Abstract

This chapter includes clinical cases, background, evidence-based practical management options, preventive measures, key-point summaries of pelvic infection after IVF and answers to questions patients ask. Transvaginal ultrasound guided oocyte retrieval (TVOR) was introduced into IVF practice in the mid 1980s, and rapidly replaced laparoscopy as the gold standard because of its simplicity, effectiveness and high patient acceptability. The incidence of post-TVOR pelvic infection (defined as pyrexia and pelvic tenderness) has been reported in different series as 0.03–1%, and that of pelvic abscess as 0.2–0.3%. Pelvic masses are also not uncommon following ovarian stimulation, and sometime laparoscopy is warranted to confirm or rule out the diagnosis. In the presence of a pelvic abscess drainage will be necessary, through laparoscopy, laparotomy or colpotomy. In infertility patients with high risk of sexually transmitted infections, consideration should be given to screening before undergoing IVF.
