The influence of laparoscopic resection of ovarian endometriosis on ovarian function

Abstract

Objective:To investigate the influence of suture,bipolar electrocoagulation and ultrasonic scalpel on ovarian function in laparoscopic resection of ovarian endometriosis cyst.Methods:A hundred and sixty-two patients of ovarian endometriosis were randomly divided into three groups,namely ultrasonic scalpel group,bipolar coagulation group and suture group with 54 cases in each group.Suture group:intraoperative bleeding site was sutured with absorbable suture.Ultrasonic scalpel group:ultrasonic scalpel was used to stop bleeding without other treatments.Bipolar coagulation group:in the case of visible intraoperative bleeding,the method of bipolar coagulation hemostasis and washing was used without other methods.Follicle-stimulating hormone( FSH) and anti-Mullerian hormone( AMH) levels were measured after surgery using enzyme linked immunosorbent assay,meanwhile,the antral follicle count( AFC) and peak systolic velocity( PSV) were detected by ultrasound.These data were analyzed and processed,and patients were followed up after operation.Results:The preoperative levels of FSH,AMH,AFC,PSV among 3 groups were not significantly different( P 0.05).The level of FSH measured in bipolar coagulation group and ultrasonic scalpel group was higher than that of the suture group,the difference was statistically significant( P 0.05).AMH of suture group was higher than that of bipolar coagulation group and ultrasonic scalpel group with statistical difference( P 0.05).In a month after surgery,the difference of the AFC among three groups was not statistically significant( P 0.05).In other months after surgery,the AFC of bipolar coagulation group and the ultrasonic scalpel group was lower than that of the suture group,the result was statistically significant( P 0.05).The level of PSV of suture group was higher than the other two groups,the difference was statistically significant( P 0.05).All cases in the three groups were not pregnant.Conclusions:Laparoscopic resection of ovarian endometriosis cyst using bipolar coagulation and ultrasound scalpel in the treatment of ovarian bleeding will cause irreversible damage to the ovaries,leading to decreased ovarian reserve function.
