This chapter describes surgical pathology examination recommendations for ovarian biopsies and wedge resections, procedures used mainly to evaluate infertility, focusing on how to measure, describe, and submit specimens for histologic assessment. It specifies that biopsies should be submitted in their entirety and briefly described by color and texture, while wedge resections should be weighed and evaluated for features including capsule thickening, “powder burns” of endometriosis, subcortical cysts, and yellow stromal nodularity consistent with hyperthecosis. A major limitation is that the content is a specimen-handling and gross/pathology protocol rather than an original study with outcomes or diagnostic performance data, and it does not provide comparative evidence. This paper is centrally about endometriosis-related ovarian findings — it explicitly includes evaluation for “powder bums” of endometriosis in wedge resections of the ovary.

Abstract

Biopsies and wedge resections of the ovary are infrequently performed procedures that are used primarily for the evaluation of infertility. Biopsies should be measured, briefly described as to color and texture, and submitted in their entirety. Wedge resections should also be weighed and evaluated for capsule thickening, “powder bums” of endometriosis, subcortical cysts, and yellow stromal nodularity indicating hyperthecosis. Sections should be taken perpendicular to the ovarian surface to demonstrate the relationship of the capsule, cortex, and medulla. Preview Unable to display preview. Download preview PDF.
