Here, we report a rare case of a 12-year-old girl with dysgerminoma with malignant hypercalcemia at diagnosis. She was managed with endocrinologist close monitoring with hydration. Oral steroid, calcitonin and Zoledronic acid. She showed a complete response on initiation of chemotherapy and underwent delayed excision after 4 cycles, unlike previously reported cases.
