame de alto campo foram também identificados nos exames do baixo campo, com acurácia, sensibilidade e especificidades de 100%. Das nove pacientes avaliadas com adenomiose pelo exame do alto campo, oito foram corretamente identificadas pelo exame do baixo campo, com acurácia, sensibilidade e especificidade de 88,9%. CONCLUSÃO: A ressonância magnética de baixo campo apresentou baixa sensibilidade para detecção de pequenos focos de endometriose, alta sensibilidade para detecção de endometriomas e focos de endometriose grandes, e boa acurácia na detecção da adenomiose quando comparada com a ressonância magnética de alto campo.
