Xoắn u nang lạc nội mạc tử cung ở buồng trứng là một thể bệnh rất hiếm gặp, xuất hiện ở phụ nữ trẻ là chủ yếu. Chẩn đoán còn gặp rất nhiều khó khăn do triệu chứng lâm sàng có sự chồng lấp giữa bệnh lý lạc nội mạc tử cung và xoắn phần phụ. Chúng tôi báo cáo 1 trường hợp bệnh nhân nữ, 20 tuổi, chưa quan hệ tình dục, đau hố chậu phải kèm buồn nôn, trên siêu âm có nang lạc nội mạc tử cung buồng trứng phải phù nề, whirlpool (+). Bệnh nhân được phẫu thuật nội soi tháo xoắn và bóc u nang buồng trứng. Kết quả giải phẫu bệnh: nang lạc nội mạc tử cung ở buồng trứng phải. Abstract Ovarian torsion with endometrioma is rare but especially occurs in young women. Because of the alike symptoms between endometrioma and ovarian torsion, the diagnosis is challenging. We report a 20 - year - old, virgin woman with right lower pelvic pain. Abdominal ultrasonography showed an endometrioma cyst with edema tissue, a whirlpool sign of twisted vascular pedicle. The patient underwent a laparoscopy to retwist right ovarian torsion. Histopathological results a confirmed ovarian endometrioma.
