Here, renal function changes in CHB patients were compared between treated with pegyIFN combined with ETV or TDF. Methods 713 patients with the combination therapy of pegIFN with ETV or TDF were included in a matched analysis comparing outcomes between cured (HBsAg < 0.05IU/mL) and uncured patients, while 104 patients received pegIFN alone as a control group for comparison with the pegIFN-ETV and pegIFN-TDF treatment groups. The primary outcome was the change in renal function. Patients were categorized into stage 1 or stage 2 based on baseline estimated glomerular filtration rate (eGFR) < 90 ml/min/m². Results In total, 125 CHB patients were matched 1:1 in both the cured and the uncured group. Renal function in the stage 1 and stage 2 groups showed a decreasing trend at 48 weeks after an initial increase. After PSM matching, the study included patients in the pegIFN-ETV or pegIFN-TDF group (n = 208, respectively), and 104 patients in the pegIFN-alone group. No significant difference in the impact of the three drug regimens on renal function at 48 weeks was found ( P = 0.955). A logistic regression model using age and baseline eGFR predicted the AUC of eGFR status at 48 weeks to be 0.851 (95% CI , 0.807, 0.895). Conclusions After 48 weeks of combined treatment in patients with CHB, neither pegIFN-TDF nor pegIFN-ETV led to an increase in renal injury. This has led to 1.5 million new infections, 296 million chronic cases, and 820,000 deaths from related liver diseases [ 1 ] . Antiviral therapy is the main treatment for CHB; however, it may not achieve HBsAg clearance, leading to lifelong treatment with drugs such as entecavir (ETV), tenofovir disoproxil fumarate (TDF) and tenofovir alafenamide fumarate (TAF) [ 2 , 3 , 4 ] . The latest guidelines recommend ETV, TDF and TAF as first-line drugs for CHB treatment. TDF may cause renal and bone safety issues, including hypophosphatemia, renal dysfunction, and osteomalacia [ 5 ] . ETV has a good safety profile, with only 0.2% of patients experiencing serious adverse reactions in a 10-year global study [ 6 ] . Multiple studies have indicated that patients with low HBsAg levels and negative HBeAg before interferon (IFN) treatment are more likely to achieve clinical cure with sequential pegIFN treatment [ 7 , 8 , 9 ] . Guidelines advise against using TDF and recommend using ETV or TAF as the preferred anti-HBV drugs for patients who have kidney disease or who are receiving renal replacement therapy [ 10 , 11 ] . Compared with TDF, ETV is considered safer for the kidneys, making it the preferred choice for patients with reduced renal function [ 12 ] . Research on renal function in CHB patients treated with pegIFN and ETV revealed that the eGFR significantly improved within 48 weeks [ 13 ] . However, no study has compared the effects of pegIFN with those of ETV and TDF on kidney function in CHB patients. Therefore, this study compared renal function changes in patients treated with pegIFN combined with ETV or TDF. 2 Methods 2.1 Study population CHB patients treated with pegIFN or combined ETV and TDF between 2018 and 2020 were included in this retrospective study. All patients were part of a larger multicentre study conducted in China [ 14 ] . The study followed clinical guidelines and ethical standards and was approved by the Medical Ethics Committee of the Third Affiliated Hospital of Sun Yat-sen University. 2.2 Data collection and follow-up Demographic, treatment, and laboratory data for patients with chronic hepatitis B (CHB) have been systematically collected from electronic medical records since the initiation of antiviral therapy (AVT). Patients were categorized into a cured group or a non-cured group based on HBsAg clearance at 48 weeks, though this classification does not represent the final treatment outcome as follow-up is ongoing. Renal function was assessed via estimated glomerular filtration rate (eGFR), serum phosphorus, and serum cystatin C. The eGFR was calculated using the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) creatinine equation. Baseline measurements of these indices were taken at enrolment, with follow-up evaluations at weeks 0, 12, 24, 36, and 48 between 2018 and 2020. 2.3 Antiviral treatment Patients were treated from the outset with either pegylated interferon (pegIFN) alone or in combination with entecavir (ETV) and tenofovir disoproxil fumarate (TDF). Clinical cure was defined as sustained hepatitis B surface antigen (HBsAg) loss after treatment cessation, with or without anti-HBs antibodies, undetectable hepatitis B virus (HBV) DNA, and normal liver enzyme levels. Serum hepatitis B e antigen (HBeAg) was semiquantified using commercial enzyme immunoassays on the Cobas e601 platform (Roche, Geneva, Switzerland), with results expressed as log10 cutoff index (COI). Plasma HBV DNA levels were quantified via real-time polymerase chain reaction (PCR) using the COBAS AmpliPrep-COBAS TaqMan HBV test (CAP-CTM; Roche Molecular Systems, Branchburg, USA), which has a lower detection limit of 20 IU/mL; results are reported as log10 IU/mL. Quantitative HBsAg (qHBsAg) concentrations in serum were measured with the Cobas e601 system (Roche, Geneva, Switzerland), which has a detection limit of 0.05 IU/mL, and are presented as log10 IU/mL. 2.4 Outcomes of renal function The research primarily found a slight decrease in the estimated glomerular filtration rate (eGFR) after 48 weeks. Liver and kidney functions were assessed using a biochemical analyzer, measuring serum alanine aminotransferase, blood creatinine, and blood urea nitrogen. According to the American Kidney Foundation's guidelines, an eGFR below 90 ml/min indicates decreased kidney function. This decrease, not meeting chronic kidney disease staging criteria, was labeled as early renal impairment. An eGFR under 90 ml/min/1.73 m² suggests mild kidney function reduction (stage 1), while 90 ml/min/1.73 m² or higher is normal (stage 2). 2.5 Statistical analysis Missing values in SPSS were imputed using the sequence average, resulting in two complete datasets. Baseline characteristics were compared across treatment groups via propensity score matching, applying a 1:1 ratio for cured versus uncured patients and a 2:2:1 ratio for patients receiving pegIFN alone, pegIFN with ETV, or pegIFN with TDF. Nearest-neighbour matching was performed with a calliper of 0.1 to ensure complete matching. Continuous variables are expressed as mean ± SD and interquartile range, while categorical variables are summarized as numbers and percentages. Multivariate and ROC curve analyses were employed to predict and evaluate renal function at 48 weeks. Binary mixed logistic regression was used to predict 48-week renal outcomes, with a significance threshold of P < 0.05. Analyses were conducted using IBM SPSS Statistics 27, GraphPad Prism 9, and web-based analysis tools. 3 Results 3.1 Baseline characteristics Based on the inclusion criteria established for the enrolled patients, a total of 625 patients diagnosed with CHB were selected for statistical analysis, comprising 278 individuals in the uncured group and 347 individuals in the cured group (Figure 1). The baseline characteristics of the patients are presented in Table 1. Prior to matching, the average age of patients with CHB in the uncured group was notably higher (43.3 years vs. 39.1 years, P < 0.001). After matching, there were no statistically significant differences in the average age or sex ratio between the CHB patients in the uncured group and those in the cured group ( P = 0.479, P = 0.054). Additionally, there were no statistically significant differences in the treatment regimen or baseline HBsAg level between the two groups ( P = 0.513, P = 0.492). Moreover, there was no statistically significant difference in baseline renal function between the two groups of CHB patients, as indicated by the estimated glomerular filtration rate (eGFR) and the inorganic phosphorus (IPHOS) and cystatin C (CysC) levels (all P > 0.05). After 12 weeks of treatment, the eGFR in the uncured group decreased, whereas the eGFR in the cured group continuously increased until the 24-week mark(Figure2),The trend of eGFR changes after 12 weeks of treatment appears to correlate well with the state of cure, which seems to be related to the patient's response to interferon, making it a key point worthy of further study. A statistically significant decrease was subsequently detected in both groups at the 48 weeks ( P < 0.05, Figure2). During the 48-week treatment period, women exhibited a greater eGFR than men did. Specifically, women exhibited an increase in the eGFR at 36 weeks, while men continued to experience a decrease. The difference between the two groups was found to be statistically significant at 48 weeks ( P < 0.001, Figure2). After initially decreasing from 0 to 12 weeks in the cured state, the IPHOS levels increased according to sex and age group. CysC levels increased up to 36 weeks and then decreased in both groups, with no significant difference between them. CysC levels were greater in males older than 40 years than in the other groups at 48 weeks ( P < 0.05,Figure2 ). 3.3 Renal function after various treatment regimens There were no statistically significant differences in the age or sex distributions among patients receiving pegIFN in combination with ETV, pegIFN in combination with TDF, or pegIFN monotherapy. The analysis of eGFR levels indicated that the three treatment regimens exhibited similar patterns of change over the 48 weeks treatment period, with no statistically significant differences observed (all P > 0.05,Table2). Similarly, the levels of IPHOS demonstrated consistent trends among the three treatment groups throughout the 48 weeks treatment duration, with no statistically significant differences at 0, 12, 24, or 36 weeks (all P > 0.05,Table2). However, a statistically significant difference was noted at the 48 weeks mark ( P = 0.013,Table2,). The levels of CysC indicated that the three treatment regimens exhibited a consistent pattern of change over a 48 weeks treatment period, with no statistically significant differences observed at 0, 12, 24, or 48 weeks (all P > 0.05,Table2). Participants were divided into stage 1 and stage 2 groups based on their baseline renal function. The renal function in both groups increased and then decreased at 48 weeks, with higher eGFR in the pegIFN-TDF group than in the other two groups. There was no significant difference in renal function between the stage 1 and stage 2 groups; however, there was a significant difference in the eGFR at 12, 24, and 36 weeks in the stage 2 group (P < 0.05,Figure3).In addition, we also analyzed the changes in renal function among the three treatment regimens for the two staged patients, and the results showed that there was no significant difference in the changes in renal function among the three treatment regimens at 12, 24, 36 and 48 weeks( F igureS3 ). 3.5 Efficacy test of predictors for renal outcome Four factors were chosen for predictive testing based on LASSO regression and multivariate analysis( FigureS4 ). The first two factors, baseline eGFR and age, were selected using the AUC. Logistic regression was used to predict the eGFR at 48 weeks in patients. The area under the receiver operating characteristic curve (AUC) of the prediction model was 0.851 (95% CI : 0.807, 0.895,Figure4 and Table3). At a prediction probability threshold of 0.853, the Youden index was maximized for patients with normal renal function at 48 weeks of treatment, yielding a sensitivity and specificity of 0.781 each. We created nomograms and analysed a calibration curve to evaluate the impact of sex, age, cure status, and eGFR on renal function after 48 weeks of treatment. The calibration curve had a C index of 0.729, indicating moderate accuracy. Table3. By augmenting the remission rates of CHB and diminishing the population of hepatitis B carriers, this study underscores the imperative for the nation to mitigate "morbidity and mortality." The establishment of a substantial cohort receiving pegIFN treatment for CHB patients aims to identify optimal patient profiles and treatment protocols conducive to clinical remission through the analysis of extensive datasets, thereby facilitating their widespread implementation. Doctors' ability to treat CHB should be enhanced through specialized training. The present study, in specifically examining renal function injury, offers long-term renal function safety data regarding patients receiving pegIFN or oral antiviral drugs. These data are crucial for understanding the impact of drugs on renal function in clinical trials and cohort studies. Patients who were clinically cured at 48 weeks had better renal function than those who were not cured, indicating a better response to pegIFN and potential benefits in terms of renal function. This finding aligns with those of previous studies suggesting that pegIFN therapy may improve the eGFR in patients with CHB [13] . There was no notable difference in renal function outcomes after 48 weeks between patients treated with pegIFN combined with ETV and those treated with TDF, suggesting that oral antiviral drugs may be a suitable choice for CHB patients receiving combined pegIFN therapy. First-line nucleoside drugs are primarily excreted in urine, with TDF showing dose-dependent nephrotoxicity in both animal and human studies. TDF is a water-soluble diester prodrug of tenofovir with a diphenylfuroate group, but its poor stability and low utilization are due to its short half-life and inability to selectively decompose and release in the liver [15] . As TDF is taken for longer periods, the number of reported kidney side effects is increasing [16] . Koklu et al. reported that TDF is more effective at reducing the eGFR than other nucleoside drugs [17] . Observational studies suggest that TDF may increase the risk of renal function injury, especially in older or menopausal patients [18] . There is still debate about treating renal function with pegIFN alone or in combination with oral antiviral drugs. Numerous studies have demonstrated that pegIFN can enhance kidney function in patients with renal insufficiency and transplants [19,20,21,22] . The exact mechanism by which pegIFN protects the kidneys is still unknown. This study suggested that the high content and the shape of pegIFN may slow drug excretion in the kidneys, potentially protecting them. Studies have shown that lower levels of peripheral CD 4 CXCR 5 follicular helper T cells are linked to higher eGFRs, indicating a potential role in improving the eGFR through pegIFN [23] . There are limited studies on how pegIFN or combined oral antiviral drugs affect patients’ kidney function. Previous research revealed [24] no significant difference in the average eGFR between TDF and ETV in patients with normal renal function. However, in patients with moderate renal impairment, the average eGFR was lower in the TDF group and decreased during treatment. This study also revealed that viral inhibition could improve renal function to some extent, potentially explaining why combined treatments did not significantly affect renal function. Our study offers clinicians a more precise selection of antiviral drug regimens by using propensity score matching to balance patient characteristics. The findings from the observation of renal function in two groups, one of which was uncured and the other cured, undergoing a 48-week treatment with pegIFN combined with oral antiviral drugs indicated that the renal function of the cured group was superior to that of the uncured group.In addition, our study also analyzed the correlation analysis of ALT and eGFR levels in patients, and the results also revealed that with the emergence of ALT firelight eGFR levels appeared to be elevated, and more than suggestive of the relevance of the changes in eGFR in the biochemical response of the patients, and behind it may be due to the clearance of HBV in the patient's body( FigureS2 ). Furthermore, the similar change trend observed in both groups indirectly suggests a potential association between the response to pegIFN and renal function outcomes in patients. Subsequently, propensity score matching was employed to mitigate any imbalances in clinical characteristics among the three cohorts receiving pegIFN alone, pegIFN in combined with ETV, and pegIFN in combined with TDF. The study revealed no discernible differences in the impact of the three treatment protocols on renal function outcomes after a 48 weeks period. Furthermore, these findings suggest that TDF offers a wider range of treatment options for combination therapy than ETV or oral antiviral drugs alone. Additionally, the high quality of the study cohort allowed for the accurate prediction of renal function outcomes at 48 weeks. Age and baseline eGFR were identified as significant predictors for assessing renal outcomes, underscoring the importance for clinicians to promptly monitor renal function in older patients or those with poor baseline renal function.Then, our study also analyzed the healing status, age subgroups and gender of patients as well as renal function staging among the three treatment regimens, but no significant differences were seen, thus, our study further established our conclusion that there was no difference in the effect of ETV and TDF on renal function of patients over the combination of pegIFN ( TableS4,FigureS1 ). This study is subject to certain limitations. The disease type prevalent in the study cohort is predominantly singular, thus limiting its ability to accurately represent the complexities present in patients with renal complications. Additionally, the duration of observation in this study was relatively short, suggesting that further research with an extended observation period may be necessary for comprehensive validation. at 12 weeks of treatment among women and patients under 40 years of age in the cured group. After 12 weeks of treatment, the eGFR in the uncured group decreased, whereas the eGFR in the cured group continuously increased until the 24-week mark(Figure2),The trend of eGFR changes after 12 weeks of treatment appears to correlate well with the state of cure, which seems to be related to the patient\u0026apos;s response to interferon, making it a key point worthy of further study. A statistically significant decrease was subsequently detected in both groups at the 48 weeks (\u003cem\u003eP\u003c/em\u003e \u0026lt; 0.05, Figure2). Studies have shown that lower levels of peripheral CD\u003csup\u003e4\u003c/sup\u003eCXCR\u003csup\u003e5\u003c/sup\u003e follicular helper T cells are linked to higher eGFRs, indicating a potential role in improving the eGFR through pegIFN\u003csup\u003e[23]\u003c/sup\u003e. There are limited studies on how pegIFN or combined oral antiviral drugs affect patients’ kidney function. Previous research revealed\u003csup\u003e[24]\u003c/sup\u003e no significant difference in the average eGFR between TDF and ETV in patients with normal renal function. However, in patients with moderate renal impairment, the average eGFR was lower in the TDF group and decreased during treatment. This study also revealed that viral inhibition could improve renal function to some extent, potentially explaining why combined treatments did not significantly affect renal function.\u003c/p\u003e\n\u003cp\u003eOur study offers clinicians a more precise selection of antiviral drug regimens by using propensity score matching to balance patient characteristics. The findings from the observation of renal function in two groups, one of which was uncured and the other cured, undergoing a 48-week treatment with pegIFN combined with oral antiviral drugs indicated that the renal function of the cured group was superior to that of the uncured group.In addition, our study also analyzed the correlation analysis of ALT and eGFR levels in patients, and the results also revealed that with the emergence of ALT firelight eGFR levels appeared to be elevated, and more than suggestive of the relevance of the changes in eGFR in the biochemical response of the patients, and behind it may be due to the clearance of HBV in the patient's body(\u003cem\u003eFigureS2\u003c/em\u003e). Furthermore, the similar change trend observed in both groups indirectly suggests a potential association between the response to pegIFN and renal function outcomes in patients.\u003c/p\u003e\n\u003cp\u003eSubsequently, propensity score matching was employed to mitigate any imbalances in clinical characteristics among the three cohorts receiving pegIFN alone, pegIFN in combined with ETV, and pegIFN in combined with TDF. The study revealed no discernible differences in the impact of the three treatment protocols on renal function outcomes after a 48 weeks period. Furthermore, these findings suggest that TDF offers a wider range of treatment options for combination therapy than ETV or oral antiviral drugs alone. Additionally, the high quality of the study cohort allowed for the accurate prediction of renal function outcomes at 48 weeks. Age and baseline eGFR were identified as significant predictors for assessing renal outcomes, underscoring the importance for clinicians to promptly monitor renal function in older patients or those with poor baseline renal function.Then, our study also analyzed the healing status, age subgroups and gender of patients as well as renal function staging among the three treatment regimens, but no significant differences were seen, thus, our study further established our conclusion that there was no difference in the effect of ETV and TDF on renal function of patients over the combination of pegIFN (\u003cem\u003eTableS4,FigureS1\u003c/em\u003e).\u003c/p\u003e\n\u003cp\u003eThis study is subject to certain limitations.
