Cardiopulmonary Exercise Tests in People with Chronic Stroke: Interpretation and Clinical Application
This secondary analysis of a randomized outpatient rehabilitation trial in 250 people with chronic stroke who could walk without assistance examined why cardiopulmonary exercise tests (CPETs) were terminated, how often criteria for maximal aerobic effort were met, and how other measures contextualized CPET interpretation. CPETs were performed pre- and post-intervention, and analyses used general linear mixed methods to assess group and time effects. Only 1.9% of tests met thresholds indicating maximal aerobic effort, and despite an intervention aimed at high-intensity walking, there were no between-group differences in oxygen consumption or ventilatory threshold; significant between-group differences were instead observed in walking capacity measures. The authors conclude that, because many CPETs are not cardiorespiratorily maximally taxing in chronic stroke, interpreting results as cardiorespiratory fitness requires caution, and stop reasons may reflect biomechanical limitations rather than cardiorespiratory impairment.
