This case report describes a 48-year-old postmenopausal woman who presented with incidental multiple pulmonary nodules detected during preoperative evaluation. Histopathological analysis of lung biopsies revealed benign endometrial glands surrounded by stroma and smooth muscle, confirming a diagnosis of pulmonary adenomyosis, which is characterized as a rare variant of thoracic endometriosis syndrome. Given the asymptomatic nature of the patient and lack of complications, the clinical team opted for expectant management involving periodic clinical and radiological surveillance rather than immediate surgical intervention. This paper is centrally about endometriosis — specifically, it presents a rare extrapelvic manifestation known as pulmonary adenomyosis within the spectrum of thoracic endometriosis syndrome.
