Laying the Groundwork for Accurate Diagnosis: Eliminating Misdiagnosis Between Huntington’s Disease and Bipolar Disorder

Abstract

Huntington’s disease (HD) and bipolar disorder (BD) share overlapping psychiatric features, including mood instability, irritability, and depressive symptoms. This overlap often leads to delayed or incorrect diagnoses, inappropriate treatment, and confusion across psychiatry and neurology. This paper critically examines current diagnostic practices and their limitations in distinguishing HD from BD. It analyzes clinical symptom overlap, the central role of genetic testing in HD, and structured psychiatric and biomarker approaches for BD. Drawing on recent research, the paper proposes a dual-path diagnostic model: a neurogenetic pathway for HD and a structured psychiatric and biomarker pathway for BD. This framework aims to enhance early differentiation, reduce misdiagnosis, and support precision-based interventions. Emerging gene therapy research further underscores the importance of accurate early diagnosis, both for clinical care and for the successful application of disease-modifying treatments. Together, these strategies advocate for a future in which HD and BD are recognized as biologically distinct conditions with tailored diagnostic and therapeutic approaches.
